Aquí clasificamos CLS como CLS convencional o selectivo, con la diferencia importante es atacar el tronco comprensivo vs communicantes grises del rami (inyección de fenol frente vs. detrás de la fascia anterior del músculo psoas mayor). Hemos implementado CLS selectiva en pacientes jóvenes, mujeres con livedo reticularis10, niños con Eritromelalgia7,9y pacientes seniles con fase final arteriosclerosis obliterante y comorbili...

Simpatectomía lumbar química (CLS) ha demostrado ser un tratamiento eficaz para enfermedades isquémicas1,2,3,4,5 incluyendo la tromboangeítis obliterante (a veces llamado enfermedad de Buerger), pie diabético isquémico, enfermedad de Raynaud, parmoplantar hiperhidrosis6, Eritromelalgia7,8,9y livedo reticularis10. Ha sustituido la cirugía abierta debido a una serie de ventajas. Es mínimamente invasiva y económicamente atractivo, no requiere hospitalización y anestesia general y puede realizarse varias veces. Sin embargo, la exquisita precisión es fundamental. Por ejemplo, paraplejia se ha divulgado en CLS usando persiana técnica11. Guía radiográfica para controlar la posición exacta de la punta de la aguja de punción y puntura trazado y profundidad, mediante el uso de medios de contraste para visualizar áreas de tratamiento se mejora la precisión del procedimiento. Comúnmente se practica para colocar la punta de la aguja más allá de la fascia anterior del músculo psoas mayor e inyecte el fármaco alrededor del tronco comprensivo8,12,13,14,15, que se define como CLS convencional. Sin embargo, debido a la ubicación del tronco comprensivo y el uréter son ambos delante de la fascia anterior, inactivar agentes puede difundir al uréter y causar daños. En informes de daño pieloureteral que presentó imágenes radiográficas12,13,14,15, se inyectó la droga delante de la fascia anterior, según el contraste que se separa.
Basado en nuestra experiencia previa en realizar procedimientos de CLS, hemos encontrado que targeting communicantes grises del rami comparable eficaz y seguro comparado contra el tronco comprensivo. Communicantes grises del rami son fibras simpáticas postganglionares situadas detrás del tronco comprensivo y delante de la raíz de nervio de madera, a lo largo del borde lateral del cuerpo vertebral, sobre todo dentro del músculo del psoas mayor. Así, la fascia anterior proporciona una buena barrera para el área de ureteropélvica y del músculo del psoas mayor proporciona una buena barrera para la raíz de nervio lumbar. Simpatectomía lumbar química (CLS) es un procedimiento mínimamente invasivo utilizado para el tratamiento de condiciones como enfermedades isquémicas de las extremidades inferiores, hiperhidrosis, etcetera. Comúnmente se practica para colocar la punta de la aguja de punción delante de la fascia anterior del músculo psoas mayor e inyectar al agente inactivador alrededor del tronco simpático, que se define como CLS convencional. Aunque es relativamente raro, daño ureteropélvica es la complicación más frecuentes de CLS convencional y puede causar graves daños a los pacientes. Encontramos que inyectando al agente inactivador detrás de la fascia anterior, que sólo objetivos gris rami communicantes, ayudó a lograr eficacia terapéutica en la vasodilatación, reducción del sudor y dolor alivio comparable al CLS convencional y serio las complicaciones se redujeron en gran medida. Entendemos este procedimiento selectivo CLS. Aquí, presentamos un protocolo de CLS selectiva. Targeting Gray Rami Communicantes in Selective Chemical Lumbar Sympathectomy

Presentamos un protocolo de sympathectomy lumbar química selectivo (CLS), que hace inactivo sólo gris rami communicantes y no el tronco comprensivo. El daño ureteropelvico es la complicación grave notificada con mayor frecuencia de la simpatectomía lumbar química convencional o CLS. Aunque es relativamente raro, es muy dañino para los pacientes. Este protocolo ayuda en la entrega precisa de los agentes activadores detrás de la fascia anterior del músculo mayor de las psoas, permitiendo la focalización selectiva de los comunicadores de rami gris, por lo que la seguridad de CLS puede mejorarse en gran medida.Antes de iniciar el procedimiento de tratamiento, confirme la presencia de indicaciones y la falta de contraindicaciones basadas en los resultados clínicos y de laboratorio del paciente. Desinfectar un área de piel de aproximadamente 30 centímetros dos veces en el sitio de punción de aguja planeado con un 2% de entoiodina y cubrir la espalda del paciente con toallas quirúrgicas. Usando guía lateral de torascopia, anestesiar la piel y todo el tracto de la aguja con 5-8 mililitros de 0.5%lidocaína sin vasoconstrictores, avanzando la aguja a la superficie vertebral gradualmente.Coloque la punta de la aguja en el área de inyección óptima utilizando rayos X lumbares de vista lateral. A continuación, inyecte 0,5 mililitros de agente de contraste para confirmar que la punta de la aguja está detrás de la fascia anterior del músculo psoas. Si el agente de contraste permanece confinado principalmente en el área óptima con la propagación a lo largo del músculo mayor de psoas, la colocación de la aguja fue exitosa.Inyecte 2 mililitros de solución de fenol 5% acuosa en el lugar de inyección, ajustando la dirección de la punta de la aguja para asegurarse de que 1 mililitro de fenol se extiende hacia la cabeza del paciente y que 1 mililitro se extiende hacia los pies del paciente. Cuando se haya inyectado el fenol, retire la aguja de la piel y comprima y cubra el sitio de punción con gasa para lograr hemostasia. Cinco minutos después de la inyección, confirme una interrupción comprensiva satisfactoria como un aumento de al menos 2 grados Celsius en la temperatura de la piel.Si no se logra una interrupción satisfactoria, realice una inyección suplementaria en otra zona óptima con un volumen total de fenol de no más de 10 mililitros en un día. Mida de nuevo la temperatura de la piel del paciente, calculando el promedio entre los aumentos de temperatura en la espinilla, plantar y pie dorsal. Antes de inyectar la solución de fenol, confirme que la punta de la aguja se encuentra dentro del área óptima.En el CLS selectivo, el agente de contraste se inyecta detrás de la fascia anterior del músculo mayor de psoas, apuntando a los comunicadores de rami gris. Los efectos clínicos esperados son vasodilatación, reducción del sudor, y alivio del dolor. Una colocación precisa de la punta de la aguja y la evaluación de la dispersión del agente de contraste son fundamentales para garantizar una focalización precisa de los comunicadores de rami gris en el tratamiento con CLS.Con CLS selectivo, el riesgo de operación se reduce en gran medida, por lo que las indicaciones para CLS podrían ampliarse.
