Urethrale strictuur reparatie door vaartuig-sparend excisie en primaire anastomose werd het aanvankelijk uitgevoerd in het Universitair Ziekenhuis Gent in 2010. Daarna werd het een norm voor een praktijk in het ziekenhuis beheer van patiënten met geïsoleerde korte bulbaire of posterieure urethrale stricturen. Als de lamp blijft gekoppeld aan de perineale lichaam, toegang tot de achterste urethra en de resectie van het fibrotische weefsel op die site kunnen worden aangetast. Urethroplasty wordt beschouwd als de standaardbehandeling voor urethrale stricturen, want het biedt uitstekende langdurige succes tarieven1,2. Een talrijke hoeveelheid operatietechnieken is beschreven, uitdagend de reconstructieve uroloog om te kiezen van de beste aanpak, overwegen verschillende strictuur modaliteiten zoals het aantal stricturen, de strictuur lengte, de strictuur locatie, de etiologie, comorbidities en eerdere urethrale interventies. Voor geïsoleerde korte bulbaire urethrale stricturen raadt het internationale overleg over urologische aandoeningen (ICUD) urethroplasty door excisie en primaire anastomose (EPA) die is gekoppeld aan een samengestelde slagingspercentage van 93,8%3,4.
Urethroplasty door EPA belichaamt een benadering waarin het hele zieke segment van de urethra is verwijderd en vervangen door gezonde aangrenzende urinebuis zonder de noodzaak van protheses of kleppen om de kloof te overbruggen. Traditioneel, omvatte deze aanpak de volledige dikte transect van het corpus spongiosum op het niveau van de strictuur5. Als EPA alleen de besnijdenis van de vernauwde segment en de omringende spongiofibrosis vereist, is een volledige dikte transect van het corpus spongiosum, en de dubbele urethrale bloedtoevoer daarin echter meestal niet nodig. Gezien deze achtergrond, introduceerde Jordan et al. het idee van een vaartuig-sparend aanpak in 2007, het aanbieden van een niet-transecting alternatief voor de klassieke transecting EPA (tEPA)6,7. Dit vaartuig-sparend EPA (vsEPA) heeft sinds zijn opstand en verschillende centra hebben — hoewel enigszins gewijzigd — deze techniek toegepast in hun chirurgische repertoire8,9,10,11 ,12.
De techniek van schip-sparend streeft naar een vermindering van chirurgische trauma, met name aan de dubbele arteriële bloedvoorziening van de urinebuis ingebed in het corpus spongiosum. Het behoud van de bulbaire slagaders vermindert mogelijk het risico van postoperatieve erectiele storing of glans ischemie. Bovendien zou het nuttig zijn voor latere urethrale interventies zoals opnieuw urethroplasty met behulp van een gratis graft, waarin een goed gevacuoliseerd transplantaat bed noodzakelijk8,9 is. Deze potentiële voordelen zijn echter alleen aannames zoals prospectieve studies vergelijken de functionele uitkomst van beide technieken met gevalideerde vragenlijsten momenteel ontbreken.
Zo belangrijk als de functionele uitkomst is, vsEPA ten minste moet kunnen bieden soortgelijke chirurgische resultaten als tEPA. 3,4,8,9 belooft op korte termijn resultaten zijn gepubliceerd en zijn in overeenstemming met de slagingspercentages gerapporteerd door de ICUD, maar een directe vergelijking tussen beide technieken tot nu toe niet is uitgevoerd ,10,11,12.
Bekken fractuur-gerelateerde urethrale verwondingen worden geassocieerd met littekenvorming en een daaropvolgende urethrale strictuur of volledige vernietiging van membraanachtig urethra. Posterieure stricturen misschien ook ontwikkelen na chirurgie of bestraling voor de prostaat13. Voor deze stricturen, urethroplasty met de besnijdenis van het littekenweefsel en Glover-prostaat wapendrager verdient ook14. Traditioneel, werden de bulbaire slagaders afgebonden tijdens deze procedure als niet reeds uitgewist door de bekken breuk. Om dit te vermijden, is een vaartuig-sparend variant geïntroduceerd en gerapporteerde evenals15,16.
Het doel van deze paper is een uitgebreide, stapsgewijze overzicht van het beheren van patiënten met geïsoleerde korte bulbaire of posterieure urethrale stricturen met vsEPA te geven. Het belangrijkste toepassingsgebied is te schetsen en visualiseren van de operatietechniek en het verslag van het resultaat van de representatieve chirurgie. Een evaluatie van de functionele uitkomst parameters valt buiten het bestek van dit document.

