Verschillend van andere sequentiëren chemici, de sequencer hier beschreven gebruikt halfgeleider voor de detectie van nucleotiden. Het kiezen van levensvatbare embryo's van goede kwaliteit met normale chromosoom kopie nummers (euploïde) voor overdracht in geassisteerde reproductie helpt de zwangerschapsuitkomsten te verbeteren. Traditioneel wordt het gevestigde morfologische beoordelingssysteem op grote schaal gebruikt voor de evaluatie van embryo's vanwege de gemakkelijke beschikbaarheid en de niet-invasieve aard. Er is echter aangetoond dat morfologische beoordeling slechts beperkte informatie kan verschaffen over embryo kwaliteit1 en implantatie potentiaal2. Een fundamentele reden is het onvermogen om de chromosomale samenstelling van de embryo's te evalueren.
Chromosomale aneuploïdie (abnormaal kopie aantal chromosomen) is een van de belangrijkste oorzaken die tot embryonale ontwikkeling aanhouding, implantatie falen of zwangerschaps verlies leidt. Het optreden van aneuploïdie is goed gedocumenteerd in menselijke embryo's, waarbij 60% − 70% is opgenomen in het splijting-stadium embryo's3,4 en 50% − 60% in blastocysten5. Dit heeft tot op zekere hoogte bijgedragen tot het knelpunt bij het verbeteren van het zwangerschaps percentage van de behandeling in vitro fertilisatie (IVF), dat ongeveer 35% − 40%6,7heeft gehandhaafd. Daarom, het selecteren van euploïde embryo's voor overdracht wordt verondersteld te zijn gunstig voor het verbeteren van de resultaten van de zwangerschap. Hiertoe is een pre-implantatie genetische test voor aneuploïdie (PGT-A) verder ontwikkeld om de leefbaarheid van embryo's te onderzoeken aan de hand van genetische benaderingen. Er zijn toenemende aantallen gerandomiseerde gecontroleerde studies en cohort studies die de cruciale rol van PGT-A ondersteunen. Het is bewezen dat de toepassing van PGT-A vermindert de miskraam tarief en verhoogt de klinische zwangerschap tarief en implantatie tarief8, lopende zwangerschaps tarief en live geboortecijfer9.
Historisch gezien zijn er verschillende methoden toegepast in PGT-A, zoals fluorescentie in situ-hybridisatie (vissen), vergelijkende genomische hybridisatie (CGH), array-CGH en single nucleotide polymorfisme (SNP)-Microarray. Uit eerdere studies is gebleken dat PGT-A voor de splijter fase van embryo's door vissen resultaten oplevert die slecht consistent zijn met die welke zijn verkregen door uitgebreide chromosomale screening (CCS) van overeenkomstige blastocysten met 59273array-cgh of SNP-microarray5927310. Deze verschillen kunnen worden toegeschreven aan chromosomale mosaïsme, vis technische artefacten, of embryonale zelf correctie van chromosoomsegregatie fouten tijdens de ontwikkeling11. Het is alom erkend dat het gebruik van blastocyst trophectoderm (te) biopsieën voor array-gebaseerde PGT-A zoals array-cgh of SNP-Microarray effectief is voor het identificeren van de chromosomale onbalans in embryo's10,12. Onlangs biedt single-cell sequentiëren van de volgende generatie (NGS) een hoge doorvoer en een kosteneffectieve aanpak voor genetische analyse en heeft het klinische toepasbaarheid aangetoond in PGT-a13,14,15, waardoor het een veelbelovend alternatief voor de momenteel beschikbare methoden.
Hier presenteren we een snelle, robuuste en goedkope NGS-methode op basis van halfgeleider-sequencing voor het screenen van aneuploïdie in menselijke embryo's. De eerste stap van de workflow is Whole genoom Amplification (WGA) van het biopsied embryo specimen, met behulp van een WGA Kit met één cel, gevolgd door de bouw van sequentiëren bibliotheek, en opeenvolgende sequentiëren op het halfgeleider volgorde systeem.
