Reuze cel arteritis (GCA) is een ontstekingsziekte van middelgrote tot grote slagaders, gericht op de aorta en haar bijkantoren en beïnvloeden van oudere volwassenen. Het presenteert met een milde tot ernstige ischemische complicaties, zoals hoofdpijn, kaak pijn, verlies van de visie, beroerte en weefsel gangreen. De diagnose wordt bevestigd door hoge inflammatoire merkers zoals bezinkingssnelheid (ESR) en een aparte histopathologische patroon op de temporele slagader biopsie (tabblad)1. GCA is de meest voorkomende volwassen vasculitis, en de toegankelijkheid van de temporele slagader verkregen voor diagnose presenteert een voordeel ten opzichte van andere vasculopathies, waardoor een te bestuderen zijn pathogenese gemakkelijker. De typische bevindingen op tabblad omvatten een infiltreren van macrofagen/histiocytes en T-lymfocyten aangetroffen in alle vasculaire lagen van de tunica intima, media en adventitia met gelijktijdige vernietiging van de elastische lamina die normaal deze scheidt compartimenten2. Het huidige bewijs toont aan dat de GCA impliceert een onbekende antigeen dat dendritische cellen in de vasculaire adventitia activeert, gevolgd door de indienstneming van helper (Th) T cellen, specifiek Th1 en Th17 subtypen die afscheiden van Interleukine-17 en interferon-gamma (IFG) respectievelijk. IFG dan werft en activeert de macrofagen voor de productie van cytokines en proteasen. Deze omvatten pro-inflammatoire cytokines; met inbegrip van IL-12, die samengebouwd activeert Th1 cellen waardoor een positieve feedback loop; Tumornecrosefactor en Interleukine-6, met als gevolg van artritis en koortsen en metalloproteinasen die schade de elastische lamina. Glucocorticoïden (GC), die de standaard chronische therapie vormen, bepalen gedeeltelijk de cytokine trajecten door verzachtende de Th17/IL-17 maar niet de Th1/IFG-arm van de immuunrespons3,4. Helaas, stopzetting van GC resulteert in ziekte herval. Als een alternatieve modaliteit, methotrexaat heeft geweest voorzien voor GCA behandeling, maar de gewenste klinische eindpunten waren niet consequent bereikt hoewel gevoeliger biomarkers niet voor therapeutische toezicht5,6 gebruikt hebben . In 2017, werd de Interleukine 6 blocker, tocilizumab, goedgekeurd voor de behandeling van de GCA zoals effectief blijkt ziektebestrijding en steroïde sparend effecten7,8.
Tot op heden zijn er geen goede biomarkers voor GCA. Dientengevolge, kan het moeilijk zijn om te controleren van de activiteit van de ziekte en titreer doses van beschikbare drugs voorkoming van negatieve effecten te verminderen van de last van de kosten voor de samenleving. Het is dus absoluut noodzakelijk om te zoeken naar een biomarker van de kandidaat-lidstaten dat correlaten met ziekte-activiteit, nieuwe inzichten over de pathogenese en therapeutische besluiten steun verstrekken.
De disregulatie van macrofaag activering is een cruciale factor in de pathogenese van de GCA. Dus, een effectieve middelen voor de behandeling van de GCA kan worden de selectieve doelgerichtheid van geactiveerde macrofagen. De folaat receptor beta (FRB) is een glycosyl-phosphatidylinositol glycoproteïne die is verankerd op de celmembraan uitgedrukt in normale MyeloMonocytaire cellen, myeloïde leukemie cellen en macrofagen geactiveerd. De ligand, foliumzuur, is een essentieel vitamine in zijn gereduceerde vorm, waarmee cellulaire DNA synthese, methylation en reparatie9. FRB expressie wordt in auto-immune ziekte en tijdens carcinogenese geïnduceerd. Haar expressie is beperkt tot het oppervlak van de monocyten of pro-inflammatoire M1 macrofagen bij reumatoïde artritis, of anti-inflammatoire M2 macrofagen in solide orgel en myeloïde maligniteiten10,11,12 , 13. Naast zijn potentiële rol als een biomarker van geactiveerde macrofagen, de FRB selectieve drug vervoer door middel van een multi stap proces dat met de receptor binden aan foliumzuur, antifolates of folaat-geconjugeerde drugs bij de cel begint kunt inschakelen oppervlakte, gevolgd door internalisering, de introductie en de uiteindelijke levering van gewenste drug aan de interne cel machines12,14,15,16. In tegenstelling tot FRB uitgedrukt in kankercellen en macrofagen geactiveerd, uitgedrukt in normale hematopoietische cellen FRB is niet in staat om te folates, waardoor ervoor wordt gezorgd dat FRB/folaat-gemedieerde drug delivery beperkt tot FRB-positieve inflammatoire of kankercellen is binden.
