肝硬変の患者は、胃または食道静脈瘤1、2、3の腹水または出血など、主に門脈性高血圧(PHT)に関連する合併症を発症する危険性がある。肝代償不全のリスクはPHT2の程度に関連している。肝静脈圧勾配(HVPG)の測定は、肝硬変患者における門脈圧を推定する金本格、すなわち正弦門門高血圧症の重症度を評価する。HVPG は ≥6 mm Hg ~ 9 mm Hg で、高いポータル圧力(「下臨床ポータル高血圧症」)を示し、HVPG ≥10 mm Hg は CSPH を定義します。このプロトコルは、機器と手順の詳細な説明を提供し、潜在的な落とし穴を強調し、トラブルシューティングのためのアドバイスを提供します。
臨床的には、(i)の測定は、副鼻腸門高血圧症の診断を確立するために示され、(ii)CSPH(HVPG≥10mmHg)を診断することによって肝代償不全のリスクがある患者を特定し、(iii)は、ワニサール出血の一次または二次的な予防における薬理学的療法を導く、および(iv)後に肝不全のリスクを評価する。HVPGは、肝線維症/機能の改善および/または悪化のための確立された代理マーカーとして使用され、HVPGの減少は臨床的に有意義な利益5に変換するので、より高いHVPG値は増加したバリセアル出血リスク6に関連している。非選択的β遮断薬(NSBB)または病因療法の下での患者におけるHVPGの変化に関する観察に基づいて、10%のHVPGの減少は臨床的に関連する7,8であると考えられる。
現在までに、HVPGと同様の精度でポータル圧力の程度を反映する代替の非侵襲的なパラメータはありません。HVPGが実際にポータルの圧力を測定する「間接的な」方法であっても、肝硬変9の患者の「直接」測定されたポータル圧力を強く相関し、正確に反映します。重要なことに、HVPG測定は、肝パレンチマ10、11、12の評価量を最大化するためにバルーンカテーテルを使用して行われるべきである。HVPG測定は侵襲的で、資源集約的であり、圧力測定値の信頼性および妥当性を解釈する上での介入スキルおよび専門知識を必要とするが、この方法は、肝硬変13、14、15の患者における門脈性高血圧を診断およびモニタリングするための現在のゴールドスタンダードである。
血小板数などの単純な検査値は、CSPH の尤度を推定するのに役立つ場合があります。しかしながら、血小板数、または血小板数を含む非侵襲的スコアは、予測値16に限定的である。肝硬変患者における脾腫17 またはポルトシステム性の副次的な18 を示す画像のモダリティはCSPHの存在を示唆するが、実際の門脈性高血圧の程度を定量化するのに役立たない。肝臓19 および/または脾臓20 のエラストグラフィーのような新規非侵襲的イメージングツールは、CSPHの存在を支配または排除するのに有用である。それでも、使用可能な方法のどれも、ポータル圧力21の動的変化を直接測定することはできません。
HVPGの予後値はいくつかの画期的な研究によって下線が引かれていて、HVPG≥10mmHg(すなわちCSPH)が静脈瘤8の形成(および、門脈性高血圧22に関連する合併症の発症)について予測的であることを示し、一方でHVPGの(薬理学的誘導)減少は、ワニス成長23および脱代償のそれぞれのリスクを調節する。HVPG応答は、(再発)バリセザル出血を予防するNSBの有効性のための唯一の確立された代理である。HVPGがNSBB治療中に≤12mmHgの値に減少するか、または≥10〜20%減少した場合、患者は、24、25に増加し、生存率が増加し、バリセザル出血から保護される。同様に、HVPG応答はまた、補償肝硬変5、26を有する患者における腹水および関連合併症の発生率を減少させる。いくつかの研究は、HVPGガイド療法27、28、29、30、31、32の使用を支持する証拠を提供している。したがって、十分な経験を持つセンターでは、HVPG応答が治療の決定を導き、門脈高血圧症患者のためのパーソナライズされた医療を促進する可能性がある。
さらに、HVPGの測定は、肝硬変および/または気道高血圧がベンチからベッドサイドに翻訳される新しい治療法の有効性を評価する概念実証研究の代理エンドポイントとして役立つかもしれない。最終的に、HVPGの測定はまた、HCC39の発症および肝障害後肝切除40のリスクに関する重要な予後情報を提供することができる。
HVPGを測定するためのインフラストラクチャは、二次および第三次ケアセンターで容易に利用可能である必要があります。HVPG測定の技術は専門的な訓練と装置を必要とするため、学術センターや移植センターが肝血球力学実験室を設立し、門脈性高血圧の最先端の診断と管理を促進することは合理的であると考えられます。大容量センターは、年間数百のHVPG測定を行います。当社の経験に基づいて、正確なHVPG測定を行うのに十分な専門知識は、通常、50-100監督されたHVPG測定の後に得られます。
