Deze studie was gericht op het verhelmaken van een stap-voor-stap kniegewricht immobilisatie methode met behulp van een mini-invasieve techniek die snelle postoperatieve revalidatie in dieren na chirurgie toestaat. Gewrichtscontracturen worden gedefinieerd als een beperking van het passieve bewegingsbereik (ROM) van een diarthrodial gewricht1,2. De huidige therapieën gericht op het voorkomen en behandelen van gezamenlijke contractuur hebben succes bereikt3,4. Het onderliggende moleculaire mechanisme van verworven gezamenlijke contractuur blijft echter grotendeels onbekend5. De etiologie van gezamenlijke contracturen in verschillende sociale gemeenschappen is zeer divers en omvat genetische factoren, posttraumatische toestanden, chronische ziekten en langdurige immobiliteit6. Het wordt algemeen aanvaard dat immobiliteit een kritieke kwestie is bij de ontwikkeling van verworven joint contractuur7. Mensen die lijden aan grote gezamenlijke contractuur kunnen uiteindelijk resulteren in lichamelijke handicap8. Zo is een stabiel en reproduceerbaar diermodel noodzakelijk voor het onderzoeken van de mogelijke pathofysiologische mechanismen van verworven gewrichts contractuur.
De momenteel gebouwde immobilisatie-geïnduceerde kniegewricht contractuur modellen worden meestal bereikt door gebruik te maken van niet-invasieve werpt, externe bevestigingen, en interne fixaties. Watanabe et al. meldde de mogelijkheid van het gebruik van cast immobilisatie op rat kniegewrichten9. Door het dragen van een speciale jas, één kant van de onderste ledematen gewricht van de rat wordt geïmmobiliseerd door een cast. Het Rat kniegewricht kan volledig worden gebogen zonder chirurgische trauma10,11. Echter, zowel de heup en enkel gezamenlijke bewegingen worden ook beïnvloed door deze vorm van immobilisatie, die kan verhogen de mate van spieratrofie in de quadriceps musculus femoris of gastrocnemius12. Bovendien moeten oedeem en congestie van de achterledematen worden vermeden door de cast op ingestelde tijdstippen te vervangen, wat de continuïteit van immobiliteit kan aantasten. Een andere geaccepteerde methode voor de oprichting van een kniegewricht contractuur model is het gebruik van externe chirurgische fixatie. Nagai et al. gecombineerde Kirschner draad en staaldraad in een externe fixator, die het kniegewricht geïmmobiliseerd tot ongeveer 140 ° van flexie13. Bij deze methode wordt een hars gebruikt om het oppervlak te bedekken om krassen op de huid te voorkomen. Hoewel externe fixatie immobilisatie robuust en betrouwbaar is14,15, percutane Kirschner Wire PIN tracks kunnen verhogen het risico van infectie16. In onze eigen ervaring kan het gebruik van de externe fixatie techniek de dagelijkse activiteit van ratten verminderen als gevolg van een toename van het geconditioneerde lik gedrag.
Als alternatief Trudel et al. beschreef een goed geaccepteerde model van gezamenlijke contractuur in het Rat kniegewricht op basis van een chirurgische interne fixatie17 (deze methode werd gewijzigd door Evans en collega's18). Met name, deze methode benadrukt het belang van het gebruik van een mini-incisie techniek om de chirurgische wonden te minimaliseren. De efficiënte ontwikkeling van gezamenlijke contractuur is bewezen in dit model19. Het Protocol over het uitvoeren van een minimale dissectie om het botoppervlak bloot te leggen is echter nog onduidelijk20. Ook is de precieze positie waar de schroef boren niet volledig begrepen. De implantatie van de inwendige fixatie via een subcutane of submusculaire manier is nog steeds omstreden21. Om deze problemen op te lossen, hebben we deze methode aangepast door een passende scheiding van de spier kloof te maken, waardoor een mini-invasieve blootstelling van het botoppervlak en de plaatsing van de implantatie door een submusculaire kanaal mogelijk is. Dit protocol leidde tot snelle postoperatieve revalidatie bij ratten na de operatie. Dieren ontwikkelden een beperkt gezamenlijk bewegingsbereik na gewrichts immobilisatie, wat consistent was met morfologische veranderingen van de capsulaire adhesie verkregen uit de histologische analyse. We beschrijven ook een exacte mogelijke locatie van de geboorde schroeven, zoals bevestigd door Röntgen analyse of micro-CT-analyse. Dus, deze studie gericht op gedetailleerd beschrijven een minimale-invasieve techniek in een kniegewricht contractuur model dat werd vastgesteld door een spier-kloof scheidings modus gecombineerd met een mini-incisie methode. Wij zijn van mening dat minimaal invasieve technieken zowel dierlijk trauma kunnen verminderen als effectief het pathologische proces van joint flexie contractuur nabootsen.

a mini-invasive internal fixation technique for studying immobilization-induced knee flexion contracture in rats

