急性右心不全(ARHF)は、最近、右心室(RV)充填および/または減少したRV流量出力に起因する全身性鬱血との急速進行性症候群として定義されている。ARHFは、左の心不全、急性肺塞栓症、急性心筋梗塞、肺高血圧症(PH)などのいくつかの条件で発生する可能性があります。PHの場合、ARHF発症は、短期死亡または緊急肺移植の40%のリスクに関連している2,3,4。ここでは、慢性肺高血圧症の設定におけるARHFの大型動物モデルの作成方法と、心エコー検査と気圧容積ループを用いて右心室を評価する方法について説明する。
ARHFの病態生理学的特徴は、RV圧力過負荷、体積過負荷、RV出力の減少、中心静脈圧の増加および/または全身圧の低下を含む。慢性PHでは、肺血管抵抗の増加にもかかわらず心拍出量を維持することを可能にするRV収縮性の初期増加がある。したがって、慢性PH上のARHFの文脈では、右心室は、特に向知性支持下で、ほぼ等体性圧力を生成することができる。一緒に取ると、慢性PHのARHFとイノトロープによる血行性回復は、最近の大型動物モデル5に記載されているように、急性RV虚血病変の発症につながります。イトロープの増加は、虚血性病変をさらに発症し、最終的に末期臓器機能不全および悪い臨床転帰の発症につながる可能性のあるエネルギー需要の増加を生み出す。しかし、主に体液管理、イノトロープ、体外循環サポートの役割に関して、PH上のARHF患者を管理する方法についてのコンセンサスはありません。その結果、急性右心不全の大型動物モデルは、ARHF臨床管理に関する前臨床データを提供するのに役立つ可能性がある。
治療に対する応答を定量化する第一歩として、右心室を表現する簡単で再現可能な方法が必要である。現在までに、ARHF患者のRV形態と機能をより良く表現する方法についてのコンセンサスはありません。RV収縮性(すなわち、収縮する本質的な能力)および心室-動脈結合(すなわち、心室後負荷によって正規化された収縮性;心室適応の指標)を評価するための金標準法は、圧力体積(PV)ループの分析である。この方法は、右心カテーテル法と、下の静脈に挿入されたバルーンを用いたRVプリロードの一過性の減少を必要とするため、2回侵襲的である。臨床現場では、右心室を評価するための非侵襲的かつ反復可能な方法が必要である。心臓磁気共鳴(CMR)は、右心室の非侵襲的評価のためのゴールドスタンダードと考えられています。集中治療室(ICU)で管理されている慢性PHのARHF患者では、CMRの使用は患者の不安定な血行力学的状態のために制限されるかもしれません。また、CMR評価を繰り返し、夜間を含め、1日数回、そのコストおよび限られた利用可能性のために制限され得る。逆に、心エコー検査は、ICU患者における非侵襲的、再現性、低コストのRV形態および機能評価を可能にする。
大型動物モデルは、侵襲的な血行力学パラメータと非侵襲的パラメータとの関係に焦点を当てた前臨床試験を行うのに理想的です。大きな白豚の解剖学は人間に近いです。その結果、ヒトに記述された心エコーパラメータのほとんどは、ブタで定量化可能である。心エコー研究のために考慮しなければならない人間と豚の心臓の間にいくつかの小さなバリエーションが存在します。.豚は、憲法上のデキストロカルディアと心臓軸のわずかに反時計回りの回転を提示します。その結果、アピカル4チャンバービューは、アピカル5チャンバービューになり、音響窓は、xiphoid付録の下に配置されます。さらに、パラスターナル長軸と短軸の眺め音響窓は胸骨の右側に位置しています。
ここでは、慢性血栓塞栓性PHの大型動物モデルでARHFを誘導し、ドブタミンを用いて血行力学的に回復させる新しい方法について述べている。また、ドブタミンによる血行力学的修復後2〜3時間以内にモデル内に存在するRV虚血性病変を報告する。また、各条件でのRV PVループおよび心エコーRVパラメータを取得して、RVの形態や機能の動的変化に関する洞察を得る方法を説明する。慢性血栓塞栓性PHおよびPVループ法の大型動物モデルとして、先に説明した6、これらのセクションについて簡単に説明する。また、ブタモデルでは難しいと考えられる心エコー評価の結果も報告した。モデル内で繰り返し心エコーを実現する方法について説明します。
この研究で報告された慢性PHのARHFのモデルは、異なる治療戦略を比較するために使用することができる。RV表現型の方法は、急性肺塞栓症7、RV心筋梗塞8、急性呼吸窮迫症候群または 左心室障害に関連する右心不全10 または左心室機械的循環サポート11のような臨床的に関連する状況を模倣する他の大型動物モデルに使用することができる。