In onze eerdere studie, we laten zien voor de eerste keer dat FRB wordt uitgedrukt in GCA macrofagen en haar expressie gecorreleerd met CD68 + en endothelin receptor bèta macrofagen, die tot de GCA pathogenese17 bijdragen. Reuze cel arteritis (GCA) is een chronische immuun-gemedieerde ziekte van middelgrote tot grote formaat slagaders die oudere volwassenen treft. GCA manifesten met artritis en occlusieve symptomen van hoofdpijn, beroerte of visie van verlies. Macrofagen en lymfocyten van T-helper infiltreren de vasculaire muur en produceren van een pro-inflammatoire respons die leiden tot beschadiging van het schip en ischemie. Tot op heden is er geen GCA biomerker dat ziekte activiteit en gids therapeutische respons kan controleren.
Folaat receptor bèta (FRB) is een glycosylphosphatidylinositol eiwit dat is verankerd op celmembranen en normaal gesproken uitgedrukt in de bloedlijn MyeloMonocytaire en in de meeste myeloïde leukemie cellen zo goed zoals in de tumor en reumatoïde synoviale macrofagen, waar haar expressie correleert met de ernst van de ziekte. Het vermogen van FRB binden folaat verbindingen, foliumzuur-geconjugeerde en antifolate drugs heeft gemaakt een druggable doel in kanker en ontstekingsziekte onderzoek. Dit verslag beschrijft de methoden van het histopathologische en immunohistochemische gebruikt om te evalueren van de expressie en de verspreiding van FRB ten opzichte van GCAimmunopathology.
Formaline-vaste en paraffine-ingebedde temporele slagader Biopten van GCA en normale besturingselementen werden gekleurd met haematoxyline en eosine te herzien Weefsel histologie en Pathognomonisch omgaan. Immunohistochemistry werd gebruikt voor het detecteren van FRB CD68 en CD3 expressie. Een microscopische analyse werd uitgevoerd om te kwantificeren van het aantal positief gekleurde cellen op 10 geselecteerde hoog-power-veldsecties en hun respectieve plaatsen in de arteriële muur.
Lymphohistiocytic (LH) ontsteking gepaard met intima hyperplasie en verstoorde elastische lamina werd gezien in de GCA met geen gevonden in besturingselementen. De LH infiltreren bestond uit ongeveer 60% lymfocyten en 40% macrofagen. FRB expressie was beperkt tot macrofagen, bestaande uit 31% van de totale CD68 + macrofaag bevolking en gelokaliseerd aan de media en de adventitia. Geen FRB werd gezien in besturingselementen.
Dit protocol blijk gegeven van een afzonderlijke numerieke en ruimtelijke patroon van de macrofaag FRB ten opzichte van de vasculaire immuun communicatie in GCA.

Histopathologische bevindingen
De H & E vlekken in normale exemplaren aangetoonde normale arteriële anatomie met endotheliale cellen in de tunica intima, zachte spiercellen in de tunica media, en een heterogeen collageen-matrix die fibroblasten en feeder vaartuigen omvatten genoemd Wasa vasorum in de tunica adventitia. An Immunohistopathologic Study to Profile the Folate Receptor Beta Macrophage and Vascular Immune Microenvironment in Giant Cell Arteritis

Dit immunohistochemische en histopathologische protocol kan worden gebruikt om foliumzuurreceptor bèta te onderzoeken, dat is een kandidaat-eiwit met potentiële prognostische en therapeutische waarde voor de vasculaire micro-omgeving in gigantische cel arteritis. Dit protocol is beproefd en op grote schaal gebruikt en kan worden gebruikt om de foliumreceptor bèta-attributen in tijdelijke slagaderweefsels te verkennen, zelfs na langdurige opslag. Om weefselsecties te verwerven, gebruik een microtome om vier tot vijf micrometer dikke delen van paraffine-ingebedde tijdelijke slagaders te snijden, die elke sectie in een 40 graden Celsius waterbad drijven aangezien zij worden verkregen, om rimpels te verwijderen.Gebruik glazen microscoopdia's om drie secties per dia vast te leggen en verwarm de dia's op een opwarmingsplaat gedurende 60 minuten in een oven van 60 graden Celsius. Wanneer de secties zich aan de glijbanen hebben gehecht, plaatst u de glijbanen gedurende 24 uur in een verticaal rek om te drogen bij 37 graden Celsius. Breng de dia's de volgende dag gedurende ten minste 12 uur over op een diacontainer voor opslag bij vier graden Celsius.Laad vervolgens in een schuifrek en hydrateer de weefsels met twee vijf