敗血症は多臓器不全や死を引き起こす可能性があります。内毒素はグラム陰性菌の基本的な成分である。内毒素の循環レベルの上昇は、より重篤な疾患を発症するリスクが高い患者を特定する可能性があります。このEndotoxin活性アッセイは、全身性内毒素症のシンプルで迅速かつ信頼性の高いバイオマーカーとして使用できます。この毒素の早期発見は、敗血症性ショック患者を治療する臨床医を助けるかもしれない。敗血症性ショックを受けた患者では、急速に進化する状態の早期同定は、より迅速かつ集中的な生命維持療法を促し、より良い結果につながる可能性がある。この手順を実証することは、私たちの研究グループと協力する分子生物学者のリサ・マティアセンです。この手順を開始するには、EAテストキットをセットし、5種類のチューブを開梱します。チューブナンバーワン、コントロールチューブを使用して、特定の抗体がない場合の患者の好中球の非特異的酸化バーストの基底活性を測定する。チューブナンバー2、サンプルチューブを用い、LPS抗体複合体に応答して酸化バーストを測定する。チューブ番号3、Maxチューブを使用して、過剰な内毒素に応答して患者の好中球の最大酸化バーストを測定する。チューブ番号4、LPSチューブを、外因性内毒素の供給源として使用する。そして、血液貯蔵のためにチューブ番号5、アリコートチューブを使用してください。EDTA抗凝固剤を含む滅菌チューブに患者の血液サンプルを収集し、EAテストを実行する前に室温でそれらを保存します。テストを開始する前に、ケミアルミノメーターとインキュベーターシェーカーをオンにし、インキュベーターを摂氏37度に温めます。まず、チューブラックに使用するEAチューブを配置し、キャップを取り外します。コンピペットを使用して、EA試薬の1ミリリットルをボトルからチューブ番号1、ナンバー2、ナンバー3にチューブの側面をピペットで移します。採血管を20回軽く反転させ、患者の血液サンプルを混合する。次いで、ピペット0.5ミリリットルの患者血液をチューブ番号4およびチューブ番号5に入れた。ボルテックスチューブ番号4を10秒間。すべてのEA試験管を備えたチューブラックをインキュベーターシェーカーに取り付けます。蓋を閉め、摂氏37度で10分間インキュベートします。この後、インキュベーターの蓋を開け、シェーカーからチューブラックを取り外します。渦管番号5。滅菌先端を使用して、ピペット40マイクロリットルの血液をチューブ1とナンバー2に、重複する。渦管番号4。同じピペットチップを使用して、チューブ番号4からチューブ番号3に40マイクロリットルの血液を複製します。6つの最終試験管を1つずつ渦。チューブラックをインキュベーションシェーカーに戻し、蓋を閉めます。シェーカーを100rpmに設定し、14分間動きを開始します。EAラベルのチップカードをケミルミニメーターに挿入し、Startを押します。14分間のインキュベーションの後、ケミルミノメーターに表示される指示に従ってください。各チューブを10秒間静かに渦を入れ、ケミルミノメーターのサンプルホルダーに置きます。サンプルドロワーを開き、チューブナンバーワンをサンプルホルダーに入れます。次に、サンプルドロワーを閉じて、相対光単位の読み取りを待ちます。テキスト プロトコルで説明されているように、チューブ 2 と 3 とすべての重複に対してこの読み取りプロセスを繰り返します。すべてのチューブが処理された後、EAの結果が自動的に計算され、印刷されます。検査が必要な血液サンプルごとにアッセイプロセス全体を繰り返します。EAアッセイキットとその部品を、摂氏2~25度の温度で元の密閉パッケージに保管してください。本研究では、重症患者の全血内毒素症を検出するために、内毒素活性アッセイが行われる。EA値が0.40未満のEA単位は、低い内毒素活性レベルを示し、循環LPS濃度が低く、これは重篤な疾患状態への進行のリスクが低いことを示す。0.40から0.59の間のEA単位の間の結果は、重度の敗血症および敗血症性ショックの発症の中間リスクを表す中間内毒素活性レベルを示す。0.60以上のEAユニットの結果は、敗血症性ショックおよび患者転帰不良のリスクが高いことを示す高い内毒素活性レベルを示す。チューブ番号4と5からの血液は、正しい試験管と混合する必要があります。プロトコルに示された正しい順序に従うように、特別な注意が必要です。すべてのチューブは重複して実行され、このフェーズで間違いにつながる可能性があります。導入後、EAアッセイは全身低灌流患者のリスク階層化のためのバイオマーカーとして提案されている。また、高度な水浄化治療戦略のための患者を選定するためのスクリーニング検査として研究されている。