Urethroplasty wordt beschouwd als de standaardbehandeling voor urethrale stricturen omdat dit uitstekende langdurige slagingspercentages biedt. Voor geïsoleerde korte bulbaire of posterieure urethrale stricturen, wordt urethroplasty door excisie en primaire anastomose (EPA) aanbevolen. Als EPA alleen de besnijdenis van de vernauwde segment en de omringende spongiofibrosis vereist, is een volledig-dikte transect van het corpus spongiosum, zoals uitgevoerd in de traditionele transecting EPA (tEPA), meestal niet nodig. Jordanië et al. introduceerde het idee van een vaartuig-sparend aanpak in 2007, gericht op het verminderen van chirurgische trauma, met name aan de dubbele arteriële bloedvoorziening van de urethra, en, zo mogelijk vermindering van het risico van postoperatieve erectiele disfunctie of glans ischemie. Deze benadering zou ook nuttig zijn voor latere urethrale interventies zoals opnieuw urethroplasty met behulp van een gratis graft, waarin een goed gevacuoliseerd transplantaat bed absoluut noodzakelijk is. Deze potentiële voordelen zijn echter alleen aannames zoals prospectieve studies vergelijken de functionele uitkomst van beide technieken met gevalideerde vragenlijsten momenteel ontbreken. Bovendien, vaartuig-sparend EPA (vsEPA) ten minste moet kunnen bieden soortgelijke chirurgische resultaten als tEPA. Het doel van deze paper is een uitgebreide, stapsgewijze overzicht van het beheren van patiënten met geïsoleerde korte bulbaire of posterieure urethrale stricturen met vsEPA te geven. Het hoofddoel van dit manuscript is te schetsen van de chirurgische techniek en het resultaat van de representatieve chirurgie. Een totaal van 117 patiënten werden beheerd volgens het protocol beschreven. De analyse werd uitgevoerd op de gehele patiënt cohort en op de bulbaire (n = 91) en posterieure (n = 26) vsEPA groep afzonderlijk. Slagingspercentages waren 93,4% en 88,5% voor de bulbaire en posterieure vsEPA, respectievelijk. Tot slot, voorziet vsEPA, zoals uiteengezet in het protocol in uitstekende slagingspercentages met lage complicatie tarieven geïsoleerde korte bulbaire en posterieure urethrale stricturen.

Tussen 2011 en 2017, een totaal van 117 patiënten met geïsoleerde korte bulbaire (n = 91) of posterior (n = 26) urethrale stricturen werden behandeld met vsEPA op Universitair Ziekenhuis Gent. Deze vat-spaarzame techniek is gericht op het verminderen van chirurgisch trauma tijdens urethroplastie door excisie en primaire anastomose. Het behoud van de bulbaire slagaders kan het risico van postoperatieve erectiestoornissen en glans ischemie verminderen. Bovendien kan het gunstig zijn voor verdere urethrale interventies die een goed aanhoudende avascularisatie vereisen.Na het bevestigen van de aanwezigheid van een vernauwing door retrograde en/of antegrade urethrography, pas de patiënt op de operatietafel in de supine positie, in een gewijzigde lithotomie positie met de knieën en heupen gebogen 60 tot 90 graden, met 45 graden van ontvoering, en de kalveren ondersteund in beenhouders. Desinfecteer het blootgestelde perineum en de uitwendige genitaliën met een antiseptische scrub op basis van jodium en plaats steriele gordijnen rond het werkveld, inclusief de penis en het scrotum, zonder de anus. Om toegang te krijgen tot de bulbar urethra, gebruik een nummer 24 scalpelblad om een midline paraniel huid incisie te maken, en ontleden het sub-cutane vetweefsel via Colles Fascia met een monopolaire elektrocauterie, totdat de bulbospongiosus spier wordt aangetroffen, cauterizing eventuele bloedende vaten als dat nodig is.Snij de bulbospongiosusspier in de middellijn en gebruik de Schaar van Jones om de spier weg te ontleden van het onderliggende corpus spongiosum. Bevestig vervolgens de bulbospongiosusspier op de perineale huid met vier zijden 2-0 blijf hechtingen. Breng een zelfbehoudend oprolmechanisme aan en bevestig elastische blijfhaken aan de bulbospongiosusspier met voldoende tractie, waardoor de elastieken worden vastgesteld aan een van de sleuven van het oprolmechanisme.Vervolgens ontleden de bulbar urethra op een omtrekige manier, te beginnen bij het mid bulbar gebied, om verdere dorsale dissectie