冠状動脈性肺高血圧症の文脈における急性右心不全のこのモデルは、この臨床的に関連する状況の病態生理をよりよく理解するのに役立ちます。体積および圧力過多の平均による急性右心不全の誘導は容易に再現可能であり、対応する臨床状況の主要な病態生理学的側面を複製する。適切なレベルの沈下を確認した後、セルディンガー法を用いて、頸静脈を通って上の静脈カヴァまで中央静脈カテーテルを配置する。次に、鼠径部に4センチメートルの横切り切開を行い、ベックマンレトラクタを切開部に挿入します。デバキー鉗子とメッツェンバウムはさみを使用して、大腿静脈と動脈の前面を分割し、20ゲージのカテーテルを大腿動脈に入れる。次に、カテーテルを液体で満たされたカテーテルで使い捨てトランスデューサに接続し、全身および中央静脈血圧を得ます。18ゲージのカテーテルとCアームとインテリア後部ビューを備えた蛍光鏡を使用して、下の大腿静脈にガイドワイヤーを挿入します。気球拡張カテーテルをガイドワイヤーの上に腹腔内レベルに置き、バルーンの可視マーカーをダイヤフラムレベルのすぐ上に置きます。次にガイドワイヤーを取り外します。カテーテルが配置された直後に、2次元で、組織ドップラーモードで、シフォイドプロセス下のアペカル5チャンバービューを取得する。2Dおよび組織ドップラーモードで胸骨の右側にあるパラスターナル短軸および長軸図と、連続およびパルスドップラーモードを使用した弁膜流れの画像を取得します。次に、側側三尖環と外側および中隔の僧帽弁環の組織ドップラー信号を取得する。右心のカテーテルのために、頸静脈に挿入された8フランスのシースに白鳥ガンツカテーテルを導入し、平均右心房、右心室、および肺動脈圧を取得する。次に、同時に打撃の容積計算のための心拍数を測定しながら、製造業者の指示に従って熱希釈方法で心拍出量を測定する。液体充填カテーテルに由来する圧力のライブ取得のために、圧力ボリュームループワークステーションに使い捨てトランスデューサを接続し、右心室にコンダクタンスカテーテルを導入するために、蛍光法を使用します。次に、圧力量ループのライブ取得に使用する品質信号を検証し、定常状態で、および終了の間に下の静脈に挿入されたバルーンを膨脹させることによって誘発される急激な前負荷低減の間に圧力体積ループファミリーを獲得する。急性右心不全を誘発するには、まずフリーフロー注入出力を使用して、1キログラムの生理食塩水あたり15ミリリットルを開始する。血行性安定化の5分後、各注入の終わりに、右心カテーテル、圧力体積ループ、心エコー測定を取得します。次に、測定終了直後に1キログラム当たり15ミリリットルの2回目の注入を開始し、2回目の測定セット終了直後に生理食塩水1キログラム当たり30ミリリットルの3回目の注入を開始します。血行圧過多を誘発するには、フルオロスコープを使用して、頸部シースを通して5つのフランスの血管造影カテーテルを右下葉肺動脈に挿入する。右下葉肺動脈をn-ブチル-2-シアノアクリレート脂質造影色素を含む150マイクロリットルのボーラスで塞栓し、ボーラスが送達されたら10ミリリットルの生理食塩水でカテーテルを洗い流す。塞栓術の2分後、全身および肺動脈圧を測定して血行力学的応答を評価し、血行力学的妥協が達成されるまで2分ごとにボーラス送達を繰り返す。血行力学的妥協に達した後、1分あたり2.5マイクログラムでドブタミン注入を開始する前に、圧力体積ループと心エコー測定を取得します。右心カテーテル法、圧力体積ループ、心エコー測定を行う前に、血力学的安定化のために10〜15分待ちます。すべての測定が取得されたら、1キログラム当たり5マイクログラムにドブタミン注入量を増加させ、右心カテーテル法、圧力体積ループ、および血行力学的安定化が達成された後の心エコー測定を繰り返します。その後、1分間に1キログラムあたり7.5マイクログラムにドブタミン注入量を増やす。急性の容積負荷は、急性右心不全を誘発せず、むしろ慢性肺高血圧モデルの適応表現型を強調する。容積負荷により、心拍出量は右心房圧の増加なしに増加し、心室と動脈の結合が安定したままである。血行力学的妥協は、心拍出量、脳卒中量、および心室と動脈の結合の有意な減少に関連しているのに対し、右心室収縮は安定したままであった。右心房圧の2倍の増加および平均肺動脈圧も典型的には観察される。実証されたようにドブタミンの送達は正常範囲内の心拍出量、打撃容積、および心室と動脈の結合を回復する。心エコー検査は、実験全体を通して右心室のサイズと機能の動的変化を定量化するために使用することができる。圧力量ループ解析により、右心室末端-収縮期エラスチンと心室-動脈結合の動的定量化が可能になります。本代表的研究では、急性肺塞栓症の直後に起きた2人の死亡は、右心腔の急性血栓症と関連していた。ヘマトキシリン・エオジンおよびサフラン染色後、ピノスティック核を有する高等球性心筋炎のクラスターを特徴とする右心室虚血病変は、右心室遊離壁の心膜下及び心膜下層で観察することができる。肺高血圧症の状況において右心室の急性右心不全表現型を誘発するために必要な急性体積および圧力過負荷の大きさ。血行力学的回復は、最良の治療選択肢を決定するために薬物または介入によって使用することができる。
