CMRは心臓質量の病因の決定的な診断を提供することができるので、心磁気共鳴またはCMRは潜在的な血栓の評価において重要である。CMRは、エコーが主に視覚化およびサイズ測定を提供する軟部組織塊のマルチシーケンス詳細な特徴付けを提供することができるので、心エコー検査のような他のモダリティよりも利点があります。MRIの前に、患者は禁忌である可能性のある金属または埋め込まれた装置についてスクリーニングされる。ガドリニウムを投与する場合は、十分な安全性を確保するためにアレルギーおよび腎機能についてスクリーニングされる。心臓MRIイメージングは、いくつかの課題に直面しています。最適なイメージングのために、患者は息を止める必要があります。息切れの患者は、このタスクを達成するのが困難な場合があります。EKGは画像化にも使用され、心臓不整脈の患者は、格子を破壊する可能性があります。この目標を達成するには、パラメータを調整する必要があります。私と一緒に心臓MRI検査を実証することは、私たちの主任MRI技術者であるデブ・ブラノンと心臓MRI技術者のチャド・ウッドハウスです。取得を開始する前に、コマンドを効率的に伝達し、左胸部の最適な位置に心電図のリードを配置できるように、技術者のマイクに接続されたヘッドフォンを患者に提供します。十分なEKG信号を確認した後、患者の位置をスキャンテーブル上で確認し、信号対雑音比を最大化するための適切なサイズの表面コイルを心臓の上に配置する。有効期限終了時に息を止めて画像撮影中に指示された場合に呼吸するように患者に知らせる。このタイプの呼吸ホールドでは、吸気性の高い呼吸保持と比べて、再現性の高い画像化が可能です。患者が準備ができたら、スカウト画像、明るい血液バランスシネ安定状態自由な精度軸スタックを完全な心臓カバレッジで取得します。次に、さらにスキャン処方に役立つおよそ2と4つのチャンバーシーケンスを取得します。組織特性評価のために、黒血球三重反転回復シーケンスを得る。ネイティブ T1 マッピング スキャンを取得します。最初のパス動脈灌流モジュールを実行するには、1キログラム当たり0.05〜0.1ミリモルの造影剤を毎秒3〜4ミリリットルの投与速度で送達する。次に、コントラストの注入中に画像の左心室心筋を通過するまで、問題の質量を最もよく強調する軸面または平面に沿って動的な最初のパス灌流画像を取得します。ガドリニウム注入の10分後、1.5テスラで約200〜450ミリ秒、3テスラで300〜500ミリ秒になる反転へのスキャン時間を定義するのに役立つT1スカウトシーケンスを実行します。その後、反転時間を設定して6〜8ミリメートル位相感受性反転回復スライスを取得します。適切な反転時間設定は、コントラスト注入を超えて毎分増加することに注意することが重要です。また、血栓はこの設定で古い黒い外観を持つ必要があり、心筋と腫瘍は中間信号を有する可能性があるため、約600ミリ秒の高いTI画像を取得することをお勧めします。シネ bSSFP イメージの質量を評価します。軸面は、心臓質量のイメージングに役立つことがよくあります。心臓血栓評価のために、血栓は、この横右心房塊が示す、またはこの右心房の強度が慢性期間において低いT2加重信号強度を示すので、亜急性期におけるT2加重信号強度の増加を示すことができる。心臓血栓評価では、問題の質量は、血栓に対して示唆し、血管腫瘍の存在を意味する内部灌流について慎重に分析される。疑わしい質量内の後期ガドリニウム増強画像について評価する。血栓内では、内部後期ガドリニウム増強の固体領域は期待されていません。ここでは、心臓血栓の評価に一般的に使用される慣例および新興のCMR配列が示されている。心臓血栓は重要なCMR配列に提示される。矢印で強調表示され、右心室に質量があります。この質量は、bSSFPおよびT2加重画像上の低い信号を有し、内部動脈灌流を欠いている。対応するCT画像は、質量内の低密度を示しています。遅延後コントラストイメージングは、内部後期ガドリニウム増強を示さない。ネイティブT1マップは、穏やかに上昇T1緩和時間を示しています。これらの知見は心臓血栓と一致する。心臓血栓とは対照的に、右心房に転移する肝細胞癌を有する患者も提示される。右のカボアトライアル接合部の質量はbSSFP上の低い信号とT2加重画像の中間信号を有する。質量内のネイティブT1緩和時間が減少し、MRA画像に軽度の内部増強が見える。これらの知見は、心臓内転移と一致する。高品質の画像を取得し、患者の動きによる劣化がある場合は画像を繰り返し、呼吸保持時間を管理可能にするようにスキャンパラメータを調整することが重要です。心臓MRは、多くの場合、灌流および増強評価を用いて優れた軟部組織特性評価を提供することにより、エコー結果に加える心臓血栓の決定的な診断を与える。
