この技術の主な利点は、低侵襲肺切除中の小、深部、および/または準固形肺結節の周術期局在化を助けることである。処置を開始する前に、結節局在化を受ける患者が低侵襲胸部手術に適した末梢肺結節を有することを確認するために、以前の胸部コンピュータ断層撮影またはCT画像化をレビューする。次いで、色素注入中の位置を模倣するために、結節に側方褥瘡位置および肺同側に位置する患者と非コントラスト胸部CTスキャンを行う。結節の動きを説明するために、呼気と吸気の両方の画像を取得します。処置の日に、ナビゲーションシステム計画ソフトウェアを使用して、標的病変をデジタル的にセグメント化する。標的病変が放射線学的に純粋なすりガラスである場合、セグメンテーションソフトウェアは病変を適切に識別できない可能性がある。その場合は、ターゲット病変の中心に仮想ターゲットを配置します。標的病変が正常にマークされたら、計画ソフトウェアを使用して、針が侵入するための経皮部位を描写する。患者を側方褥瘡の位置に置き、同側胸壁の3つの電子基準点パッドを結節に取り付け、選択した入り口から少なくとも5センチメートル離す。パッドを電磁ナビゲーションシステムに接続し、電磁ナビゲーションフィールドジェネレータを基準パッドの上に置き、患者をシステム内に登録します。次に、電磁ナビゲーションシステムが提供するプロンプトを使用して、キャリブレーションを微調整します。フィールドジェネレータが所定の位置に収まったら、電磁航法経胸部結節定位プラットフォームを使用して、リファレンスパッドの仮想スナップショットを撮ります。主気道の程度を描写するデータ点群を生成するために、まず、独自の電磁航法管使い捨てスコープカテーテルを二重管腔気管内チューブの右内腔に挿入する。カテーテルをメインカリナに合わせ、システムによって停止するように促されるまで、ゆっくりと気管内に引き戻します。次に、使い捨てスコープカテーテルを右下肺葉に打ち込み、別の緑色のチェックマークで停止するように促されます。データポイント収集が停止したら、使い捨てスコープカテーテルを気管内チューブの二重ルーメンの右肺ルーメンからチューブの左肺ルーメンのカテーテルに移す。次に、使い捨てスコープカテーテルを左主幹気管支に近位2〜3センチメートル、左上下葉に分岐させる。停止を促されるまで、使い捨てスコープカテーテルを左下葉に打ち込むデータ収集を再開します。完全なデータ点群が収集されたら、誘導のために電磁ナビゲーションプラットフォームを使用して、胸壁皮膚侵入部位に経皮針を位置合わせする。胸腔への入り口の皮膚に印を付け、入り口が肋骨よりも優れているように注意し、既知の血管系またはオッセウス構造を避けます。2%クロルヘキシジン溶液で最低15秒間皮膚をきれいにし、溶液を最低30秒間乾燥させます。滅菌技術を用いてフィールドをドレープし、滅菌手袋および滅菌ガウンを着用した。局所麻酔の入り口に1〜2ミリリットルの1%リドカインを皮下注射する。10枚刃の手術用メスを使用して、表皮を通る侵入部位に5ミリメートルの表面皮膚切開を行います。無菌の19ゲージ電磁針をマークされた入り口に置きます。電磁システムスクリーンの横方向および冠状ビューを使用して、標的病変の中心に揃うように進入角度を調整します。進入角度が確認されたら、電磁航法管の針を胸壁に対して安定させます。麻酔チームが患者を呼気で保持している間、胸壁を通して針をしっかりと進めます。オペレータは、電磁ナビゲーションシステムによって提供される3つのビューのうち少なくとも2つで針を標的病変に適切に位置合わせしてから、針を安定させ、まっすぐな下向きの推力で前進させることが重要です。胸壁から標的病変の遠位側に到達したら、トラクトスタイレットを取り外し、針を外さずに指で針ハブを覆う。次に、適切な濃度のメチレンブルーを2〜3ミリリットル含むシリンジを針に接続し、0.5ミリリットルの溶液を標的病変部に注入する。次いで、針をゆっくりと引き抜いてトラックを作成しながら、別の0.5ミリリットルの溶液を徐々にそして連続的に堆積させる。すべての色素が送達されたら、色素マーキングを使用して、低侵襲胸部手術によって肺結節を局在化および切除する。今実証したように電磁航法経胸郭結節局在化の後、局在化色素マーキングを収縮肺で識別することができ、診断楔切除を実施することができる。この手順を試みている間は、適切な登録と針胸壁の配置を確実にすることを忘れないことが重要です。この技術は、低侵襲手術法を用いた小結節切除の改善と、診断的葉切除術および/または開胸術への転換率に対するこの方法の効果を探求する上で、うまくいけば道を開くであろう。
