脳卒中後の痙攣は一般的であり、痛みや拘縮を含む合併症を誘発することが示されており、その結果、生活の質が低下する1、2、3.痙攣の測定は、適切に治療の経過を計画し、治療の有効性を決定するために重要です。臨床設定で一般的に使用されるツールは、受動的な動きに対する耐性の公称測定システムである修正アッシュワーススケール(MAS)4と、キャッチ角度を測定する修正タルデュースケール(MTS)であり、痙攣の速度依存特性5.しかしながら、これらの測定ツールは、評価者間の信頼性が限られていることが示されており、満足のいく信頼性を維持するためにこれらの試験を実行するために同じ評価者を必要とする8。

痙攣の測定は、治療計画と治療後の有効性の決定において重要です。しかし、臨床設定で使用される現在のツールは、評価者間の信頼性に制限が示されています。この低い評価者間信頼性の要因の1つは、キャッチ(AoC)測定の角度を測定しながら受動運動の変動です。したがって、手動関節運動を標準化するために、アイソキネティック装置が提案されている。しかし、AoC測定のためのアイソキネティック運動の利点は標準化された方法でテストされていない。このプロトコルは、アイソキネティックモーション自体がAoC測定のレート間信頼性を向上させることができるかどうかを調べ取ります。この目的のために、表面筋電図(EMG)と組み合わせたロボットアイソキネティックデバイスが開発されました。2つの条件、手動とイソキネティックモーションは、キャッチの角度と主観的な感じを測定するための標準化された方法と比較されます。軽度の肘屈曲痙攣を有する17人の脳卒中患者において、AoC測定の間信頼性に対するクラス内相関係数(ICC)をEMGによる0.890[95%信頼区間(CI):0.685-0.961に改善したことがわかった。0.931 (95% CI: 0.791-0.978) のトルク基準により、手動モーションで 0.788 (95% CI: 0.493-0.920) から。結論として、アイソキネティック運動自体は、軽度の痙攣を有する脳卒中患者におけるAoC測定の評価間信頼性を向上させることができる。このシステムは、より大きな標準化された角度測定と感触のキャッチを提供する可能性があることを考えると、臨床現場での痙攣の評価のための良い選択肢かもしれません。

信頼性は、ICC値に応じて4つのグレードに分かれています:非常に優れた(>0.90)、優れた(0.75<ICC ≤ 0.90)、公正に良い(0.40 < ICC ≤ 0.75)、および貧弱な(<0.40)。測定の標準誤差(SEM)を計算し、分散の誤差成分を決定しました。検出可能な最小差(SDD)は、テスト再テストデータのSEMから計算された。
