Endotrakeal administrasjon er fortrinnsrett rute for å levere eksogene agenter i lungene. Siden flere år har direkte injeksjon av bleomycin inn i luftrøret blitt mye brukt til å indusere pulmonal fibrose13 og, nylig, mer avanserte, ikke-invasive teknikker har blitt utviklet for å oppnå dette14,15 ,16.

Pulmonal fibrose er en progressiv patologisk prosess preget av overdreven deponering av ekstracellulære matrise komponenter, hovedsakelig type I kollagen, i lunge interstitium, som fører til nedsatt lungefunksjon. Det er kjennetegnet på flere menneskelige lungesykdommer med en annen etiologi og representerer en dårlig klinisk Prognostisk faktor. Siden dagens terapier er ganske begrenset1, mus modeller fortsette å være et viktig verktøy både for videre etterforskning av sykdomsfremkallende mekanismer påvirke utbruddet og progresjon av sykdommen og for å utvikle nye antifibrotic strategier2,3.
Hittil har administrasjonen av bleomycin vært den mest brukte modellen av eksperimentelt indusert pulmonal fibrose4. Ved siden av flere levere metoder (inkludert intravenøs, intraperitoneal, subkutan, og inhalativ), intratrakeal eller endotrakeal injeksjoner av bleomycin har dukket opp som de mest brukte rutene4,5. Metoden som vi beskriver her har blitt utviklet for å unngå forbrenning effekten av bleomycin på tracheal mucosa. Pulmonal fibrose er et kjennetegn på flere menneskelige lungesykdommer med en annen etiologi. Siden dagens terapier er ganske begrenset, mus modeller fortsette å være et viktig verktøy for å utvikle nye antifibrotic strategier. Her gir vi en effektiv metode for å undersøke in vivo antifibrotic aktivitet av menneskelige mesenchymal stromal celler Hentet fra hele navlestreng (HUC-msc) i demping bleomycin-indusert lungeskade. C57BL/6 mus mottar en enkelt endotrakeal injeksjon av bleomycin (1,5 U/kg kroppsvekt) etterfulgt av en dobbel infusjon av hUC-MSC (2,5 x 105) i halen blodåre, 24 h og 7 dager etter bleomycin administrasjon. Denne protokollen kan brukes både for å indusere viktig inflammatorisk og fibrotisk respons i begge lungene ved endotrakeal injeksjon av Bleomycin og for å validere nye antiinflammatoriske og antifibrotiske systemiske behandlinger ved haleveneinfusjon. Den største fordelen med denne teknikken er at direkte injeksjon av Bleomycin i musetrachea sikrer rask målretting av lungene uten søl i de øvre luftveiene slik at sikker, robust og reproduserbar induksjon av lungeinflammatorisk og fibrose. Å demonstrere prosedyren vil være Dr.Fiorenza Orlando, en veterinær lege fra vårt samarbeidende laboratorium.For å begynne, plasser den bedøvede musen som ligger på ryggen på en kirurgisk plattform. Fest beina delikat med kirurgiske båndstrimler for å holde det på plass. For å forlenge musens hals, legg en støtte som dental bomullsrull under livmorhalsområdet.Bruk deretter et barberblad for å barbere halsen forsiktig. Bruk et par anatomiske tang for å klemme huden. Bruk et par ring håndtert buet stump saks for å gjøre et kort snitt omtrent en halv centimeter i lengde i korrespondanse med musen sternohyoid muskel.Bruk bomullsullpinner for å stoppe blødninger. Kollagendeponering ble målt ved hydroksyprolinanalyse.Etter bleomycininjeksjon økte hydroksyprolinnivåene gradvis fra dag åtte til dag 14 og 21. Mens tillegg av human navlestreng mesenchymal stromal celler betydelig redusert effekten av Bleomycin. Uttrykksnivåer av Col1A1 type en kollagengen i hele lunge mRNA viste også en progressiv økning på dag åtte, 14 og 21 som ble delvis redusert av human navlestreng mesenchymal stromal cellebehandling.Immunstaining med spesifikke antistoffer viste at infunderte humane navlestrengmesenchymale stromale celler raskt og effektivt nådde muselunger, selv om bare noen få celler ble oppdaget. Disse dataene tyder på en rask forvridning av cellene fra skade på stedet til tross for den langvarige beskyttende effekten. Bruken av et driftsmikroskop sikrer høy tillit til suksess ved å la operatøren nøyaktig overvåke riktig plassering av nålen i musetrachea før installasjon, og dermed minimere risikoen for å skade den.
