この方法は、右心室障害のメカニズムを研究するための有用な技術を提供する。この技術の主な利点は、時間を節約し、手術の成功率を増加させることです。この技術は、肺動脈の収縮によって右心肥大およびヒトの障害を模倣し、肺動脈圧過多を作り出すのに役立つ。この方法は、横断大動脈収縮技術に適用することができ、ポスト心臓移植合併症、および先天性心疾患研究への洞察を提供することができます。この手順は、私の研究室の技術者であるXiaoxia Huangによって実証されます。手順を開始する前に、1ミリリットルのシリンジから分解した25ゲージの針を通して6-0編み込みシルク縫合糸の一部を渡します。針を90度曲げるために止血鉗子を使用してください。その後、22ゲージ針を120度曲げます。つまみつまみへの応答の欠如を確認した後、麻酔マウスを37°Cの加熱パッドの上の上のsupine位置に置き、縫合糸で切歯を固定します。手足をテープで固定し、脱毛クリームを使用して首からxiphoidプロセスに髪を取り除きます。露出した皮膚をシーケンシャルヨウ素と75%エタノールワイプで消毒し、尖ったペンチを使用して舌をわずかに抽出します。グロティスを露出させるためにカスタムメイドのヘラで下顎を高め、そして、グロティスを通してカスタムメイド気管カニューレの慎重な挿入を指示するために冷たい光源を使用しています。ミニ換気装置のパラメータを調整し、呼吸数を毎分150呼吸、潮量を300マイクロリットルに設定します。次に、チューブと人工呼吸器を接続して、カニューレがテープで固定する前にカニューレが正しく挿入されていることを確認します。胸部を開くには、約10ミリメートルの皮膚切開を第2肋骨に平行にする。はさみと肘のピンセットを使用して、2番目の肋間空間の上の大胸筋と軽度の筋肉を分離します。胸骨の角度からリブを数えて、第2肋間空間を特定し、はさみを使用してこのスペースを開きます。その後、肺幹が見えるまで、パレンチマと胸腺をぶっきらぼうに分離する。肺動脈を収縮させるためには、肘ピンセットを使用して、肺幹と上大動脈をぶっきらぼうに分離します。90度ラッチ針を使用して、肺幹と上昇大動脈の間の結合組織を通して縫合を通過させます。肺幹に120度のパディングニーを置きます。6-0編組シルク縫合糸を使用して、パディング針を肺幹にリゲートし、肺の孔を充填した直後にパディング針を取り除くことは観察される。縫合糸の両端を切断し、収縮が存在するかどうかを評価するために肺の孔の充填を観察し続ける。その後、再び動物のペダル反射を評価して、ライゲーションの成功を確認し、5-0ナイロン縫合糸で胸と皮膚を閉じます。手術後少なくとも3日後、肺動脈収縮動物の胸部に超音波ゲルを適用する。30メガヘルツプローブを左寄縁線に相対する30度反時計回り位置に胸部に配置し、尾部方向にノッチを向けます。僧帽弁、大間、左心室がはっきりと見えるまで、Bモード下でY軸とX軸を調節します。プローブをそのY軸で胸部に向かって30~40度回転させます。肺の孔がはっきりと見えるまでY軸とX軸を調節し、肺弁リーフレットの先端にカーソルを置いてピーク流速を測定する。色とPWを選択し、脈波破線を血流の方向に平行に配置します。右側よりも高くなるようにパッドの左側を調整し、プローブの方向を右内側の腋線に沿って水平線に 30 度設定し、ノッチを尾部方向に向けます。右心室がはっきりと見えるまで X 軸と Y 軸を調整します。次に、Mモードを2回クリックして、インジケータラインを表示します。手術の8週間後、デモンストレーションのようにマウスを挿管し、横隔膜を通してxiphoidプロセスの下に1.5センチメートルの両側切開を行い、胸腔を開く。肋骨を切り抜いて心臓を露出させ、23ゲージの針を使用して右心室の自由な壁を貫通する。穿刺ポイントを綿棒で軽く押して出血を止め、カテーテル先端を使用して創傷を通して心室を穿刺します。曲線が安定している場合は、右心室流出路に向かってカテーテルの先端を調節する。右心室の収縮期血圧と拡張期血圧、上昇率、指数関数的な緩和の時定数、心拍数を10分間記録するために、選択して分析します。次に、カテーテルを心臓から生理的な容器に移します。シャム群と比較して、より高いピーク速度、より大きな圧力勾配、およびより大きな右心室壁厚さは、肺動脈収縮群の手術後8週間のパラスターナル長軸図から得られる。さらに、右心室の収縮機能は、手術後8週間のシャム群と比較して、肺動脈収縮群において有意に低下する。肺動脈収縮群は収縮期および拡張期のより高い右心室圧を有し、そして収縮指数はシャム群のそれに比べて減少する。右心室タウと最大上昇率は、シャム群よりも肺動脈収縮群でも大きい。右心室機能障害が8週間の肺動脈バンディングの後にマウスで誘発されることを実証する。右心室での侵襲的な血行力学的検査は、心拍数がカテーテルモニタリングの前後に安定したままであることを明らかにする。シャム群と比較して、右心室の寸法は著しく拡大され、右心室重量は高い。さらに、組織学的検査は、心筋細胞がカバーする領域における心臓線維症が、シャム群よりも肺動脈収縮群においてより大きいことを示している。手順の最も重要な部分は、肺動脈が正しく完全に分離されていることを確認することです。間違った分離を避けるために、分離点は、動脈間の結合組織が緩くなる肺動脈の枝の近くにあるべきである。この方法により、右心室肥大を誘導するためのより便利で効率的なアプローチを提供しました。一般的に、外科的処置および評価アプローチは非常に安全である。
