この免疫組織化学的および組織病理学的プロトコルは、巨大細胞動脈炎における血管微小環境に潜在的な予後および治療的価値を有する候補タンパク質である葉酸受容体ベータを調べるために使用することができる。このプロトコルは、時間試験され、広く採用されており、長期保存後でも側頭動脈組織における葉酸受容体ベータ属性を探索するために使用することができる。組織切片を得るためには、ミクロトームを使用して、パラフィン埋め込まれた側頭動脈の厚さ4~5マイクロメートルの部分をスライスし、各セクションを得られる40°Cの水浴に浮かべ、しわを取り除きます。ガラス顕微鏡スライドを使用してスライドあたり3つのセクションをキャプチャし、60°Cのオーブンで60°Cのオーブンで、暖かいプレート上のスライドを温めます。セクションがスライドに付着したら、スライドを垂直ラックに入れ、37°Cで24時間乾燥します。翌日、スライドをスライドコンテナに移し、摂氏4度で少なくとも12時間保管します。次に、スライドをスライドラックに積み込み、2つの5分、100%キシレン浸漬、および濃度エタノール浸漬シリーズあたり5分の降下で組織を水和し、30秒ごとに10秒間の攪拌を行います。70%エタノール浸漬後、スライドを2回二重蒸留水で5分間リンスし、30秒ごとに10秒間撹拌します。抗原の検索のために、ラックを95°Cの200マイクロリットル、1リットルクエン酸バッファーあたり10ミリモルのガラス容器に30分間移動させます。インキュベーションの終わりに、スライドを室温まで20分間冷却してから、流水で5分間すすいます。内因性ペルオキシダーゼ活性を除去するには、まず疎水性マーカーを使用して各組織サンプルの周りに円を描き、200マイクロリットルの過酸化水素3%を10分間インキュベートし、続いて200ミコリ酸トリスバッファー状の生理食塩水(TBS)で3回洗浄します。最後の洗浄の後、各スライドを軽く手を出して余分なバッファーを取り除き、目的の一次抗体カクテルの200マイクロリットルを追加し、各スライドにカバースリップを加え、室温で湿度の高いチャンバーで1時間インキュベーションします。インキュベーションの最後に、カバースリップを捨て、新鮮なTBSSで3回の2分間のスケでスライドをすすきます。余分な緩衝液を取り除いた後、適切なビオチン化二次抗体溶液の200マイクロリットルを加え、カバースリップを各スライドに加え、室温で湿度の高いチャンバーに45分間インキュベーションします。カバースリップを破棄した後、実演したようにTBSSでスライドをすすきます。余分なバッファを削除します。そして、200マイクロリットルのDAB基板バッファーを室温で10分間ラベル付けします。インキュベーションの最後に、スライドをTBSSで洗浄し、水道水を10分間流し、ヘマトキシリンで3分間対抗します。次に、各スライドに適切な取り付け媒体の70マイクロリットルを加え、カバースリップが所定の位置に下げられると泡が発生するのを避けるために、ゆっくりとガラスカバーストリップを取り付け媒体にチップします。組織病理学的分析のために、スライドを光学顕微鏡のステージに置き、最初の組織セクションの血管アーキテクチャを評価します。次に、セクションあたりのリンパ球数に対するマクロファージの数を定量化する。免疫組織化学分析のために、各組織セクションの葉酸受容体ベータ染色パターンを調べ、1セクション当たり無作為に選択された10個の高出力フィールドにおける葉酸受容体β陽性、CD68陽性、およびCD3陽性細胞の数を定量化する。正常な標本におけるヘマトキシリンおよびエオシン染色は、チュニカ内皮内の内皮細胞、チュニカ培地中の平滑筋細胞、およびチュニカ・アドベンティティアの血管血管の血管を含む異種コラーゲンマトリックスを有する正常な動脈解剖学的解剖を明らかにする。巨大な細胞動脈炎陽性側頭動脈は、凝集体およびマクロファージ、またはヒステオサイト、リンパ球、時折形質細胞、および多核巨大細胞を有する中等度から重度の炎症を示す。ルミナル狭量も注目され、内弾性薄層の内増粘および90%の破壊を伴う。CD3陽性リンパ球およびCD68陽性マクロファージは、すべての巨大細胞性動脈炎陽性検体にも存在し、同様の染色パターンが全ての生検で観察される。葉酸受容体ベータの染色は、マクロファージおよび多核巨細胞に限定され、アドベンチアおよび培地に優先的に局在する。特に、葉酸受容体ベータは、マクロファージ全体の約30%を占めた。側頭動脈生検処理、切断、および脱蝋は、その後のステップを実行するために慎重なデキステリアを必要とする。スライドの解釈は経験豊富な心血管病理学者の専門知識を必要とします。検体や試薬を扱う際には、手袋や換気フードなどの接触や呼吸の注意を使用するように視聴者に知らせる。
