Endotracheal administration er den præferentielle vej til at levere eksogene stoffer i lungerne. Siden flere år er den direkte injektion af bleomycin i luftrøret blevet udbredt til at inducere pulmonal fibrose13 , og for nylig er der udviklet mere avancerede, ikke-invasive teknikker til at opnå denne14,15 ,16.

Pulmonal fibrose er en progressiv patologisk proces karakteriseret ved overdreven aflejring af ekstracellulære matrixkomponenter, hovedsagelig type I kollagen, i lunge interstitiumet, hvilket fører til nedsat lungefunktion. Det er kendetegnende for flere menneskelige lungesygdomme med en anden ætiologi og repræsenterer en dårlig klinisk prognostisk faktor. Da de nuværende terapier er ret begrænsede1, er musemodeller fortsat et vigtigt redskab både til yderligere undersøgelse af de sygdomsfremkaldende mekanismer, der påvirker sygdommens indtræden og progression, og til udvikling af nye antifibrotiske strategier2,3.
Til dato, administration af bleomycin har været den mest almindeligt anvendte model af eksperimentelt induceret pulmonal fibrose4. Ved siden af flere leveringsmetoder (herunder intravenøs, intraperitoneal, subkutan og inhalationel) er intratrakeal eller endotracheal tubus injektioner af bleomycin dukket op som de hyppigst anvendte veje4,5. Den metode, som vi beskriver heri, er blevet udviklet for at undgå skalding effekten af bleomycin på trakeal slimhinden. I virkeligheden, ved at uddrive luftrør og visualisere det gennem et opererende mikroskop, er det muligt at opnå instillation af hele volumenet af bleomycin opløsning direkte i den nedre luftvejene uden spild i de øvre luftveje. Pulmonal fibrose er kendetegnende for flere menneskelige lungesygdomme med en anden ætiologi. Da de nuværende terapier er ret begrænsede, er musemodeller fortsat et vigtigt redskab til udvikling af nye antifibrotiske strategier. Her giver vi en effektiv metode til at undersøge in vivo anti fibrotisk aktivitet af humane mesenchymal stromale celler opnået fra hele navlestrengen (HUC-msc) i formildende bleomycin-induceret lungeskade. C57BL/6-mus får en enkelt endotracheal tubus injektion af Bleomycin (1,5 e/kg legemsvægt) efterfulgt af en dobbelt infusion af hUC-MSC (2,5 x 105) i hale vene, 24 timer og 7 dage efter administration af bleomycin. Ved offer på dag 8, 14, eller 21, inflammatoriske og fibrotiske ændringer, kollagen indhold, og hUC-MSC tilstedeværelse i explanted lungevævet analyseres. Injektion af bleomycin i muse luftrøret muliggør direkte målretning af lungerne, hvilket fører til omfattende lungebetændelse og fibrose. Den systemiske administration af en dobbelt dosis af HUC-MSC resulterer i den tidlige sløvhed af den bleomycin-inducerede lungeskade. Denne protokol kan med held anvendes både for at fremkalde vigtige inflammatoriske og fibrotiske reaktioner i begge lunger ved endotrakeal injektion af Bleomycin og for at validere nye anti-inflammatoriske og anti-fibrotiske systemiske behandlinger ved hale vene infusion. Den største fordel ved denne teknik er, at direkte injektion af Bleomycin i musens luftrør sikrer hurtig målretning af lungerne uden spild i de øvre luftveje, hvilket muliggør sikker, robust og reproducerbar induktion af lungeinflammatorisk og fibrose. Demonstration af proceduren vil være Dr.Fiorenza Orlando, en dyrlæge fra vores samarbejde laboratorium.Til at begynde, placere bedøvet mus liggende på ryggen på en kirurgisk platform. Fastgør forsigtigt benene med kirurgiske båndstrimler for at holde den på plads. At hyper udvide halsen af musen, placere en støtte såsom dental bomuld roll under sin livmoderhalskræft region.Brug derefter et barberblad til forsigtigt at barbere halsen. Brug et par anatomiske kraftbecer til at klemme huden. Kollagen aflejring blev målt ved hydroxyprolin assay.Efter injektion af bleomycin steg hydroxyprolinniveauet gradvist fra dag otte til dag 14 og 21. Der henviser til tilsætning af menneskelige navlestreng mesenkymale stromale celler væsentligt reduceret virkningerne af Bleomycin. Ekspressionsniveauer af Col1A1 type et kollagen gen i hele lunge mRNA viste også en progressiv stigning på dag otte, 14 og 21, som blev delvist reduceret ved human navlestreng mesenkymale stromal cellebehandling.Immunostaining med specifikke antistoffer viste, at infunderes menneskelige navlestreng mesenkymale stromale celler hurtigt og effektivt nåede muselunger, selv om kun få celler blev opdaget. Disse data tyder på en hurtig dislokation af cellerne fra stedet skade på trods af den langvarige beskyttende virkning. Brugen af et driftsmikroskop sikrer høj tillid til succes ved at give operatøren mulighed for præcist at overvåge den korrekte placering af nålen i muserøret før installation, hvilket minimerer risikoen for at beskadige den.
