ネストされたRT-PCRは、承認されたすべての16のICTVおよび2つの新しいリッサウイルス種を含むすべてのリサウイルスを検出するために開発されました。この方法の検証は、中国における臨床狂犬病の診断と監視の10年間で9000以上の動物の脳標本を使用することによって証明されています。入れ子になったRT-PCRは、新鮮な脳組織サンプルだけでなく、劣化したサンプルにも信頼できる結果を生み出すことができます。この方法の浸潤は、脳脊髄液、唾液、尿などの生体液検体をアッセイするために使用することができ、それによりこの方法を最後通告診断に適用することができる。PCRの持ち越し汚染を最小限に抑えるためには、負および肯定的なコントロールを準備することが重要です 対策を講じるのは本当に重要です。例えば、各RNA抽出の必要な準備、PCRのセットアップ、ゲル電気泳動は物理的に別々の領域で操作する必要があります。また、ペプチド鎖への狂犬病汚染を防ぐために、RNA抽出および逆転写にスパイラルテープを使用することも推奨されます。氷上での薬剤の調製、手袋の交換、リボヌクレアーゼフリー材料の使用などの適切な取り扱いにより、リボヌクレアーゼの導入を防止し、RNAの分解を排除します。この手順を開始するには、狂犬病の疑いのある組織、皮膚生検、唾液、脳脊髄液またはRABV感染細胞培養からRNAを抽出する。準備ができたら、必要な試薬を冷凍庫から取り出し、氷の上に保管します。使用前に各試薬を解凍し、渦を出してください。ウイルスRNAのcDNAへの逆転写については、清浄な状態でのテキストプロトコルの表1に概説されているように、氷上の各サンプルに対して12マイクロリットルの反応ミックスと1.5ミリリットルの遠心分離管を調製する。ピペッティングのバリエーションを考慮するために、マスターミックスの少なくとも1つの必要以上の大きさの反応を準備し、その後、0.2ミリリットルのPCRチューブに反応ミックスを分割します。テンプレートルームのPCRワークステーションで、サンプルまたはコントロールの1つの8マイクロリットルを反応ミックスに追加します。陽性対照は、固定RABV株CVS11に感染した細胞培養物から抽出されたRNAであり、陰性対照にはRnaseフリーの二重蒸留水が含まれる。最初の渦を、次に短い遠心分離によって各管の内容物を混ぜます。この後、反応管をサーモサイクラーに積み込みます。テキストプロトコルで概説されているように、cDNA合成プログラムを設定して実行します。まず、必要な試薬を取り出し、使用できる状態になるまで、クリーンルームで氷の上に保管します。準備ができたら、試薬を解凍し、渦を出します。次に、テキストプロトコルの表2に示すように、各サンプルに対して48マイクロリットルの第1ラウンドPCRミックスを1.5ミリリットル遠心管で調製し、反応ミックスを0.2ミリリットルPCRチューブに分割します。テンプレートルームの PCR ワークステーションで、cDNA の 2 マイクロリットルサンプルを 1 回目の PCR ミックスに追加します。正の対照としてCVS11 cDNAの2マイクロリットル、負の対照として二重蒸留水の2マイクロリットルを含める。次に、テキストプロトコルの表3に示したパラメータを使用して、密封チューブをPCRサーモサイクラーに移し、サイクルします。まず、テキストプロトコルの表4に示すように、サンプルごとに48マイクロリットルの第2ラウンドPCRミックスを1.5ミリリットル遠心管に調製し、反応ミックスを0.2ミリリットルPCRチューブに分割します。第 1 ラウンド PCR 産物の 2 マイクロリットルを第 2 ラウンド PCR ミックスに追加します。この PCR ラウンドの負のコントロールとして二重蒸留水を含めます。次に、テキストプロトコルの表3に示したパラメータを使用してPCRサーモサイクリングを実行します。まず、1.5グラムのアガロースをトリス酢酸EDTAの100ミリリットルに加え、電子レンジで加熱して十分に溶解します。次に、0.01%の最終濃度で臭化エチジウムを加え、ゲルを金型に注ぎ、少なくとも30分間周囲温度で固化させます。各PCR産物の5マイクロリットルを6xローディングバッファの1マイクロリットルと混合して、ローディングサンプルを準備します。別にサンプルと適切なDNAマーカーをウェルにロードし、染料ラインがゲルを約75〜80%下げるまで120ボルトで約30〜45分間ゲルを実行します。実行が完了したら、電源を切り、電源から電極を取り外します。その後、ゲルボックスからゲルを慎重に取り除きます。UVゲルの文書化装置を使用して、DNA断片を可視化し、撮影します。テキスト プロトコルで説明されているように、入れ子になった逆転写 PCR を特徴付けます。18種のリサウイルス種を検出する代表的なネストされたRT-PCRの結果をここに示す。PCR陽性制御は、第1ラウンド増幅で845塩基対、第2ラウンドの371塩基対で予想されるバンドを示す。負のコントロールにはバンドが表示されません。18個のリサウイルスはすべて、陽性対照と同じ2つのバンドを産生することが分かっているため、入れ子になったRT-PCRが18個のリサウイルスすべてを検出したことを示している。プラスミドの16は2ラウンドのPCRで効率的な増幅を有するが、そのうちの2つ、アラバンウイルスおよび生駒ウイルスは、それぞれ第1または第2のラウンドでのみ増幅される。この方法の感度は、異なるリサウイルスプラスミドの検出において、マイクロリットル当たり2.24~240分子の範囲の値からさまざまです。これらの違いは、ウイルス配列の多様性によるプライマーとテンプレートの間の不一致に起因する可能性があります。入れ子になったPCRによってテストされた臨床検体の9,624個の脳組織とFATでテストされた組織の比較は、ネストされたRT-PCRが100%の感受性および99.97%の特異性であることを示し、2つの方法間の適合性は99.07%の他の狂犬病研究所がネストされたRT-PCRをさらに検証するためにテストを行うように招待された。10の研究所は、すべて、偽陰性や偽陽性を持たないFATに従って100%の入れ子になったRT-PCRの結果を得て、入れ子になったRT-PCRは高い特異性と再現性を有することを示す。狂犬病技術を使用するために、OIEがRT-PCRを推奨するようになりました。そして、私たちの入れ子になったRT-PCRは、2018年に中国の狂犬病技術の国家標準として証明されています。
