远程实验室单元的设计适合离网太阳能电池板系统的功能。太阳能电池板不需要额外的燃料, 由于太阳辐射较高, 可以在非洲、亚洲和拉丁美洲的热带和亚热带地区以高生产率运行。目前, 商用太阳能电池板系统的一个单元允许每天使用高达 44 kWh/day 的电力。
无论选择哪种类型的替代电能, 实验室设施都强烈建议并预先安装脏电过滤器, 以提高电能质量并保护实验室设备。使 PCR 系统远离强的和非屏蔽的电磁辐射源, 因为强电磁辐射可能会干扰设备的正常运行。同样重要的是, 不要使用 PCR 系统附近的强振动源, 如离心机或泵, 因为过度振动会影响仪器性能。实验室设备只能安装在具有非导电污染物的环境中, 如灰尘颗粒或木屑。确保房间远离任何可能将颗粒物排出到仪器组件上的通风口。
实验室用水的使用取决于每天进行的诊断测试的数量和在该设施工作的实验室技术人员的数量。在包括提取和 PCR 测试在内的诊断过程中, 混合器的制备需要无核酸酶水, 并且必须作为其他用品和化学品提前交付。运行一次诊断试验至少需要50毫升的无核酸酶水;所需的无核酸酶水的体积取决于工作负荷 (即样品的数量)。运行高压灭菌器需要蒸馏水。一次循环的高压灭菌器耗水量为160-180 毫升;建议使用高压灭菌器进行日常使用。大多数塑料 (管道、移液器吸头等) 都是一次性的, 但也有一些是可重复使用的, 需要清洗 (大型容器、机架等)。常规自来水用于洗手之间的程序和其最小体积估计为15-20 升每天。水需要泵送压力;建议采用沉淀预过滤系统, 保护水工免受泥沙的破坏影响, 提高自来水质量。

所有实验室废物必须在产生时加以隔离。将固体、非锋利的传染性废物放入标记为生物危害的防漏废物袋中。如果产生的废物是尖锐的, 它必须放在防刺穿容器中。在贴有适当标签的生物危害容器中收集潜在的传染性液体废物。容器和袋子的填充量不应超过 2/。所有漂白剂产品的处置必须在自己指定的垃圾桶中进行分类。实验室废物必须温和处理, 以避免产生气溶胶和袋式容器破损。含有生物危害废物的收集袋/桶必须密封, 外部表面在使用0.5% 次氯酸钠溶液后必须进行净化。在焚烧前, 在121°c 高压灭菌器中对所有实验室废物进行30分钟的消毒。有关高压灭菌器的正确使用, 请参阅功能手册。如有可能, 在高压灭菌器中添加化学或生物指示器, 以确保适当的灭菌。所有蒸压固体和液体废物必须清楚地标记为消毒, 并设置、日期、时间和操作方式。然后, 在焚烧之前, 必须将标记的废物放置在安全、独立的区域。
正如所料, 诊断测试的工作流程取决于疾病和标本。如果建议进行病毒鉴定以采集血液样本 (例如埃博拉19), 则可以在-20°c 而不是-80°c (呼吸道病毒所必需) 储存样本。从病人身上取样时, 总是比以后对标本进行细分要好。如有可能, 对于每种类型的试样, 必须在单独的试样管中至少采集两个试样。如果不可能进行额外的采样, 则必须对样本进行细分。
如果替代样品不能在适当的温度下储存 (例如, 没有冰柜), 则应将棉签储存在纯 (100%) 中。乙醇或99% 甲基化的精神 (仅限甲醇添加剂)。在这种情况下, 必须将棉签尖端放入含有1-2 毫升乙醇的小瓶中。请注意, 此类标本仅适用于 PCR。另外, 请注意, 对于每一种特定的病毒诊断, 都需要有一个完善的检测方法, 即8、23和未知的病毒样本必须送往指定的实验室进行进一步调查19、20、 21岁
必须承认对呼吸道病毒诊断 PCR 检测实验室设备清单的强制性和建议性要求。表 3着重说明了基本和最低先进 (推荐) 设备以及 RT-PCR 诊断测试的要求。对于 BSL-3 实践, 人员的额外负压保护 (如手套盒) 至关重要。
连接的实验室模块最好是增加参与实验室测试的人员数量, 并加快一次测试所需的时间。使用自动 qPCR (例如, QiaCube) 可以替换耗时的手动 RNA 提取。虽然该仪器是笨重的 (宽度65厘米, 长度62厘米, 高度86厘米), 它可以适合移动实验室工作空间后, 重新排列家具在 BSL-2 或 BSL-3 单位。

remote laboratory management: respiratory virus diagnostics

根据实验室标准作业程序, 需要适用于 BSL-2 实践的 PPE (实验室外套、防护鞋、手套、高级面罩、防护眼镜等)。对于 BSL-3 实践, 负压 PCR 实验室模块配备了经过认证的手套盒。实验室单位通过外部直通窗口进行升级, 以在样品接收步骤中保护人员。注册过程可以通过以前开发的基于平板的应用程序 (图 3d) 进行简化。在笔记本电脑上运行的其他可接受的应用程序也可以使用。
这种特殊的呼吸道病毒诊断测试可以在连接的实验室模块中进行, 以单独的诊断程序步骤, 以避免污染或生化试剂之间的潜在干扰, 这可能会影响测试结果。为了最大限度地提高诊断质量, 快速诊断测试实践利用了 (一) 基本实验室 BSL-2 和横向连接的 PCR 室 (第3节) 或 (ii) 通过直通窗口连接的 GB 和 PCR 室 (第4节)。图 6显示了所建议的实验室工作流程图, 并强调了个人防护。该图认识到每个指示步骤对人员保护的重要性, 特别是在偏远地区的实验室工作人员接受最低限度培训的情况下。
流感的快速诊断测试是通过 RT-PCR 技术完成的。该过程包含四个主要步骤。请注意, 将为协议的每个阶段分配单独的工作区。

如果样品预计将被归类为 BSL-3 + (例如, 埃博拉、寨卡、中东呼吸和储存、结核病), 则必须使用手套箱设施。在远程实验室中, GB 室有自己的直通窗口来接收样品和笔记本电脑或平板电脑进行样品注册。样本的脂肪和病毒的失活都必须在手套箱内进行。建议使用 UV-C 照明, 以避免在手术过程中受到污染。在样品失活后, 协议的进一步步骤类似于基本的实验室 bsl-2 和 bsl-3 测试, 并遵循清单第三部分 (表 1,图 6).
