clinical protocol of producing adipose tissue-derived stromal vascular fraction for potential cartilage regeneration

変形性関節症は、高齢者の間で最も一般的な衰弱性疾患の一つです。現在、変形性関節症の根本的な原因に対する明確な治療法はありません。この臨床プロトコルは、組織のような軟骨を再生することによって、ヒト膝変形性関節症の有望で低侵襲的で代替的な治療のための革新的な手順を記述する。変形性関節症は、高齢者の間で最も一般的な衰弱性疾患の一つです。現在、変形性関節症の下線の原因に対する明確な治療法はありません。この臨床プロトコルは、軟骨様組織を再生することにより、ヒト膝変形性関節症の有望で低侵襲で代替治療のための革新的な手順です。バイオハザードクラスAフードを持つ手術室に患者を連れて来て、彼または彼女を説得力のある位置に置きます。5%ベタジン-ポビジンヨウ素で患者の腹部を洗浄し、無菌技術を使用して患者をドレープし、脂肪吸引のために腹部のきれいな領域を露出させます。臍帯から約5センチメートルの陰性に、左と切開の右側の2つの部位を麻酔する。エピネフリンを含まない2%リドカインの2ミリリットルを、表皮レベルで各部位を注入することにより、5ミリリットルシリンジに25ゲージ1.5インチの針を用いた。5ミリリットルのチュームセント溶液と10ミリリットルの注射器と針25ゲージ1.5インチを使用して行われる切開部位を麻酔します。皮膚をつまんで皮下の深さを上げることで、臍の下に約5センチメートル下の0.5センチメートルの2つの切開を行います。番号11の刃を使用して、上げられた皮膚が皮下レベルまで浸透するのに役立ちますが、腹壁を貫通しないようにします。20センチメートル60ゲージカニューレを使用することにより、700〜800ミリリットルの房液で脂肪吸引される下腹部領域全体の皮下レベルを麻酔する。下腹部全体に浸透した後、手動脂肪吸引用または遠心分離キットに接続された3.0ミリメートルのカニューレを手動で使用するために、60ミリリットルまたは30ミリリットルの注射器に接続された3.0ミリメートルのカニューレを接続して脂肪吸引装置を準備します。クローズドシステムシリンジは、真空補助脂肪吸引のために真空機械に接続された無菌性を維持する目的で、FDA CDRHによって開発され、承認されました。脂肪吸引を行い、房液を除いた脂肪組織の100〜110グラムを得た。脂肪吸引を行う場合、脂肪組織は、房状溶液と一緒に得られるが、分離して除去する必要があります。最初に房状溶液を重力によって分離するために、遠心分離機キットの脂肪組織を60ミリリットルのシリンジに移し、注射器を下に置くことができる。例えば、シリンジの開口部部分は底部にあります。5~6分待つことで、脂肪組織と房液が分離されます。シリンジプランジャーの上部を押して、シリンジの底の流体を取り除きます。膝当たりの脂質吸引脂質の100グラムから110グラムの合計が蓄積されるまで、上記のステップ9〜11を行う。火口液を重力で分離し、各60ミリリットル遠心分離キットの無菌閉じたシステムごとに50グラムから55グラムの脂質吸引装置を蓄積した後、2つの遠心分離機キットを遠心分離容器バケツに入れ、1600 Gで5分間回転し、脂質吸引を凝縮し、脂肪組織から液体を分離する。このプロセスは、さらに脂質吸引を凝縮し、特定の場合に脂肪油を産生する可能性がある。振らないために、遠心分離機キットの下部にある安全キャップとプラグを取り外します。遠心分離機キットのプランジャーの上にゆっくりと押し下げて下液を取り除きます。別々の60ミリリットルシリンジで、コラゲザーゼ10ミリグラム、結合組織に特異的な5ミリグラムのコラゲターゼ、および正常な生理食塩水の50ミリリットルの脂肪組織に特異的なコラゲターゼの5ミリグラムを溶解する。約25〜30ミリリットルの凝縮脂質吸気物を溶解したコラゲナーゼと混合コラゲターゼ、結合組織に特異的なコラゲナーゼ5ミリグラム、脂肪組織に特異的なコラゲナーゼ5ミリグラムを1対1の比率で、体積対体積で、60ミリリットルシリンジを遠心分離機キットに特殊なコネクタを使用して混合します。ロッドまたはプッシャーを使用して、60ミリリットルのシリンジと遠心分離機キットの間の内容を押し出すことによって、準備吸引とコラゲナーゼを凝縮するために十分に混合します。脂質吸気とコラゲエーゼの混合物を60ミリリットルのシリンジに戻します。各60ミリリットルのシリンジを、ブレードを含む組織ホモジナイザーと混合して接続します。空の60ミリリットルのシリンジをホモジナイザーのもう一方の端に接続します。混合物をホモジナイザーを通して他の60ミリリットルのシリンジに4〜6回押し込み、脂質吸引の切断とミンチを生じる。均質化された脂質吸気およびコラーゲン混合物を特殊なコネクタを介して60ミリリットル遠心分離機キットに戻します。遠心分離機キットを容器に入れ、摂氏37度で予熱したインキュベーターに入れます。45 rpmで回転しながら、37°Cで均質化した混合物を2つの遠心分離機キットを40分間インキュベートします。40分間のインキュベーションの後、滅菌方法でインキュベーターから容器を取り出します。次に遠心分離機のキットを取り外し、遠心分離機に入れます。