我们在一些棋盘阵列检测中遇到的一个问题是大量的跳井。这发生在与某些抗生素不成比例的比率。具体而言, 在针对一系列抗碳肠杆菌科的组合筛选中, 我们发现521项试验中有49项 (9.4%)由于多次跳井无法使用, 在这些试验中, 12 种抗生素 (fosfomycin 和头孢哌啶) 中有2种占46种 (94%)。这种增加的比率在特别容易受到接种效果31、32、37的药物中可能更有可能。值得注意的是, CLSI 不建议检测 fosfomycin 在肉汤稀释 25 , 由于担心这种方法的结果的可靠性, 这可能解释了不可靠的结果看到这种药物。可以根据调查人员的喜好对自动棋盘方法进行一些修改。如果出于实验室工作流程的原因, 最好将抗菌素分配到已经含有细菌悬浮液的板材中, 而不是空井中。虽然这里使用了384个井板, 但该方法也可以在96孔板检测中进行, 并对井体进行适当的修改。使用96孔板格式可能有助于减少对接种的小变化特别敏感的抗生素的跳过井。在计算 FICi时, 可能会出现 mic 不大规模 (即高于测试) 的情况, 包括被检测的药物与被测试的生物体类型单独没有活动的情况。在这些情况下, FIC 可以根据假设 MIC 比测试的最高浓度高出一稀释。这是最保守的策略, 因为它假定在协同测试中观察到抑制的任何稀释都是可能的 FIC 值。例如, 如果实际的 MIC 是两个加倍稀释高于最高浓度测试, 那么相应的 Fic 将比保守分配低两倍, 依此类推。
为了准确评估药物在时间杀伤试验中的杀菌活性, 培养物必须处于对数阶段的生长状态, 特别是在对细胞壁活性抗生素进行测试时 28。对于本例中使用的快速生长的细菌 (肺炎 k), 3小时的晃动孵育适合于达到这个生长阶段, 但不同的生物可能需要不同的时间。一般来说, 这种文化应该显得明显, 但不应该严重浑浊。通过在连续时间点 (例如, 4-6 每 30分钟) 构建一个生长曲线来确定适当的时间量.意欲的开始接种在时间杀害研究也是重要的。起始接种的目标浓度约为 5 x 10 5 至 1 x 10 6 chu/ml.这里描述的稀释 (100μl 的 1.0 Mfarland 悬浮液在10毫升的介质) 产生这种接种肺炎克雷伯菌和其他肠杆菌科物种,我们已经在其上进行了测试。如果在使用不同生物的实验中开始接种的密度明显高于或低于这一点, 则可能需要不同的稀释。(特定物种所需的适当稀释可以通过执行0.5 或 1.0 McFarland 悬浮液的板块计数来确定, 以确定这种浊度代表多少生物, 然后计算初始悬浮液必须为多少生物稀释以达到适当的最终浓度。如果在对协同作用研究中的板块计数进行回顾时, 发现任何含抗生素管的起始接种都明显低于生长控制的起始接种量, 这可能表明抗生素的结转或非常在短时间内迅速杀灭细菌, 从将细菌添加到含抗生素的试管和取出电镀用的脂肪。如果未稀释的滴落中的实际菌落数量低于随后稀释中的菌落数量,这表明抗生素的结转效应。为防止这种影响, 已经描述了不同的选择, 包括将一个单一的子分散在整个板38上或纺下样品, 去除上清液, 并在电镀39之前在无菌盐水中重新悬浮。在时间终止法的每个时间点, 调查人员也必须有效但准确地从每个培养管中取出一个子, 并执行串行稀释。在这一过程中的延迟, 特别是在连续发生的早期时间点, 可能会导致长时间的培养过程, 在这段时间内, 培养物不会被孵化和动摇, 而不小心分配和连续稀释可能会导致板材不准确计数。与板计数的扩散板方法相比, 每个稀释的100Μl 分布在整个琼脂板上, 所描述的落板方法要快得多, 需要的琼脂板数量要少得多, 并且允许更快的计数, 作为最大值每滴的殖民地数可数为 30, 而通常可以从扩散板计算多达300个菌落。然而, 如果调查人员对这种技术更适应, 扩散板方法也是一种选择。如果滴剂在与多通道移液器一起分配后扩散到对方, 则可以使用单通道移液器进行更宽间距的滴液的单独应用。根据我们的经验, 在分配滴之前在4°C 下冷却板似乎可以减少过度扩散。
这里描述的技术的一个局限性是, 两种类型的协同分析 (棋盘阵列和时间终止) 的结果并不总是一致的, 而且由于大多数发表的协同分析文章使用一种或另一种方法, 而不是两者兼而有之, 它可以很难知道如何集成这两种类型的检测数据。由于我们开发的自动棋盘阵列方法简单且吞吐量高, 因此我们实际上已将其作为一种屏幕来测试对更多分离物的组合, 并确定哪些浓度组合是协同作用的。然后, 我们执行了较少数量的时间终止研究, 选择了在棋盘阵列中有效的组合和浓度。值得注意的是, 由于棋盘检测通常是在微液稀释量表上进行的, 而时间杀伤性检测使用的体积较大 (类似于宏观稀释), 我们发现, 这两种方法有时不同, 而且更高时间测定中通常需要的浓度, 以证明其活性。这种现象已经注意到, 当宏观肉汤和微肉汤稀释 MIC 检测结果比较革兰氏阴性杆菌26和较大的接种 (用于时间终止研究) 与标准接种使用微肉汤稀释和棋盘阵列检测32。棋盘阵列的一个具体局限性是微液稀释 mic 测试22的内在变异性。虽然 FIC i用于协同作用的截断在数学上考虑到了这种可变性, 但这种可变性不可避免地引起了对棋盘阵列结果的可靠性和一致性的关注。