Door het detecteren van de H+ -ionen die vrijkomen uit elke deoxyribonucleoside trifosfaat opname tijdens de DNA-strand synthese, draagt het systeem de chemische signalen (pH-verandering) over die door de halfgeleiderelementen worden opgevangen naar directe digitale gegevens , die verder worden geïnterpreteerd in DNA sequentie-informatie. Het elimineren van de eis voor dure optische detectie en complexe sequentie reacties, deze eenvoudige sequentie chemie vermindert de totale reagens kosten en verkort de sequentiëren looptijd in 2 − 4 uur16. Wat nog belangrijker is, op basis van de prestatiespecificaties van de fabrikant, kan het platform voor het sequentiëren van halfgeleider tot 15 GB aan sequentiegegevens genereren (afhankelijk van de kwaliteit van de bibliotheek) per run, wat aanzienlijk hoger is dan sommige van de andere sequencers produceren slechts ongeveer 3 − 4 GB gegevens (met 2 x 75 BP Lees lengte)17. In klinische toepassingen van PGT-A, dit platform kan bereiken 24 monsters per chip genereren tot 80.000.000 leest17 en ten minste 1.000.000 unieke leest van elk monster. De Lees diepte kan ervoor zorgen dat elk monster ten minste 0,05 x hele genoom dekking heeft. De bovenstaande voordelen van dit platform maken het een ideale screeningsmethode en faciliteren zo de brede toepassingen in PGT-A18.

Chromosomale aneuploïdie, een van de belangrijkste oorzaken die leiden tot de arrestatie van embryonale ontwikkeling, implantatie falen of zwangerschaps verlies, is goed gedocumenteerd in menselijke embryo's. Preimplantatie genetische testen voor aneuploïdie (PGT-A) is een genetische test die de voortplantings resultaten aanzienlijk verbetert door chromosoomafwijkingen van embryo's te detecteren. Next-generation sequencing (NGS) biedt een hoge doorvoer en een kosteneffectieve aanpak voor genetische analyse en heeft klinische toepasbaarheid aangetoond in PGT-A. Hier presenteren we een snelle en laaggeprijsde NGS-methode op basis van halfgeleider-sequencing voor het screenen van aneuploïdie in embryo's. De eerste stap van de workflow is Whole genoom Amplification (WGA) van het biopsied embryo specimen, gevolgd door de bouw van sequentiëren bibliotheek, en opeenvolgende sequentiëren op het halfgeleider volgorde systeem. Over het algemeen kunnen voor een PGT-A-toepassing 24 samples worden geladen en gesequentieerd op elke chip die 60 − 80 miljoen leesbewerkingen genereert met een gemiddelde lengte van 150 basis paren. De methode biedt een verfijnd protocol voor het uitvoeren van sjabloon versterking en verrijking van sequentiëren bibliotheek, waardoor de PGT-A-detectie reproduceerbaar, hoge doorvoer, kostenefficiënt en tijdbesparend. De looptijd van deze halfgeleider-sequencer is slechts 2 − 4 uur, verkorting van de doorlooptijd van het ontvangen van monsters voor het uitgeven van rapporten in 5 dagen. Al deze voordelen maken deze test een ideale methode om chromosomale aneuploidies uit embryo's op te sporen en zodoende de brede toepassing ervan in PGT-A te vergemakkelijken.

Op basis van dit gewijzigde protocol werd het platform voor halfgeleider sequencing voor het eerst toegepast voor PGT-A. We testten op biopsieën van zowel de decolleté-stage blastomeres en blastocyst-stage embryo's. Er wordt gesuggereerd dat de biopsied cellen zo snel mogelijk een WGA ondergaan om eventuele afbraak van DNA te voorkomen. Hier introduceren we een methode van halfgeleider sequencing voor pre-implantatie genetische testen voor aneuploïdie (PGT-a) met de voordelen van korte doorlooptijd, lage kosten en hoge doorvoer.

