1. 包容性标准

</ol>2. 排除标准

</ol>3. 定位和规划
<ol><li>使用标准技术对患者进行全身麻醉。</li>
	<li>通过无菌准备皮肤和拖曳手术区域，在整个手术过程中保持无菌状态。</li>

</ol>4. 开放
<ol><li>在自然皮肤折痕内沿皮肤做一个 2 厘米的线性水平切口。</li>
	<li>使用 5 毫米切割毛刺的高速钻头，用直径为 1 到 1.2 厘米的颅骨打开头骨。试图使颅骨切除术与血肿的长轴对齐，但避免中线结构和雄辩的大脑区域。 注意：穿孔器大于必要性，特别是当缺陷在额头上时。</li>

	<li>使用骨蜡、血栓中的凝胶泡沫和双极烧灼来达到血吸虫病。</li>
	<li>使用超声波可视化底层血肿，以确认其大小和位置。 注：在二分法之前，在潮湿的场地使用毛刺孔传感器，超声波质量最高。</li>
	<li>以十字形的方式打开杜拉，将杜拉尔叶烧灼到骨边缘一毫米以内。 注意：避免大静脉或动脉。</li>
	<li>在烧灼之前，用 #11 刀片将皮亚母校 1 厘米化。如果获得脑活检，这是理想的时间。使用肿瘤钳或彭菲尔德1仪器的杯端。避免烧灼，直到获得活检。</li>

</ol>5. 第一阶段疏散
<ol><li>沿计划轨迹插入介绍护套，并在护套内放置导航样式。样式提供提示位置的实时反馈。 注：鉴于1厘米颅骨切除术体积小，通常不可能采用"跨阴道"方法：因此，皮亚被切开，并进入颅骨切除术正下方的非血管空间。</li>

	<li>将内窥镜插入护套。将魔杖插入内窥镜的工作通道内，用占主导地位的手握住魔杖。 注：对于此阶段的程序，保持内窥镜和魔杖在护套的末端，约0.5至2厘米从护套的末端。</li>
	<li>使用指尖手指缓冲内窥镜和魔杖手柄之间的距离，以保持对护套内设备尖端位置的持续感知。</li>
	<li>将吸入系统的吸力设置为 100%，并将灌溉流量设置为低（+25%）。</li>
	<li>吸气任何液体血肿，呈现在护套的末端，同时保持魔杖在护套的1厘米内。 注：当血肿被吸气时，由于质量效应的降低，腔内会向内塌陷。在疏散的第二阶段，持续灌溉保持腔的结构。</li>
	<li>如果遇到不单独吸气的固体血块，激活魔杖内的支气管以消化血块。</li>
	<li>如果一块血块太大或纤维不能吸吸，并粘附在魔杖的尖端，请取出整个内窥镜和魔杖与血块一起，注意不要将血块从魔杖中取出。 注：这被称为适应（直接吸入第一通技术）技术，参考在血栓切除过程中切除血管内血栓的急性缺血性中风15的实践。</li>

</ol>6. 第二阶段疏散
<ol><li>将魔杖的吸力降低到 25%，将灌溉增加到 100%，以提高腔内的能见度。探索残余血肿，确定出血动脉。 注：在这个阶段，尽量减少近亲脑组织的吸力至关重要。</li>
	<li>以低吸力有针对性的方式吸入任何残留血肿，注意不要损害周围可能导致额外出血或对腔壁造成创伤的脑物质。 注：血液制品最初可能会干扰最佳可视化，但腔将清除耐心，连续灌溉。如果腔不清除，识别和烧灼出血血管。</li>

</ol>7. 评估和关闭
<ol><li>缓慢地取出内窥镜和护套，在退出时用内窥镜检查切口壁，以监测是否有额外的出血。 注：一些操作员主张此时将血栓蛋白注入腔中，要么直接将血栓添加到灌溉液中，要么通过内窥镜注射与血栓混合的明胶。这是一个合理的选择，但注射明胶与血栓混合将使超声波成像是不可能的。</li>

	<li>如果可用，执行术中 Dyna-CT 扫描，以评估疏散程度。 注：该程序的目标是获得至少80%的疏散。如果剩余的血肿超过 20%，在继续以下步骤之前，重新启动疏散的第 2 阶段。</li>
	<li>将止血凝胶泡沫涂在大脑表面的毛刺孔中。</li>

据报道，在内窥镜内窥镜血肿疏散期间灌溉导致颅内压力增加16时，结果不佳。SCUBA 程序通过在第 1 阶段疏散血肿来避免这种情况，该阶段可降低腔内的压力，随后在第 2 阶段灌溉。在第二阶段，内窥镜有第二个接入端口，允许灌溉外流，从而避免血肿腔的过度分散和高升的内腔压力。此外，护套和滑道不会形成防水密封，灌水液会在护套周围丢失。

有待解决的一个领域是程序的时限。许多协议主张在72小时内和6小时稳定扫描后撤离，以确保血肿稳定。许多医生选择这一行动方案，作为2004年的一项小型研究报告并发症，再出血，和不良的结果，在一小系列患者谁接受了颅骨切除术的超早期手术19。最近关于微创内窥镜疏散的研究报告了超早疏散20，21的良好结果。报告内窥镜疏散的手稿表明，出血在超早期疏散中是可识别和可控的。ENRICH 研究协议要求在 ictus 的 24 小时内疏散，并且不要求进行稳定性扫描。超早手术可能是未来的一种选择，但需要进行额外的研究来评估超早期疏散的风险和益处。

minimally invasive endoscopic intracerebral hemorrhage evacuation

SCUBA 疏散技术在 2015 年 12 月至 2017 年 9 月间接受内窥镜 ICH 疏散的 47 名患者中进行了描述。平均术前ICH体积为42.6厘米3（标准偏差 = 29.7厘米3;术后ICH体积平均为4.2厘米3，SD6.6厘米3），平均疏散率为88.2%（SD 20.8%）（表1）。图1显示了术前和术后CT扫描的例子。在23例（48.9%）病例中，检测出活动性出血血管，其中12例（52.2%）发现多血管出血（表2）。仅用灌溉就解决了出血问题，5例（10.6%）和18例（38.3%）（表2）中用电化处理出血问题。术后出血仅被隔离到一例（2.1%），其中术后第1天进行的常规头部CT显示，疏散腔已补充出血，似乎源于表面的大风血管出血进入通道和腔（表2）。这个病人的检查没有恶化，他不需要额外的手术。
