Effectieve cardiopulmonale reanimatie (reanimatie) voor een hartstilstand is de sleutel tot een succesvolle opleving van een patiënt. Circulatie (C), luchtweg (A) en ademhaling (B) zijn de drie cruciale componenten in ofwel basic (BLS) of geavanceerde levensondersteuning (ALS). De evaluatie van de C-A-B-status tijdens reanimatie is in principe gebaseerd op pulscontroles, auscultatie en capnografie1. In echte klinische omstandigheden is reanimatie echter vaak complex en chaotisch, en deze methoden kunnen beperkingenhebben 2,3. Bijvoorbeeld, de nauwkeurigheid van pulscontroles is vaak suboptimaal, zelfs wanneer uitgevoerd door zorgverleners4. Meer dan de helft van een longintubaties kan worden misleid als geleid door auscultatie alleen5. Zelfs golfcapnografie kan worden beïnvloed door factoren zoals lage cardiale output, lage longstroom, en het gebruik van epinephrinum tijdens reanimatie2. Daarom is een effectievere, nauwkeurigere evaluatie van de C-A-B-status verplicht.
Bij het zoeken naar de etiologie van een hartstilstand of een mogelijk omkeerbare factoren tijdens reanimatie, medische voorgeschiedenis en lichamelijk onderzoek zijn vaak onvoldoende. De detectie van omkeerbare oorzaken, zoals de 5Hs (d.w.z. hypoxie, hypovolemie, hyperkalemia/hypokalemia, acidose en onderkoeling) en de 5Ts (d.w.z. harttampponade, spanning pneumothorax, longtromboemisme, coronaire trombose, en toxines of tabletten), vereist vaak geavanceerd onderzoek.
Echografie (VS) is een nuttige beeldvorming modaliteit in noodsituatie en kritische zorg instellingen. De Richtlijnen van de Europese Reanimatieraad (ERC) suggereren dat de VS een integraal onderdeel kunnen zijn van reanimatie6. DE V.S. kunnen gemakkelijk kritieke maar potentieel omkeerbare factoren identificeren die CPR, zoals harttamponade, longtrombo-embolie, en hypovolemie7,8,9beïnvloeden . VS helpt ook uit te sluiten pneumothorax en acute coronaire syndroom. Bovendien verleent de VS prognostische implicaties, zelfs wanneer reanimatie wordt uitgevoerd. De aanwezigheid van sonografische hartactiviteit tijdens reanimatie is gemeld aan hogere kansen op ROSC, overleving, en ziekenhuis ontslag10,11verlenen . Naast de sonografische evaluatie van de hart- en circulatiestatus wordt tracheale VS gebruikt voor de bevestiging van een goede endotracheale intubatie12, terwijl long-US wordt toegepast om een goede bilaterale longventilatie te bevestigen13.
Hoewel hoogwaardige reanimatie met minimale onderbrekingen van de compressies de belangrijkste factor is voor reanimatie, is het belangrijk om te begrijpen hoe u de VS naadloos integreren in het proces van reanimatie zonder de reanimatie te verstoren. Daarom ontwikkelden we een nieuw, ALS-compliant US-CAB protocol voor gebruik tijdens CPR14. Dit document presenteert een gedetailleerde beschrijving van de instellingen, het personeel en de instrumenten die nodig zijn voor een betere integratie van het Amerikaanse protocol in het reguliere reanimatieproces dat eerstelijnsartsen gemakkelijk kunnen integreren en implementeren in hun praktijk.

Het US-CAB (Ultrasound, Circulation/Airway/Breathing) protocol integreert verschillende sonografische technieken in een gestructureerde beoordeling van de circulatie, luchtwegen en ademhalingsstatus van een patiënt tijdens cardiopulmonale reanimatie (CPR) op een geavanceerde levensondersteuningsconforme manier. US-C biedt een subxiphoïde weergave van het hart, om te zoeken naar potentieel omkeerbare oorzaken van de ziekte, zoals pericardiale effusie, longembolie, hypovolemie, en acute coronaire trombose. Sonografische hartactiviteit tijdens reanimatie helpt niet alleen pseudo-pulsloze elektrische activiteit (PEA) te onderscheiden van echte PEA, maar vertegenwoordigt ook een hogere kans op de terugkeer van spontane circulatie (ROSC) en overleving. Evaluatie van de inferieure vena cava (IVC) toont de vloeistofstatus van de patiënt en geeft de beste methoden aan om te gebruiken voor vochtreanimatie. Als aortadissectie wordt vermoed, wordt een subxiphoïde weergave van de aorta voorgesteld voor het identificeren van een intimale flap. Zodra intubatie is gedaan, tracheale echografie (US-A) op de suprasternale inkeping helpt onderscheiden endotracheale intubatie (een lucht-muscopische interface met een komeet-staart) van slokdarmintubatie (dubbele tract teken). Onmiddellijk na de US-A, bilaterale long US (US-B) moet worden gedaan om een goede bilaterale ventilatie met behulp van de long schuifteken te bevestigen. Bovendien kan US-C serieel worden gevolgd om de dynamische veranderingen in de hartkamers en IVC te zien, of een hartcontractie die wijst op ROSC. US-B kan ook naast elkaar bestaande long- of pleurale pathologieën detecteren zonder de prestaties van reanimatie te verstoren. De belangrijkste zorg bij de uitvoering van deze methode is het handhaven van hoogwaardige reanimatie zonder vertragingen in borstcompressies bij het uitvoeren van US-CAB. Rigoureuze training en voortdurende praktijk zijn de sleutel tot het minimaliseren van eventuele onderbrekingen tijdens reanimatie.