为了提高棋盘阵列方法的效率和吞吐量, 我们利用了以前在实验室开发的自动化 MIC 测试技术, 该技术使用喷墨打印技术精确、一致地分配小批量。抗生素库存溶液进入井在微滴板 14.该平台无需复杂的计算和多个移液步骤。相关软件计算和分配适当数量的抗生素, 以创建一个二维棋盘阵列, 如果用户只是输入所需的浓度范围和库存溶液浓度的抗生素。我们最初对一系列 CRE 分离株15进行了测试, 随后重点测试了含有大肠菌素的组合对抗大肠菌群的活性16。大肠菌素是一种最后手段的药物, 通常用于治疗 mdr 革兰氏阴性病原体17,18, 大肠菌素的耐药性使已经 mdr 细菌几乎具有抗性19, 使它们成为理想的候选人开发新的治疗策略使用药物, 他们是不敏感的单独。我们发现, 大肠菌素和蛋白质合成抑制剂抗生素米诺环素的组合具有很高的协同作用率, 即使对这些药物中的每一种单独产生耐药性的菌株也是如此, 大概是因为大肠菌素产生了亚抑制作用对甚至抗结肠炎细菌的渗透性作用。我们选择了这个组合作为本文的一个例子。值得注意的是, 协同试验也可用于评估两种单独有效的药物的疗效。
自动棋盘阵列方法有助于快速、高吞吐量的协同测试。但是, 棋盘数组方法确实有限制。作为一种改进的 MIC 检测方法, 它只提供关于抑制细菌生长的数据, 而不提供关于杀死的数据, 而不提供关于随着时间的推移抗生素影响的数据。相比之下, 人工性能的时间杀人协同检测是更多的劳动密集型, 但提供了在24小时时间路线20,21的抑制和杀死的信息。我们在数量较少的分离物上使用了时间杀伤分析来确认我们的棋盘阵列结果, 并确定我们确定的协同组合是否也是杀菌的。
棋盘阵列和时间杀伤协同方法都提供了关于药物组合活性的宝贵信息, 对于评估高耐药细菌病原体的潜在新治疗方案特别有用。这些方法也有其固有的局限性。标准的微肉汤稀释 MIC 方法具有已知的预期误差范围为1二倍稀释22, 当两种药物一起在棋盘阵列中测试时, 误差范围就会增加。协同作用的标准定义认为, 只有在药物在各自中等收入中等收入国家6倍的情况下共同活跃, 才会考虑到这种预期的变异性, 但也考虑到这种变异性 (这被认为是结果)从生物和技术波动的结合中不可避免地产生了对协同结果可靠性的不确定。缺乏既定的协同测试质量控制标准也是目前的一个限制。也许所有协同试验方法最重要的局限性是, 当使用组合治疗24患者时, 体外结果与临床结果之间缺乏既定的相关性。更简单、更快速的协同测试方法, 如此处描述的自动棋盘阵列方法, 可促进在临床试验或对患者结果的其他评估中整合体外协同测试, 以便更好地描述了未来体外效应与体内效应的关系。

图 2 a 和图 2A显示了类似于图 1 a 和图 1 b 中的网格, 但在这种情况下, 该组合并不显示与试验过的分离物 (k.肺炎分离体 bidmc 4) 的协同作用, 因为抑制生长的最小 fic i 为 1, 这是 & gt;0.5。
图 3说明了检查板协同网格中的光学密度读数, 其中发生了几口跳井 (阴沟肠杆菌复合物分离 bidmc 27)。跳过的井是指细菌生长受到抑制的井, 尽管相邻的井中存在细菌生长和较高浓度的抗生素。这种现象在标准的 MIC 测试中也有发生, 很可能是由于细菌生长特性从井到井的生物变异性, 以及一些抗生素对细菌接种的小差异的敏感性,31,32. 如果在棋盘阵列中发生了不止一个跳过的事件, 我们就放弃了结果, 重复了分析。
图 4给出了针对肺炎 k.分离器 bidmc 32 测试的三种组合的时间-杀协同结果的例子。殖民地计数以y轴上的对数刻度表示, 时间 (以小时为个时间) 以 x 轴为标的. 用红棒和数说明了含有药物组合的管内的启动接种与该管中细菌浓度的差异, 而管间 2 4小时的细菌浓度差异包含组合和单独包含最活跃的单剂的管用蓝条和数字来说明。图 4a显示了大肠菌素和米诺环素联合使用的结果;这种组合是协同的 (暴露在组合中的细菌浓度与大多数活性物质单独的细菌浓度之间的差异≥2 log 10 ccu/ml 在 24小时) 和杀菌 (从开始接种到浓度在24h≥3日志10下降)。图 4B显示了大肠菌素和克林霉素的组合结果, 这种组合是协同的, 但不是杀菌的。这种组合抑制了细菌的生长, 这两种药物都不是单独的, 但并没有杀死它们。图 4C显示了大肠菌素和红霉素的结合结果, 它们既不是杀菌的, 也不是协同作用的。

Antimicrobial Synergy Testing by the Inkjet Printer-assisted Automated Checkerboard Array and the Manual Time-kill Method
