Zowel de Endoscopische submucosal dissectie (ESD) en de Endoscopische mucosale resectie (EMR) zijn momenteel voorkomende behandelingen voor beginnende gastro-intestinale kanker1,2. Een submucosal injectie materiaal (SIM) injecteren in de submucosa is een van de belangrijkste stappen voor zowel de EMR en ESD procedures2,3. Grote submucosal hoogte en onderhoud submucosal hoogte zijn essentiële criteria voor het veilig uitvoeren van EMR/ESD.
Hoewel normale zout (NS) heeft gebruikt als een SIM-kaart sinds de uitvinding van Endoscopische therapie4,5, Natrium hyaluronaat (HA) werd geïntroduceerd als een behandeling in de afgelopen jaren6,7. HA werd op grote schaal gebruikt in Endoscopische behandelingen als een superieure SIM toe te schrijven aan zijn hoge prestaties8,9,10,11. Op dit moment een prestatiesvergelijking tussen de bestaande SIMs werd uitgevoerd, en krachtige SIMs werden ontwikkeld om het identificeren van een andere superieure SIM-5,12,13,14, 15,16,17,18.
De ex vivo -model met behulp van een monster van de varkens maag is gebruikt voor het evalueren van de prestaties van de SIM, omdat de schatting van de prestaties van de SIM in de menselijke maag-darmkanaal zeer moeilijk19,20,21 is , 22. dit model conventionele ex vivo is echter zeer eenvoudig, en heeft de ruimte voor verbetering. Een omgeving dichter naar de menselijke gastro-intestinale mucosa te reproduceren kan nauwkeurige evaluatie van de prestaties van de SIM.
In onze eerdere studie ontwikkelden we een nieuwe ex vivo model dat kan worden gebruikt voor het evalueren van de werking van verschillende SIMs in detail door de constante spanning op de uiteinden van het model toe te passen. Wij ook bevestigd dat de voorgestelde nieuwe ex vivo model, toelaat nauwkeurige meting van de ze onder uniforme voorwaarden en een gedetailleerde vergelijking van de prestaties van verschillende soorten SIMs23.
In deze studie, presenteren we een volledige weergave van het nieuwe model ex vivo en de gedetailleerde installatie-methodologie van het nieuwe model ex vivo wordt uitgelegd in detail met behulp van video's en cijfers. Het nieuwe model ex vivo bestaat uit onderdelen die gemakkelijk beschikbaar zijn en kan snel worden ingesteld. Beschrijvingen van gedetailleerde setup methodologie zal bijdragen tot de verspreiding van het nieuwe model.

Verhogen van de prestaties van submucosal injectie materialen (SIMs) is belangrijk voor Endoscopische therapie van vroege gastro-intestinale kanker. Het is essentieel om een ex vivo -model dat SIM prestaties nauwkeurig, voor het ontwikkelen van hoogwaardige SIMs kan evalueren. In onze eerdere studie ontwikkelden we een nieuwe ex vivo model dat kan worden gebruikt voor het evalueren van de werking van verschillende SIMs in detail door de constante spanning op de uiteinden van het model toe te passen. A New Ex Vivo Model for the Evaluation of Endoscopic Submucosal Injection Material Performance

Deze methode kan helpen bij het beantwoorden van belangrijke vragen in de biomedische gebied over medische kwestie van belang. Het belangrijkste voordeel van deze techniek is dat de prestaties van verschillende submucosale injectiezaken nauwkeuriger kunnen worden ontwijken dan conventionele methoden. Begin met het snijden van zes bij zes centimeter vierkanten van het bovenste derde deel van een varkensmaag en onmiddellijk opslaan van de exemplaren op min 30 graden Celsius.Om het tractieapparaat in te stellen, sluit u een roestvrijstalen clip aan op een toetsdraad en een S-vormige haak en sluit u de toetsdraad en haak aan op een gewicht. Sluit de haak aan op het andere uiteinde van de draad en bevestig katrollen aan beide uiteinden van de basis. Bevestig vervolgens de scriber op de hoogte van het slijmvliesoppervlak en druk vooraf op om een nulpuntaanpassing van de hoogtemeter uit te voeren. Gebruik voor het uitvoeren van een submucosale hoogteprocedure een spuit van 2,5 milliliter uitgerust met een naald van 23 meter om twee milliliter natriumhyaluronaat van 0,4% te injecteren, horizontaal in de submucosa van de monstermarges.Gebruik vervolgens de digitale meter om de submucosale hoogte onmiddellijk en op de aangegeven tijdstippen na de injectie te meten. Het registreren van de hoogte weergegeven op de hoogte meter, bij de vaststelling van de scriber aan de bovenkant van de submucosale hoogte. In deze representatieve experimenten werd de submucosale hoogte gemeten na normale zoutoplossing, of hyaluronate zuurinjectie, met behulp van de conventionele en nieuwe ex vivo-analysemodellen.Hoewel de submucosale hoogte na de injectie van beide oplossingen na verloop van tijd afnam, waren in beide modellen de resulterende submucosale hoogten, na hyaluronate zuurinjectie, constant hoger dan die welke na de normale zoutoplossing werden gemeten, ongeacht de post-injectietijd. Met name de toepassing van een constante spanning, resulteerde in een lagere algehele variabiliteit binnen de metingen, in vergelijking met de no-tension conventionele model van de meting. Inderdaad de submucosale hoogten verkregen bij een spanning van 3,0 Newton, waren aanzienlijk lager dan de submucosale hoogten verkregen op 1,5 newton.Terwijl er geen significante verschillen in submucosale hoogten werden waargenomen met behulp van het conventionele model, in vergelijking met het model met constante spanning. Na de ontwikkeling maakt deze techniek de weg vrij voor onderzoekers op het gebied van biomedische zaken, om het gebruik van hoogwaardige submucosale injectiematerialen voor endoscopische therapie uit te drukken.
