Siden Salmonella spp. /Shigella spp. er ofte forbundet med matforgiftning og fecal-muntlige overføring av akutt gastroenteritt28,29 og rutinemessig metoden er arbeidskrevende eller tidkrevende 7, beskriver vi en høy gjennomstrømming plattform for Salmonella spp. /Shigella spp. screening, bruker sanntid PCR kombinert med guidede kultur.

Diaré er fortsatt en vanlig sunnhet problem med en høy forekomst rate globalt1,2. Om dødeligheten er relativt lav, noen pasienter viser ulike symptomer i uker (for eksempel løs og vannaktig avføring, et haster å gå på do), som gjør sosioøkonomiske innvirkning svært høy3,4. Mer alvorlig, noen pasienter kan selv utvikle irritabel tarm hvis venstre ubehandlet5. Det finnes ulike typer bakterier, virus og parasitter som kan forårsake diaré6. Salmonella spp. /Shigella spp. er blant de vanligste bakteriene for overføring av akutt gastroenteritt,7,,8,,9,,10,,11. Derfor mange fylker har utstedt lover eller forskrifter for vanlige Salmonella spp. /Shigella spp. screening blant folk som skulle håndtere mat og vann. For eksempel den kinesiske regjeringen har utstedt lover for obligatoriske Salmonella spp. /Shigella spp. screening en gang i året.
Metoden gullstandarden for Salmonella spp. /Shigella spp. gjenkjenning er bakterier kultur. Gjennom bakterier kultur og påfølgende biokjemisk identifisering og serologisk karakterisering, kan vi identifisere arter av bakterier, som kan lette sykdom utbrudd ledelse og antimikrobielle profilering hjelp behandlingen av pasienter 12. det kan også hjelpe spore smittekilden under Salmonella spp. /Shigella spp. utbruddet13. Denne metoden er imidlertid arbeidskrevende (krever manuell drift) og tidkrevende (ta flere dager), spesielt for testing av store antall prøver7. Videre levedyktig men ikke-culturable (VBNC) Salmonella spp. /Shigella spp.kan finnes i noen avføringsprøver14. I lys av disse ulempene, mange laboratorier har forsøkt å utvikle nye teknikker for påvisning av Salmonella spp. /Shigella spp.15,16,17,18 , 19 , 20 , 21 , 22 , 23 , 24 , 25. alle disse metodene bruk kjernefysiske syre forsterkning testen (NAAT), blant annet polymerasekjedereaksjons (PCR) er den vanligste. Ettall større begrensningen av metodene NAAT basert er at døde bakterier, selv bakteriell rusk som inneholder ufullstendig genomisk DNA, kan positive resultater26, som kan i stor grad påvirke den nøyaktig diagnosen av sykdommen. Blanco et al. viste at molekylær analysen er svært følsom, ikke bare levedyktig Salmonella i kulturer, men også delvis genomer og død eller unviable bakterier fra siste infeksjoner eller forurensning26. Derfor bør ny teknologi utvikles.
Her beskrev vi en roman metode som kombinerer NAAT basert metode og dyrking. Som vist i figur 1, denne nye metoden gjelder sanntid PCR screening først og deretter positiv eksemplene sendes for bakterier kultur og identifikasjon.

Hvis sanntid PCR resultatet er positiv, så den andre fasen (guidet kultur) utføres: selektiv kultur, biokjemisk identifisering og serologisk karakterisering. Vi illustrerer også representant resultatene fra den. Denne metoden gir en høy gjennomstrømningsplattform for screening av Salmonella og Shigella. Den største fordelen med denne teknikken er gjenta, spesifikk, følsom og høy gjennomstrømning. Denne teknikken kombinerer NAAT rask metode og kultur der sanntids PCR screening ble brukt først, og deretter positive ble sendt for bakterier kultur og identifikasjon.Selv om denne metoden fokuserer på Salmonella og Shigella screening, kan den også brukes på andre patogener som Vibrios, Staphylococcus, E.Coli og et cetera. Visuell demonstrasjon av denne metoden er kritisk da valg av riktig koloni er vanskelig på grunn av bakgrunnsflora i prøver. Videre sanntid PCR indikerte at utvalg to var positivt for Salmonella og ble valgt for guidet Salmonella arter kultur.I den guidede kulturen til Salmonella-arter ble de rosa og lilla koloniene belagt separat og utsatt for biokjemisk identifikasjon. Resultatene indikerte at koloniene fra prøve to var Salmonella paratyphi B.Using real-time PCR, det var vellykket forsterkning i FAM-kanalen, noe som indikerer at prøven var positiv for Shigella arter. Videre sanntid PCR indikerte at utvalg 10 var positivt for Shigella og ble valgt for guidet kultur.I den guidede kulturen til Shigella-arter ble de rosa-røde og fargeløse koloniene belagt separat og utsatt for biokjemisk identifikasjon. Resultatene indikerte at prøve 10 inneholdt Shigella sonnei fase II bakterier. Mens du prøver denne prosedyren, er det viktig å huske begrensningen av metoden på grunn av sekvensvariasjon.Etter denne prosedyren kan andre metoder, som direkte kultur, utføres for å unngå falske negative real-time PCR-resultater. Denne teknikken baner vei for forskere innen Salmonella og Shigella deteksjon, da den nye protokollen kan øke den positive hastigheten med toganger og redusere arbeidsmengden og TAT betydelig. Ikke glem at det å jobbe med Salmonella og Shigella kan være ekstremt farlig, og det personlige verneutstyret, PPE, bør alltid tas mens du utfører denne prosedyren.