Gezamenlijke contractuur, als gevolg van een langdurige gezamenlijke immobilisatie, is een gemeenschappelijke complicatie in orthopedie. Momenteel is het gebruik van een interne fixatie om knie gewrichts mobiliteit te beperken een algemeen aanvaarde model om experimentele contractuur te genereren. Echter, implanterende toepassing zal onvermijdelijk leiden tot chirurgische trauma aan de dieren. Gericht op het ontwikkelen van een minder invasieve aanpak, combineerden we een spier-kloof scheidings modus met een eerder gemelde mini-incisie vaardigheid tijdens de chirurgische ingreep: twee mini huid incisies werden gemaakt op de laterale dij en been, gevolgd door het uitvoeren van spier-gap scheiding om het botoppervlak bloot te leggen. Het Rat kniegewricht werd geleidelijk geïmmobiliseerd door een voorgebouwde interne fixatie bij ongeveer 135 ° knie flexon zonder storende essentiële zenuwen of bloedvaten. Zoals verwacht, deze eenvoudige techniek maakt snelle postoperatieve revalidatie bij dieren mogelijk. De juiste positie van de interne fixatie werd bevestigd door een x-ray of micro-CT scan analyse. Het bewegingsbereik was significant beperkt in het geïmmobiliseerde kniegewricht dan dat waargenomen in de contralaterale kniegewricht aantonen van de effectiviteit van dit model. Bovendien onthulde histologische analyse de ontwikkeling van de vezelafzetting en adhesie in de posterior-superieure knie gewrichtscapsule na verloop van tijd. Dit mini-invasieve model kan dus geschikt zijn voor het nabootsen van de ontwikkeling van geïmmobiliseerde kniegewricht contractuur.

We hebben geconstateerd dat ratten minimaal invasieve chirurgie kregen, kunnen slechts één dag na de operatie terugkeren naar het reguliere dieet. Deze eenvoudige en effectieve spier-gap scheiding model techniek combineren de manieren van mini-invasie methode is de beste interne fixatie in onze rat knie gewricht contractie model. Het belangrijkste voordeel van deze techniek is dat het proces reproduceerbaar is en het minimaal invasieve alternatief voor het induceren van contractuur van het kniegewricht bij dieren. Het aantonen van de procedure zal Shihai Jiang en Fei Zhang, afgestudeerde student van mijn laboratorium.Na bevestiging van een gebrek aan reactie op teen-pinch, scheren het onderlichaam van de verdoofde rat, met inbegrip van de twee achterste ledematen. Desinfecteer de blootgestelde huid twee keer met tinctuur povidone jodium en drie keer 75%ethanol. Breng zalf aan op de ogen van het dier en plaats de rat lateraal onder een chirurgische gordijn met een achterste ledemaat en heup blootgesteld.Desinfecteer het chirurgische gebied met extra povidone jodium. Aan het distale uiteinde van het dijbeen grotere trochanter, trek een lijn langs het lichaamsoppervlak projectie van de spierkloof tussen de vastus lateralis en biceps femoris om de richting van de huid incisie te markeren. Maak vervolgens een incisie van 1,5 centimeter in de opperhuid langs de lijn.Gebruik weefsel tangen om botweg ontleden de spierkloof tussen de vastus lateralis en biceps femoris tot ongeveer een centimeter van de dijbeen schacht wordt blootgesteld. Gebruik een rechte mug-type hemostatische klem om een submusculaire cursus te vormen van het scheenbeen gat naar het dijbeen gat, het passeren onder de gastrocnemius in het scheenbeen einde en boven de gluteus medius onder de biceps femoris in het dijbeen einde. Gebruik vervolgens een m1.4 bij acht millimeter stalen schroef om het ene uiteinde van een plastic plaat in het proximale dijbeen te beveiligen.Gebruik een m1.2 bij zes millimeter stalen schroef om een ander uiteinde van de plastic plaat in het distale scheenbeen te beveiligen. Wanneer de plaat is beveiligd, gebruik maken van 4-0 opneembare hechtingen om de myofascia, diepe fascia, en onderhuids weefsel te sluiten. Sluit de huid met zijden hechtingen.Röntgenbeeldvorming onthult een correcte plaatsing van de stalen schroeven in het dijbeen of het scheenbeen. Driedimensionale reconstructieanalyse na het scannen van microgecomputeerde tomografie met hoge resolutie toont de laterale plaatsing van de schroeven ongeveer acht millimeter onder de onderste rand van de grotere trochanter bij het proximale dijbeen en ongeveer vier millimeter onder de rand van de tibiofibular-fusie bij het distale scheenbeen. De arthrogene tekorten in de waaier van motieverhoging tijdens immobilisatie op een tijd-afhankelijke manier.Zoals blijkt uit de hogere gemiddelde arthrogene tekorten gemeten voor het geïmmobiliseerde kniegewricht in vergelijking met het controlekniegewricht op 56 dagen na immobilisatie. Op dag één van de immobilisatie wordt geen hechting waargenomen door elastische vlekken van de achterste superieure kniegewrichtscapsule in de gezamenlijke ruimte tussen de achterste-superieure gewrichtscapsule in het dijbeen in het geïmmobiliseerde of het contralaterale zijkniegewricht. Door 28 dagen van immobilisatie echter, fibroadipose weefselafzettingen en hechting ontwikkelen zich in de gezamenlijke ruimte van de geïmmobiliseerde knie.Na 56 dagen van immobilisatie worden de vezelige weefsels gedeeltelijk vervangen door de afzetting, terwijl dit type hechting niet wordt waargenomen in het contralaterale kniegewricht. Deze methode is gewijzigd van die gebruikt door professor Trudel en collega's van de Universiteit van Ottawa. Het is de sleutel tot het identificeren en scheiden van de spierhiaten tijdens de operatie.Na verschillende immobilisatietechnieken kunnen andere experimentele processen, zoals de cultuur van de kniegewrichtscapsule, worden uitgevoerd om de onderliggende moleculaire mechanismen te bestuderen.