minuten, 100% xyleenonderdomies en een dalende, vijf minuten per concentratie ethanol onderdompelingsreeks, met 10 seconden agitatie om de 30 seconden. Na de 70%ethanol onderdompeling, spoel de dia's twee keer in dubbel gedestilleerd water gedurende vijf minuten, met 10 seconden van agitatie om de 30 seconden. Voor het ophalen van antigen, breng het rek in een glazen container met 200 microliters van 95 graden Celsius, 10 millimolar per liter citraat buffer gedurende 30 minuten.Laat de glijbanen aan het einde van de incubatie 20 minuten afkoelen tot kamertemperatuur voordat u vijf minuten met stromend water spoelt. Om de endogene peroxidaseactiviteit te verwijderen, gebruikt u eerst een hydrofobe marker om een cirkel rond elk weefselmonster te tekenen, voordat u de monsters in 200 microliters van 3% waterstofperoxide gedurende 10 minuten uitbroedt, gevolgd door 3 wasbeurten in 200 micoliters van tris-gebufferde zoutoplossing, of TBSS, per wasbeurt. Na de laatste wasbeurt, dep elke dia voorzichtig om een overtollige buffer te verwijderen en voeg 200 microliters van de primaire antilichaam cocktail van belang en een cover slip aan elke dia voor een een uur incubatie in een vochtige kamer bij kamertemperatuur.Gooi aan het einde van de incubatie de afdekbonnen weg en spoel de glijbanen af met drie wasbeurten van twee minuten in verse tbs's. Voeg na het verwijderen van de overtollige buffer 200 microliter van een geschikte biotinylated secundaire antilichaamoplossing en een afdeksel toe aan elke dia voor een incubatie van 45 minuten in de vochtige kamer bij kamertemperatuur. Na het weggooien van de dek uitglijders, spoel de dia's met TBSS zoals aangetoond.Verwijder eventuele overtollige buffer. En label de secties met 200 microliters van DAB substraat buffer gedurende 10 minuten bij kamertemperatuur. Aan het einde van de incubatie, was de dia's met TBSS en lopende kraanwater gedurende 10 minuten, voordat tellers met hematoxylin gedurende drie minuten.Voeg vervolgens 70 microliters van een geschikt montagemedium toe aan elke dia en tip langzaam een glazen afdekkingsstrip op het montagemedium om te voorkomen dat er bellen ontstaan terwijl de afdeksel op zijn plaats wordt neergelaten. Voor histopathologische analyse plaatst u de dia op het podium van een lichtmicroscoop en beoordeelt u de vasculaire architectuur van de eerste weefselsectie. Kwantificeer vervolgens het aantal macrofagen ten opzichte van het aantal lymfocyten per sectie.Voor immunohistochemische analyse, onderzoeken de folium receptor bèta-kleuring patroon op elke weefselsectie en kwantificeren van het aantal folium receptor bèta-positieve, CD68 positieve, en CD3 positieve cellen in 10 willekeurig geselecteerde high power velden per sectie. Hematoxylin en eosine kleuring in normale exemplaren onthult een normale arteriële anatomie, met endotheelcellen in de tunica intima, gladde spiercellen in de tunica media, en een heterogene collageen matrix, die fibroblasten en feeder vaten genaamd vasa vasorum in de tunica adventitia. Giant cell arteritis positieve tijdelijke slagaders tonen een matige tot ernstige ontsteking, met aggregaten en macrofagen, of histiocyten, lymfocyten, incidentele plasmacellen, en multi-nucleated gigantische cellen.Luminale vernauwing wordt ook opgemerkt, en gepaard met intimale verdikking en een 90% verstoring van de interne elastische lamina. CD3 positieve lymfocyten en CD68 positieve macrofagen zijn ook aanwezig in alle gigantische cel arteritis positieve exemplaren, met vergelijkbare kleuring patronen waargenomen in alle biopten. Kleuring voor folium receptor beta is beperkt tot macrofagen en multi-nucleated gigantische cellen die bij voorkeur lokaliseren naar de adventitia en media.Met name foliumzuurreceptor bèta bestond ongeveer 30% van de totale macrofagen. De tijdelijke slagader biopsie verwerking, snijden, en dewaxing vereisen zorgvuldige behendigheid om latere stappen uit te voeren. De dia-interpretatie vereist de expertise van een ervaren cardiovasculaire patholoog.Herinner de kijkers eraan contact- en ademhalingsvoorzorgsmaatregelen te gebruiken, zoals handschoenen en geventileerde kappen, bij het hanteren van specimens en de reagentia.
