Con A注射は、密なリンパ球浸潤を特徴とする涙腺で強い炎症反応を誘発した(図13)、涙液産生の減少を伴う。すべての破傷パラメータが著しく変更されました(表1および表2)。さらに、涙液ラクトフェリンレベルが抑制された(コントロール= 3.1±0.45対コンA注入= 2.7±0.02 ng/mgタンパク質(平均±SEM);p<0.03)。最終的な結果は、角膜および結膜上皮を損ない、バラベンガル染色の増加によって証明された(図6)。
3つの軌道LG組織の注入は、以前の方法18、27によって達成された状態とは異なり、一貫した均一なDED状態を生じた。この結果の主な貢献者は、ILGの米国ガイド注入とOSLGの注入でした。表1は、この方法の顕著な結果をまとめたものである。すべての変更は、重大な DED と一致しています。
コン A 注射の単一のセットは、約 1 週間持続する DED を生成します。;すべての臨床パラメータは10日目(表2)によって正常化します。シーケンA注射は、約1週間離れてDEDの持続時間を延長します。たとえば、7 日目の 2 番目の Con A 注射では、2 週間の DED が維持されます。約5セットの注射後、DED状態は、多くの場合、さらなる注射を必要とせずに永久的になります。
コンA誘導DEDを有するウサギを新しい薬剤ホスホススリンダックで治療した際、この疾患を著しく抑制した。たとえば、 この薬剤TBUTによる1週間の治療に続いて、車両処理動物(43.6±4.0対12.2±2.8s;p<0.001;平均±SEM、これらの値および以下の値)と比較して著しく増加し、涙の浸透度は正規化された(2944±4.6対311±2.0mM/L&0;l2)。機械学的には、ホスホスリンダックは、IL-1β(8.4±1.2対21.2±6.6 pg/mgタンパク質;p<0.03)およびIL-8(4.9±1.7対13.5±5.0 pg/mgタンパク質;p<0.05)の2つの重要なインターロイキンのレベルを低下させる。

我々の方法は、すべての涙腺を注入し、注射を繰り返す我々の方法のおかげで急性または慢性することができるドライアイ病の不足している信頼性の高い動物モデルを提供します。この技術は、広く可変サイズの腺に超音波ガイダンスを使用して、この方法を完全なアプローチにする優れた涙腺まで、劣った涙腺にコンカナリンAを確実に提供する。このモデルは、水欠乏性のドライアイ病を誘発する、シンプルで最適化された非外科的手段を提供する。薬効や病態生理学の研究に適しています。後部の不安を特定し、超音波の局在化を利用することは、最初は困難な場合があります。すべての毛皮を取り除くことは、これらのステップの両方のための超音波の局在化を助けるために重要である。この手順を実演するのが、私の研究室の大学院生であるウェイ・ホアンです。耳葉が完全に直立しなくなったリラックスした頭部位置を観察することによって軽度の鎮静を確認した後、マイクロピペットを使用して25マイクロリットルの防腐剤を含まないリドカインを最初の目に塗布し、まぶたの間に柔軟なワイヤー蓋鏡を配置する。0.3鉗子を使用して、その頂点でニクタンス膜をつかみ、角膜の上にそれを伸ばします。26ゲージの鋭い針を使用して、リドカイン1%を1:100,000番目のエピネフリンの結合膜の基部に注入する。適度なブレは膜の上に形成されるべきである。その後、鏡を取り除き、左nictating膜に同じ注射を行います。約5分後、蓋鏡を仲間の目に戻し、0.3鉗子を使ってその頂点のニッキング膜を引き込みます。ウエストコットはさみを使用して、そのベースでニクタンス膜を切断し、鏡を取り除きます。その後、目に局所抗生物質軟膏を置きます。上皮腺注射のパルペブラル部分の場合、沈殿を確認した後、前眼窩および頭皮領域の毛皮を剪断し、脱毛クリームを使用して残留毛皮を完全に除去する。2分後にクリームを取り出し、適切な目に1%防腐剤を含まないリドカインの25マイクロリットルを塗布します。上まぶたをエグレットし、腺の手のひらの部分を示す突起が見えるまで後眼窩縁に穏やかな内側圧力を加える。細かい歯の鉗子と27ゲージ針を装備した結核シリンを使用して、 針を2ミリメートル進める経結膜アプローチで直接組織に侵入し、注射直後にコンカナバリンAまたはCon A.の500マイクログラム溶液の100マイクロリットルを注入し、地球に内側圧力を加えて軌道上の涙腺を後部から突き出させる。閉じた湾曲した鉗子を使用して、頭蓋骨の骨の開口部が感じられるまで領域をインデントし、鉗子でさらに控えめな圧力を加えて、針の配置のランドマークとして皮膚にくぼみを残します。約4分の1インチのインデントマークの上に皮膚に垂直な27ゲージ針を装備した結核注射器を挿入し、注射部位と骨軌道リムの中間点を目指す横カンサスに向かって後部および外部に針をリダイレクトする。針のハブに達したら、ゆっくりとILG注射のためのCon A.の1,000マイクログラム溶液の0.2ミリリットルを注入し、眼窩の下前部に沿ってILGの目立つ位置を見つけるために側面から動物を見る。外科用マーキングペンを使用して、ILG腺の表面的な部分がザイゴマ骨の表面的な休息場所から軌道上のより内側の位置に移行する皮膚に垂直線を引きます。垂直に保持された超音波プローブを皮膚のラインを横切って掃引し、ザイゴマ骨の端を特定する。ILG 遷移は、周囲の高響部の線から認識可能な内側の境界線のない線に腺のイメージが変化する場合に発生します。この変化が観察されたときに皮膚に描かれた線に対するハンドピースの相対位置は、注入部位となる。コンAをザイゴマチックアーチの骨の内側のポイントに腺に配置するには、必要な注入深さを、ザイゴマ骨超響信号の深さに加えて、針の既知の長さを1ミリメートル引いたものとして設定します。注射部位の腺に約12ミリメートルの針を挿入してから、身体外の露出針の長さが計算された差と等しくなるまでゆっくりと引き出します。その後、Con A溶液の1,000マイクログラムの0.2ミリリットルを注入し、超音波による注射の成功を確認する。ILGは、特徴的な超響性空間を示すべきである。A注射は涙液産生の減少を伴う密なリンパ性浸潤によって特徴付けられる涙腺の強い炎症反応を誘発する。涙ラクトフェリンレベルは抑制され、角膜および結膜上皮が損なわれ、ローズベンガル染色が増加する。3つの軌道LG組織の注入は、一貫した均一なドライアイ病状態を生成する。Con A注射の単一のセットは、10日目までにすべての臨床パラメータを正規化して約1週間続くドライアイ病を産生する。約1週間離れた連続的なCon A注射は、それに応じてドライアイ病の持続時間を延長する。約5セットの注射の後、ドライアイ病の状態は、多くの場合、さらなる注射を必要とせずに永久的になる。涙腺構造を最適に局所化することが最も重要である。頭蓋の解剖学と超音波を使用してスキルと一緒に毛皮の除去を含む細部への注意に精通して、すべての結果を向上させます。この簡単な技術はシャープの使用を必要とするので、調査は針刺しの怪我を防ぐために適切な予防措置を取る必要があります。