Het US-CAB protocol is een eenvoudig, gestructureerd protocol, dat wordt gebruikt tijdens reanimatie. Het werd vernoemd naar de CAB sequenties in de geavanceerde richtlijnen voor levensondersteuning. Het US-CAB protocol kan worden gebruikt tijdens de reanimatie.Het heeft een positieve invloed op de resultaten van de patiënt. Idealiter moet een reanimatieteam bestaan uit een leider, een lid voor luchtwegbeheer en ventilatie, een lid voor borstcompressies, een lid voor defibrillatie, een lid voor intraveneuze katheterisatie en medicatie, en een recorder. Een sonograaf moet ook aanwezig zijn als een onafhankelijk lid van het team dat goed is opgeleid en ervaring heeft met reanimeren en die tijdig kan ingrijpen en de echobeelden kan interpreteren zonder de reanimatie-inspanningen te onderbreken of uit te stellen.Wanneer echografie moet worden geïntegreerd in het reanimatieproces, plaatst u een draagbare ultrasone machine in het staartgebied van de patiënt en stelt u elke twee minuten een alarm in voor reanimatie en elke 10 seconden voor pulscontroles. Om het hands-off interval voor pulscontroles ritmeanalyse te beperken, en gelijktijdige echografie evaluatie tot niet langer dan 10 seconden. Aan het begin van de reanimatie en aan het einde van de eerste vijf cycli van borstcompressies gebruik maken van de subxiphoïde vier kamer weergave om te controleren op pericardiale effusie, de grootte van de linker en rechter ventrikels, en sonografische hartactiviteit.Draai vervolgens de sonde 90 graden, parallel aan de lange as van de patiënt om de diameter van de inferieure vena cava te meten. Om de endotracheale buislocatie na intubatie te controleren, plaatst u de sonde dwars op de suprasternale inkeping en noteer de enige luchtslijmvliesinterface met één komeetstaartartefact voor tracheale intubatie. Verplaats de sonde naar de zijkant van de nek om het enkele teken van het darmkanaal te bevestigen.En re-intuberen, als er twee luchtslijmvlies interfaces met twee komeet staart artefacten. Om te controleren op een goede ventilatie, plaats de sonde aan beide zijden van de borst op de vierde tot vijfde intercostale ruimten over de mid oksellijn en kijk voor long sliding om longventilatie te evalueren. Als longschuiven aan één kant afwezig is, pas dan de diepte van de endotracheale buis aan tot bilateraal long slidings wordt opgemerkt.Herhaal vervolgens de cardiale echografie om de twee minuten wanneer de borst compressie wordt gestopt voor pulscontroles. Blijf de luchtwegen herhalen en echo's inademen na het vervoer van de patiënt en bedoverdracht. Als compressie van het rechteratrium en de ventrikel wordt opgemerkt tijdens hartechografie met subxiphoïde evaluatie, wordt pericardiocentese aangegeven en moet onmiddellijk worden uitgevoerd.Pericardiale effusie is ook van diagnostische waarde. Als de echogeniciteit hoog is of bloedstolsels aanwezig zijn in de hartzak, kan de etiologie wijzen op ernstige complicaties. Cardiale echografie met subxiphoïde evaluatie van de inferieure vena cava kan worden aangetoond door een verticale benadering, en visuele identificatie van de inferieure vena cava diameter helpt bij het beoordelen van de vloeistofstatus van de patiënt.De inferieure vena cava kan ook worden geëvalueerd in de subxiphoïde dwarse weergave. Cardiale echografie met subxiphoïde verificatie van de dalende abdominale aorta kan worden benaderd via een verticale of dwarse weergave. Deze optionele evaluatie wordt aanbevolen als aortadissectie wordt vermoed uit klinische presentatie of wanneer hemopericardium wordt waargenomen door cardiale echografie cardiale evaluatie.Endotracheale intubatie wordt bevestigd als een enkel teken van het traktaat wordt waargenomen. Als er een dubbele tract teken slokdarmintubatie is zeer waarschijnlijk. Ademhaling echografie wordt meestal onmiddellijk uitgevoerd na luchtweg echografie wanneer auscultatie of capnografie wordt uitgevoerd, maar het kan ook op elk moment worden uitgevoerd tijdens reanimatie wanneer verplaatsing van de endotracheale buis met een longintubatie wordt vermoed of wanneer specifieke etiologieën zoals pneumothorax of hemothorax moeten worden uitgesloten.Hoewel de scanvolgorde is georganiseerd, kan de volgorde worden gewijzigd op basis van de ervaring van de sonograaf. Gerichte training en voortdurende praktijk zijn essentieel om processen tijdens borstcompressies te minimaliseren. Het handhaven van een goed beeld van kwaliteit is een belangrijk punt voor verdere studies.
