臨床的に関連する大型動物モデルを用いて、新しい心臓保存戦略を評価し、信頼性の高い臨床翻訳を行うことが重要です。心臓手術の研究のためのブタの心臓モデルは、人間とブタの心臓が解剖学、生理学、さらには大きさの面で多くの類似点を共有しているので、研究のためのほとんど理想的なプラットフォームであるため、非常に有用なものです。麻酔付きドナー豚の心膜を開いた後、助手に右心室流出路を下に慎重に引き込み、肺動脈を左に肺動脈を慎重に引き込み、肺動脈への直接病変を避けるために大動脈を右に慎重に引き込む。大動脈肺空間を解剖し、結合組織から上昇大動脈の前側面をクリアするために、焼灼を使用してください。慎重に右心室流出管を引き込んだ後、4.0 PROLENE縫合糸を使用して、近位上昇大オルタアドベンティシアに財布のひもステッチを配置し、止血帯で縫合糸を固定します。財布のひも縫合糸の中の上にカーディオプレギア配達カニューレを挿入し、カニューレを固定するために止血帯を締めます。次に、左下肺静脈と上側および下静脈静脈を開いて、十分な心臓ベントを確保し、遠位上昇大動脈に大動脈クロスクランプを配置する。80~100ミリメートルの水銀の大動脈根圧を標的とした心筋圧注入を開始する。0.9%の塩化ナトリウム氷スラッシュを胸腔と器官の上に塗布します。心筋血管注入が終了したら、従来の方法で心切除術を進め、開始動脈と肺動脈の後に大動脈を切り離し、分岐で移植に十分な長さを確保する。レシピエントで心肺バイパスを開始するには、まずメッツェンバウムと直角鉗子を使用して、上の大静脈とインノミノ動脈と下胸静脈と麻酔付きレシピエント動物の心膜の間を解剖する。上手で下の大静脈を0シルクのネクタイで個別に囲み、止血帯でテープを固定します。4-0 PROLENE縫合糸を使用して、遠位上昇大オルタアドベンティシアに2つの同心円状の財布ひも縫合糸を配置し、心筋反射のレベルで劣った優れた静脈に財布のひも縫合糸を置きます。各縫合糸を止血帯で固定し、セルディンガー技術を使用して17-21フランスの動脈カニューレを大動脈に挿入します。3/8-3/8コネクタを使用して、カニューレをバイパス回路の動脈線に接続します。大静脈に置かれた財布のひも縫合糸の中心に5ミリメートルの切開を行い、小さな角度の楽器で切開を拡張します。24-28フランスの直角、単段静脈カニューレを使用して、優れた、より劣った大静脈カバをカヌレートし、優れた大静脈で優れた角度を導き、心臓から離れた下の静脈に劣っています。財布の弦の縫合糸を保持している止血帯を締めてカニューレを固定し、3/8-3/8 1/2-Yコネクタを使用して、カニューラをバイパス回路の静脈ラインに接続します。その後、心肺バイパスを開始します。心肺バイパス開始後、鋭い解剖器と直角を使用して左半接静脈を囲み、Oシルクのネクタイで血管を遠回しにします。受け手の大動脈を動脈カニューレに近応してクロスクランプし、以前に配置されたOシルクのネクタイで両方の静脈カバエをスネアします。ドナーとレシピエントの左心房カフサイズを比較し、必要に応じてそれぞれを変更します。以前に配置された心筋機能カニューレを使用して、ドナー心臓に最初の心筋薬量を送達する。次に、標準的なビアトライアルの麻酔技術とSH針を使用して、左心房と下の静脈の右下マージンとの接合部に4-0 PROLENE縫合を1つ配置します。ドナーとレシピエントカフを接続するために最初の縫合糸から180度で別の縫合糸を置き、その後および前壁のアナストモーゼを上縫合糸から下縫合糸に完成させます。右心房については、付属器から下側の静脈に向かってドナー右心房を開き、レシピエントカフサイズに一致するドナーカフを作成し、その後、下の大静脈と右心房の間の接合部から、内壁吻合と横壁を完成させる。肺動脈の場合、レシピエントとドナーの肺動脈の両方の縁を整え、一致するサイズを作成します。BB針を使用して、左側壁に5-0 PROLENE縫合糸を配置し、ドナーとレシピエントの血管を結び付け、もう1本を右横端に配置します。次いで、下壁吻湿と前壁吻湿を完了します。大大仏の場合は、ちょうど実証したように縁をトリミングし、ドナーとレシピエント容器を接続する左側の側壁に縫合を配置します。その後、下壁と前壁吻湿を完了してから、心肺バイパスで60分間の再灌流を開始します。移植片の機能的評価のために、下の大静脈の周りに0シルクのネクタイを置き、左心室の気圧容積関係の連続的な測定を可能にするために、小さな心房術を通して左心室に圧力体積伝導カテーテルを挿入する。ここでは、ベースラインで撮影した気圧容積関係および心エコーパラメータの代表的な結果、および5つの実験のセットでの移植後3時間での代表的な結果が示されている。定常状態評価の間、体積依存のパラメータは、発達圧力の最大および最小率など記録されます。体積非依存のパラメータは、左心室の体積が徐々に減少し、終期収縮期および拡張期末の圧力量関係が記録される下の大静脈の断続的な閉塞によって得られる。プリロード募集可能なストローク作業データを使用して、ストローク作業と対応するエンド拡張期容積との関係を決定することができます。このモデルでは、他の様々な代謝パラメータも測定可能です。この手順では、各研究グループの研究プロトコルを最適化するために、少なくとも1人の訓練を受けた外科医と約5回の練習実験が必要です。豚の心臓は壊れやすく、壊れやすく、特に操作や虚血に敏感で、涙を流しやすい。すべてのアナストモーゼを改訂して、出血を最小限に抑える必要があります。このプロトコルは、虚血-灌流損傷を減少させ、原発移植不全を予防するための新しい心筋保存戦略の開発に役立つ。