800Gで5分間遠心分離し、脂肪組織由来のSVFを分離する。遠心分離機の後、コラゲナーゼと消化脂肪組織を含む上清は、プランジャーの上にシリンジキャップを取り除き、シリンジロック接続を介してプランジャー開口部に30ミリリットルの注射器を置くことによって、各遠心分離機キットから取り除かれます。30ミリリットルのシリンジをゆっくりと押し下げて上清を入れ、30ミリリットルのシリンジを充填します。遠心分離キットの底の最後の3〜4ミリリットルまで30ミリリットルのシリンジバレルを押し下げ、脂肪組織由来のSVFの最後の3〜4ミリリットルだけを残します。上清は破棄されます。プランジャーの上部から30ミリリットルのシリンジを取り出し、D5LRとしても知られている5%デキストロースラクチンゲル溶液でシリンジを充填します。プランジャー開口部の上部にD5LRで満たされた30ミリリットルのシリンジを取り付けることで、脂肪組織由来SVFの3〜4ミリリットルを含む遠心分離キットをD5LRで最大55ミリリットルで満たします。その後、30ミリリットルのシリンジを取り出し、プランジャー開口部をキャップし、遠心分離機キットを300 Gで再び遠心分離機を約4分間遠心分離します。合計 4 回の洗浄に対して、手順 17 から番号 21 を繰り返します。4回目の遠心分離機の後、最終的なSVFを射出用に得るために、遠心分離機キットを振ったり回したりすることなく、遠心分離機キットの下部開口部にある安全キャップとプラグを取り外します。特別に設計されたコネクタを使用して、20ミリリットルのシリンジを遠心分離機キットの底口に取り付けます。20ミリリットルのシリンジのプランジャーを数回前後に引っ張り、遠心分離機キットの底に落ち着いた細胞を揺さぶります。ASTとECMの両方を含むSVFの所望の総容積を他の細胞および組織とともに除去する。通常、膝関節注射用の各遠心分離機キットから約3〜4ミリリットル。我々は抗凝固薬クエン酸デキストロース溶液の2.5ミリリットルと一緒に自家血の30ミリリットルを行った。その後、引き出された血液を60ミリリットル遠心分離機バレルキットに移した。その後、730重力で5分間遠心分離した。そして上清を取り除き、新しい60ミリリットル遠心砲身キットに入れました。上清を1350重気で4分間遠心し、PRPの4.4ミリリットルを得た。5%ベタジンで患者の膝をきれいにし、滅菌的な方法でそれをドレープする。脛骨と大腿骨の間の関節空間のために膝の前部を触診する。注射部位を表面的に希釈リドカインで麻酔し、1%リドカインを4ミリリットルの通常の生理食塩水で希釈し、皮膚から関節カプセルのすぐ外に挿入する。次に、関節カプセルの内部を希釈ロピバカイン、1ミリリットル0.75%ロピバカインを3ミリリットルの通常の生理食塩水で希釈して麻酔します。20ミリリットルのシリンジに含まれるSVFの6〜8ミリリットルに2ミリリットルのHAをシリンジコネクタにシリンジを介して混合します。注射器から注射器へのコネクタを使用して、すでに準備され、5ミリリットルの注射器で準備されている自家PRPの3〜4ミリリットルに塩化カルシウムの0.4ミリリットルを追加します。5ミリリットルのシリンジに3.5~4.5ミリリットルのPRP塩化カルシウムを組み合わせ、8~10ミリリットルのHA-SVF混合物を注射器から注射器コネクタまで20ミリリットルのシリンジで組み合わせます。直ちに混合物を注入し、約12〜15ミリリットル、膝の前脛骨-大腿部関節にゆっくりと、超音波誘導の有無にかかわらず38ミリメートル18ゲージ針を使用する。射出後、4x4綿ガーゼを4回折り、折り畳まれた4x4ガーゼの上にテープを置くことによって、注入部位を圧力で包帯する。患者に対して、細胞の付着を可能にするために60分間じっとしているよう指示する。患者に対して、退院後1週間の活動を最小限に抑える。3週間にわたって塩化カルシウムによって活性化PRPの3つの追加注射のためのクリニックに戻ります.第3段階のOAを有する患者は、プロトコルセクションに記載されているように治療を受けた。ASV-ECM混合注射の第2週後、患者の痛みおよびROMは改善した。16週目までに、患者の疼痛およびROMは、16週目以降に採取された70%以上のポスト治療MRIを有意に改善し、膝の内側側の軟骨様組織の厚さの増加を示した。第3段階のOAを有する別の患者は、プロトコルセクションに記載されているように治療を受けた。ASV-ECM混合注射の第2週後、患者の痛みおよびROMは改善した。18週目までに、痛みおよびROMは、18週目以降に採取された80%反復MRIに有意に改善し、膝の前内側側の軟骨様組織の高さの増加を示した。第3のOAを有する第3の患者は、プロトコルセクションに記載されているように治療を受けた。ASV-ECM混合注射の第2週後、患者の痛みおよびROMは第22週までに約50%改善し、疼痛およびROMは22週目以降に採取された80%反復MRIを大幅に改善し、膝の内側側の軟骨様組織の高さの増加を示した。現在、OAの治療には治癒療法は利用できません。しかしながら、塩化カルシウムで活性化されたHAおよび類似PRPとともにASCおよび均質化ECMを用いた幹細胞療法の経皮的注射は、膝OAを治療する現在の治療療法に代わるものであってもよい。
