Sepsis initieert een complex samenspel van gastheer pro-inflammatoire en anti-inflammatoire processen en wordt gekenmerkt door een hyperinflammatory reactie en de daaropvolgende immuun dysfunctie1,2. Sepsis vertegenwoordigt een globale gezondheids prioriteit en veroorzaakt een hoog aantal sterfgevallen in intensieve zorgeenheden (Ic's)3. De incidentie van sepsis wordt geschat op meer dan 30.000.000 gevallen wereldwijd per jaar, met sterftecijfers zo hoog als 30%, ondanks de vooruitgang in de ICU Management4,5. In 2017, de Wereldgezondheidsorganisatie een resolutie aangenomen ter verbetering van de preventie, diagnose en het beheer van deze dodelijke ziekte5. Recente studies hebben echter geïllustreerd dat de dood niet het gevolg is van primaire infectie bij ernstige septische patiënten, maar eerder van secundaire ziekenhuis infectie (met name longontsteking) die veroorzaakt door immunosuppressie6,7 . Daarom, het begrijpen van de mechanismen van waarom septische patiënten ontwikkelen secundaire infectie en het ontdekken van effectievere behandelingen zijn dringend vereist. Hierin, een tweevoudig model, ook wel bekend als een double-hit model, om de immunosuppressieve verschijnsel dat zich voordoet bij patiënten met langdurige sepsis studie wordt beschreven.
Als de gouden standaard experimentele model in onderzoek naar polymicrobiële sepsis, cecal afbinding en punctie (CLP) is een operatie gekenmerkt door blindedarm afbinding en perforatie, die bijdraagt aan polymicrobiële peritonitis en sepsis8,9 . De pathofysiologische proces en cytokine profielen, samen met de kinetiek en de omvang, zijn vergelijkbaar met klinische sepsis. De positie van de afbinding, de naald grootte die voor de punctie wordt gebruikt, en het aantal cecal puncties zijn belangrijke factoren die de mortaliteit na CLP beïnvloeden.
De ziekenhuis ontsteking is de belangrijkste oorzaak van sterfte bij kritisch zieke patiënten met sepsis. Het belangrijkste type van organismen veroorzakend strenge sepsis omvat Staphylococcus aureus (20,5%), Pseudomonas soorten (19,9%), Enterobacteriacae (hoofdzakelijk E. coli, 16,0%), en paddestoelen (19%). Ondertussen, hebben de recente studies een stijgende weerslag van gram-positieve organismen voorgesteld, die nu bijna net zo gemeenschappelijk zoals gram-negatieve besmettingen3zijn.
De methode beschreven in dit protocol omvat CLP, uitgevoerd als de "eerste hit" te induceren subdodelijke polymicrobiële peritonitis, die een immunosuppressie voorwaarde manifesteert. Sepsis, een ernstige en ingewikkelde levensbedreigende infectie, wordt gekenmerkt door een onbalans tussen Pro-en anti-inflammatoire reacties in meerdere organen. Met de ontwikkeling van therapieën, overleven de meeste patiënten de hyperinflammatory fase maar vorderen aan een immunosuppressieve fase, die de totstandkoming van secundaire besmettingen verhoogt. Daarom is een beter begrip van de pathogenese onderliggende secundaire ziekenhuis-verworven infecties in de immunosuppressieve fase tijdens sepsis is van enorm belang. Gemeld hier is een model om besmettelijke resultaten te testen door het creëren van Double-hit infecties bij muizen. Een standaard chirurgische ingreep wordt gebruikt om te induceren polymicrobiële peritonitis door cecal afbinding en punctie (CLP) en gevolgd door intranasal infectie van Staphylococcus aureus te simuleren longontsteking die zich in het immuunsysteem onderdrukking die vaak wordt gezien bij septische patiënten. Dit dubbele model kan weerspiegelen de immunosuppressieve toestand die zich voordoet bij patiënten met langdurige sepsis en de gevoeligheid voor secundaire infectie van ziekenhuis ontsteking. Bereid de muis voor op een operatie. Scheer de onderste kwadranten van de buik met behulp van een scheermes en desinfecteer het gebied met jodium. Plaats de muis op een schoon werkoppervlak op de rug met het hoofd dat van de bediener wijst.Maak een longitudinale huid incisie ongeveer een halve centimeter lang in de linker onderbuik. Zorg ervoor dat u de buikholte niet binnendringt. Gebruik een kleine schaar om de incisie ongeveer een tot twee centimeter uit te breiden.Maak een intramusculaire incisie om toegang te krijgen tot de buikholte. Muizen begonnen te sterven rond 12 uur na inductie van peritonitis.De mortaliteit was veel hoger voor muizen met CLP voor een longontsteking. Drie dagen na de operatie hadden CLP-muizen meer sterfte met een intranasale toediening van staphylococcus aureus. De niveaus van serum pro-inflammatoire cytokinen aanzienlijk toegenomen op 24 uur na bacteriële instillatie in sepsis muizen in vergelijking met de muizen die CLP alleen had of de Sham geopereerde muizen met een tweede hit.Echter, double-hit muizen vertoonden een aanzienlijke daling van bal vloeibare cytokine niveaus in vergelijking met zowel SA en Sham chirurgie plus SA muizen. Vandaar CLP schaadt gastheer verdediging en verhoogt de gevoeligheid voor infectie. Dit dual-hit model is een waardevol hulpmiddel om de moleculaire mechanismen voor gastheer-pathogene interactie te begrijpen, die de weg kunnen openen om nieuwe, effectieve geneesmiddelen en andere therapieën voor superbug infecties te ontwikkelen.
