من أدنى والشريان الرئوي إلى اليسار وتراجع بعناية الشريان الأورطي إلى اليمين لتجنب الآفات المباشرة إلى الشريان الرئوي.استخدم الكيوتي لتشريح المساحة الرئوية الأورطية ومسح الجانب الأمامي من الشريان الأورطي الصاعد من النسيج الضام. بعد سحب بعناية في المسالك تدفق البطين الأيمن استخدام 4.0 خياطة برولين لوضع غرزة سلسلة محفظة على الشريان الأورطي الصاعد القريبة adventitia وتأمين خياطة مع tourniquet. إدراج cannula تسليم القلبية في الشريان الأورطي التصاعدي داخل خياطة سلسلة محفظة وتشديد tourniquet لتأمين القنية.المقبل فتح الوريد الرئوي السفلي الأيسر و الأجوف الوريدي العلوي والأدنى لضمان كافية تنفيس القلب ووضع الأبهري عبر المشبك على الشريان الأورطي الصاعد. بدء ضخ القلب بجنبا استهداف ضغط جذر الأبهري من 80 إلى 100 ملليمتر من الزئبق. تطبيق 0.9٪ كلوريد الصوديوم جليد طين على تجويف الصدر وفوق الجهاز.عند الانتهاء من ضخ القلب وجنبا، والمضي قدما مع استئصال cardiectomy بطريقة تقليدية، قسم الشريان الأورطي بعد الشريان السام والشريان الرئوي وعند التشعب لضمان طول كاف لعملية الزرع. لبدء الالتفافية القلبية في المتلقي، أولا استخدام Metzenbaum والملباب الزاوية اليمنى لتشريح بين الأجوف الوريدية متفوقة والشريان إينومينات والكافا الوريدية السفلية والبيريكارديوم من الحيوان المتلقي مخدر. تطويق فردي فينا cavae متفوقة والدونية مع ربطة عنق 0 الحرير وتأمين كل شريط مع tourniquet.استخدام خياطة برولين 4-0 لوضع اثنين من خياطة سلسلة محفظة متحدة المركز على الشريان الأورطي الصاعد السبتية وضع خياطة سلسلة محفظة على الوريد والكاواة السفلية والمتفوقة على مستوى انعكاس pericardial. تأمين كل خياطة مع tourniquet واستخدام تقنية Seldinger لإدراج 17-21 كانية الشرايين الفرنسية في الشريان الأورطي. استخدم موصل 3/8-3/8 لتوصيل القنية بخط الشريان في الدائرة الالتفافية.إجراء شق من خمسة ملليمترات في وسط خياطة سلسلة محفظة وضعت على cavae فينا وتمدد شق مع أداة صغيرة الزاوية. باستخدام 24-28 الفرنسية الزاوية اليمنى، قنينة الوريدية في مرحلة واحدة، cannulate فينا الكافا متفوقة والأدنى، وتوجيه الزاوية متفوقة في الكافا vena متفوقة ودونية في الكافا الوريدية السفلية، بعيدا عن القلب. تشديد tourniquet عقد خياطة سلسلة محفظة لتأمين كانولا، واستخدام موصل 3/8-3/8 1/2-Y لتوصيل القنولا إلى الخط الوريدي من الدائرة الالتفافية.ثم الشروع في الالتفافية القلبية. بعد بدء الالتفافية القلبية، استخدم أداة تشريح حادة وزاوية قائمة على اليمين لتطويق الوريد الهيميازيجوس الأيسر وligate الوعاء مع ربطة عنق O-silk. عبر المشبك الشريان الأورطي المتلقي proximally إلى القنية الشريانية ومأبوس على حد سواء فينا كافا مع العلاقات O-الحرير وضعت سابقا.قارن أحجام الكفة الأذينية اليسرى للمتبرع والمتلقي وعدّل كل منها حسب الضرورة. تسليم أول جرعة القلب الشللي إلى القلب المانح باستخدام قنية القلب التي وضعت سابقا. المقبل استخدام تقنية biatrial anastomotic القياسية وإبرة SH لوضع واحد 4-0 خياطة برولين عند تقاطع بين الأذين الأيسر والهامش السفلي الأيمن من الوريد السفلي cava.وضع خياطة أخرى في 180 درجة من خياطة الأولى لربط الأصفاد المانح والمتلقي، ثم إكمال أنستوموز الجدار الخلفي والأدارية من خياطة متفوقة إلى خياطة أدنى. بالنسبة للأذين الأيمن ، افتح الأذين الأيمن من الملحق نحو الوريد السفلي ، مما يخلق كفة متبرع يطابق حجم الكفة المتلقي ، ثم ، بدءًا من التقاطع بين الوريد السفلي والأذين الأيمن ، أكمل الجدار الداخلي للاستئصال ثم الجدار الجانبي. بالنسبة للشريان الرئوي، اقطع حواف كل من الشرايين الرئوية للمتلقي والشرايين الرئويين للمتبرع لإنشاء أحجام مطابقة.استخدم إبرة BB لوضع خياطة برولين 5-0 في الجدار الجانبي الأيسر، وتوحيد السفن المانحة والمتلقية، وأخرى عند الحافة الجانبيّة اليمنى. ثم إكمال أدنى جدار anastomosis وosistomosis الجدار الأمامي. بالنسبة للارهاد الأبهر، تقليم الحواف كما أظهرت للتو ووضع خياطة على الجدار الجانبي الأيسر الذي يربط بين السفن المانحة والمتلقية.ثم أكمل الجدار السفلي وosistomosis الجدار الأمامي قبل بدء ضخ 60 دقيقة على الالتفافية القلبية. للحصول على تقييم وظيفي للكسب غير المشروع، ضع ربطة عنق 0-حريرية حول الوريد السفلي cava وأدخل قسطرة إجراء بحجم الضغط في البطين الأيسر من خلال بطين صغير للسماح بقياسات مستمرة للعلاقات ذات حجم الضغط البطيني الأيسر. هنا تظهر النتائج التمثيلية من العلاقات بين الضغط وحجم والمعلمات صدى القلب التي اتخذت في خط الأساس، وفي ثلاث ساعات بعد زرع في مجموعة من خمس تجارب.أثناء عمليات التقييم الثابتة، يتم تسجيل بارامترات تعتمد على الحجم مثل المعدل الأقصى والأدنى للضغط المتطور. يتم الحصول على المعلمات المستقلة عن الحجم عن طريق انسداد متقطع من الكافا الوريدي السفلي الذي ينخفض خلاله حجم البطين الأيسر تدريجيا ويتم تسجيل العلاقات الضغط-الانقباضي ونهاية-الانبساطي- حجم. يمكن استخدام بيانات عمل السكتة الدماغية القابلة للتحميل قبل الحمل لتحديد العلاقة بين عمل السكتة الدماغية وحجم الانبساطي النهائي المطابق.يمكن أيضاً قياس مختلف المعلمات الأيضية الأخرى مع هذا النموذج. يتطلب هذا الإجراء جراحًا واحدًا على الأقل مدربًا وحوالي خمس تجارب عملية لتحسين بروتوكول البحث لكل مجموعة بحثية. القلب porcine هش، قابل للتفتيت، وحساسة بشكل خاص للتلاعب وهزيمي، وبالتالي يجري عرضة للدموع.من المهم مراجعة جميع الأورام لضمان الحد الأدنى إلى عدم النزيف. يمكن أن يساعد هذا البروتوكول في تطوير استراتيجيات جديدة للحفاظ على عضلة القلب لتقليل إصابة التناقص في الدم ومنع فشل الكسب غير المشروع الأساسي.