Het protocol dat we hier presenteren is een snelle en goedkope halfgeleider, sequencing-gebaseerde methoden voor de screening van aneuploïdie in embryo's. Het verfijnde protocol voor het uitvoeren van sjabloonversterking en de indelingen van de sequencing-bibliotheek maken de PGTA-detectie reproduceerbaar, hoge doorvoer, kostenefficiënt en tijdbesparend. De runtime van deze halfgeleidersequent is slechts twee tot vier uur.Verkorting van de doorlooptijd van het ontvangen van monsters tot het uitgeven van rapporten in vijf dagen. Deze methode biedt genoomsequencing voor aneuploïdiescreening, kopienummervariatie en enkele nucleotide polymorfismeroepingen. Vanwege verschillende sequencing chemie, kan de bibliotheek die door dit protocol wordt voorbereid niet direct op andere sequencing systemen worden gesequenced.Maar dezelfde sequencer, maar verschillende bibliotheek voorbereiding, het is in staat om niet-invasieve prenatale screening te doen. Bereid vervolgens een 2400 microliter ligatiemix, door 172 microliter nuclease-vrij water, acht microliters van de gemengde bibliotheek, 120 microliter van de enzymmix en 100 microliter van de boldeeltjes toe te voegen aan de buis met 2000 microliter van de mastermix PCR-buffer.Stel een pipet in op 800 microliters en laad het ligatiemengsel op het reactiefilter via de monsterpoort. Keer de oliefles drie keer om en gebruik een P1000 pipet om 200 microliter van de reactieolie aan het reactiefilter toe te voegen. Centrifuge de nulpunt twee milliliter buis weer. Gooi de supernatant weg en bewaar 10 microliter van het verrijkingsproduct.Voeg 90 microliter nuclease-vrij water toe aan de buis en meng door vortexing. Om de sjabloon voor te bereiden, vortex de positieve controle en spin kort. Voeg vijf microliter van de positieve controle toe aan 100 microliter van het verrijkingsproduct.Vortex en centrifuge op 15, 500 keer G gedurende vijf minuten. Gooi de supernatant weg en bewaar 10 microliter van de sjabloon. Voeg vervolgens 20 microliter van de sequencing primer en 15 microliter van de annealing buffer toe aan de sjabloonbuis.Vortex de buis en kort draaien op een mini centrifuge voor drie seconden. Broed de buis in een thermische cycler met een verwarmd deksel. Voeg vervolgens 10 microliter van de laadbuffer toe aan de buis.Meng door pipetting op en neer. Meng de 55 microliter van het monster door pipetting op en neer en laad het monster aan de lading goed van de chip. Houd de gebruikte pipettip en de nulpunt twee milliliter PCR-buis.Plaats de chip op de chip mini centrifuge wanneer een monster in de chip. Controleer de positie en centrifugeer de chip gedurende 10 minuten. Bereid 50% annealing buffer door het toevoegen van 500 microliter van annealing buffer en 500 microliters van nuclease-vrij water in een een punt vijf milliliter buis.Bereid de spoeloplossing voor, door 500 microliter annealing buffer en 500 microliter van 100% 2-propanol toe te voegen aan een één punt vijf milliliter buis. Bereid vervolgens het schuimmengsel voor, door 49 microliter van de 50%-annealingbuffer en één microliter van de schuimoplossing toe te voegen aan twee nieuwe éénpuntsvijftig milliliterbuizen. Pipette lucht in een van de schuimende mengsels, en laad 120 microliter van de bellen in de lading put.Breng de overmatige verdreven vloeistof van de uitgang goed naar de lading goed, en centrifuge de chip voor 30 seconden op de chip mini centrifuge. Herhaal het proces voor het tweede schuimende mengsel. Herhaal deze laadstap één keer. Laad 100 microliters van de 50%annealing buffer, in de chip belasting goed.Herhaal deze laadstap in totaal drie keer. Voeg 60 microliter van de 50% annealing buffer en zes microliters van de sequencing enzym in een nieuwe one point vijf milliliter buis. Meng door pipetting op en neer.Langzaam pipette 65 microliters van deze gemengde oplossing in de chip goed laden, het vermijden van bellen. Houd de chip uit de buurt van licht en incubbateren op kamertemperatuur gedurende vijf minuten. Tot slot, na de incubatie, laad onmiddellijk de chip op de sequencer en klik op Start The Sequencing Run op het scherm om te beginnen met sequencing.Een retrospectieve statistische analyse van 186 decolletéfase en 1135 blastocyst fase embryo's resulteerde in meer dan 95% WGA slagingspercentages in beide soorten monsters. Sequencing kwaliteitscontrole mislukking tarieven waren 3,4% in de decolleté fase groep en slechts 1,9% in de blastocyst fase groep. Naast aneuploidy kunnen de sequencing gegevens worden gebruikt voor andere analyses.Bijvoorbeeld, kopieer nummer variaties met een grootte minder dan vijf megabase, of myconriaal DNA-analyse, afhankelijk van de volgorde diepte en kwaliteit. Met de verbetering van het bellen van het aantal exemplaren kunnen onderzoekers de toekomst van chromosomale mozaïekisme in vroege embryo's in het vroege stadium van de mens verder onderzoeken.