van de plasbuis en een detachement van de dorsale kanalen fascia van de tunica albuginea van de corpora cavernosa mogelijk te maken. Omring de blootgestelde bulbar urethra met een vatlus en bevestig de lus met een kocherklem om manipulatie te vergemakkelijken. Vervolg de dorsale dissectie en onthechting in de distale richting in de richting van de penoscrotale hoek met behulp van een scherpe schaar, en nauw na de avasculaire chirurgische vlak van de fascia Buck's.Ga vervolgens verder met de dorsale dissectie proximally naar het urogenitale diafragma, zonder de bol van het corpus spongiosum aan de anale kant te ontleden en de bulbaire slagaders in tact achter te laten. Om de vernauwing te openen, introduceer een 20 Franse siliconen urethrale katheter, of een 20 Frans metaal geluid in de meatus urethri, en laat het in de richting van de distale omvang van de urethrale vernauwing. Gebruik op dit niveau een nummer 24 scalpelblad om de urethra dorsali op een longitudinale manier te openen en plaats een zijde 3-0 blijfhechting aan elke kant van de geopende urethra, om de blootstelling van het urethrale lumen te vergemakkelijken.Steek een drie Franse urethrale katheter door de plasbuis en proximally omhoog door de scrictured gebied, het openen van de vernauwing langs de katheter tot de gezondheid urethrale weefsel wordt aangetroffen. Introduceer een 20 Franse metalen geluid naar de blaas om de patency en kaliber van de proximale urethra te evalueren, en plaats een zijde 3-0 verblijf hechting aan elke kant van de geopende proximale urethra. Dan resect de vernauwing, evenals de omliggende spongiofibrose, totdat gezonde spongeous wordt aangetroffen ventrally, en pak de urethrale mucosa met een debakey tang met een grote grip.Het is zeer belangrijk om de urethrale vernauwing en de omliggende spongiofibrose volledig te resecteren om twee goed gevasculariseerde urethrale uiteinden te verkrijgen. Met behulp van onderbroken 4-0 hechtingen, licate de ventrale uiteinden van de proximale en distale urethra van de binnenkant van het urethrale lumen voor de oprichting van de ventrale urethra plaat. Dorsaal spatten de gezonde urethrale randen voor ongeveer een centimeter, en verwijder het verblijf hechtingen.Gebruik vervolgens onderbroken 4-0 hechtingen om de rugranden van de urethrale uiteinden dwars te sluiten en het corpus spongiosum over de urethra te sluiten. Om urethroplastiefalen te voorkomen, is het van het grootste belang om de anastomose zonder spanning te voltooien. Verwijder het oprolmechanisme, en snijd het verblijf hechtingen met mayo nog schaar.Vervolgens, perforeren de huid van de juiste inguinale regio met de 10 Franse afvoer naald. Laat de siliconenkatheter op zijn plaats, plaats een 10 Franse zuigafvoer tussen het corpus spongiosum en de bulbospongiosusspier. Gebruik een 3-0 lopende hechting om de bulbospongiosus spier te sluiten over de onderliggende plasbuis en corpus spongiosum.Dan, hecht Colles fascia op een lopende manier, en hecht de huid met onderbroken Donati steken, zowel met behulp van 3-0 hechtingen. Gebruik een zijde 2-0 hechting om de zuigafvoer vast te stellen aan de lies. Breng vervolgens een pararenale drukdressing aan.Tussen 2011 en 2017 werden in het Universitair Ziekenhuis Gent in totaal 117 patiënten met geïsoleerde korte bulbar- of posteriorurethrale vernauwingen behandeld met vattende excisie en primaire anastomose, met een mediane opvolgtijd van respectievelijk 35 en 45 maanden voor bulbar- en achterste strictures. De complicatie was even laag in beide groepen, en bestond voornamelijk uit low-grade gebeurtenissen, zoals wonddehiscence, wondinfectie, urine-infectie, blaasspasmen, en hematoom. Over het geheel genomen bedroeg de geschatte verzuimvrije overleving respectievelijk 95,3%95,3% en 87% op één, twee en vijf jaar voor de bolvormige vatvrije excisie- en primaire anastomosegroep en 88,3% over dezelfde controleperioden voor de achterste vaartuigsparende excisie- en primaire anastomosegroep.Tot slot, vaat-spaarzame excisie en primaire anastomose biedt uitstekende slagingspercentages met lage complicatie tarieven voor geïsoleerde, korte bulbar en achterste urethrale strictures.
