경련을 측정하는 것은 치료 계획 및 치료 후 효능을 결정하는 데 중요합니다. 그러나, 임상 설정에서 사용되는 현재 의 도구는 평가자 간 신뢰성이 제한되는 것으로 나타났다. 이 불량한 간 평가자 신뢰성의 한 가지 요인은 캐치(AoC) 측정의 각도를 측정하는 동안 수동 동작의 가변성입니다. 따라서, 수동 조인트 모션을 표준화하기 위해 이소키네틱 장치가 제안되었다; 그러나 AoC 측정을 위한 이소키네틱 운동의 이점은 표준화된 방식으로 테스트되지 않았습니다. 이 프로토콜은 아이소키네틱 모션 자체가 AoC 측정을 위한 평가자 간 신뢰성을 향상시킬 수 있는지 여부를 조사합니다. 이를 위해 표면 근전도(EMG)와 결합된 로봇 이소키네틱 장치가 개발되었습니다. 수동 및 이소키네틱 모션이라는 두 가지 조건이 표준화된 방법과 비교하여 캐치의 각도와 주관적인 느낌을 측정합니다. 가벼운 팔꿈치 굴곡 경련을 가진 17명의 뇌졸중 환자에서, 이소키네틱 운동은 EMG에 의해 AoC 측정의 상호 평가자 신뢰성에 대한 계급 간 상관계(ICC)를 0.890 [95% 신뢰 구간(CI): 0.685-0.961]으로 개선한 것으로 나타났습니다. 수동 동작으로 0.788(95% CI: 0.493-0.920)에서 토크 기준에 따라 0.931(95% CI: 0.791-0.978)을 기준으로 합니다. 결론적으로, 이소키네틱 운동 자체는 경미한 경련을 가진 뇌졸중 환자에서 AoC 측정의 평가자 신뢰성을 향상시킬 수 있습니다. 이 시스템이 더 큰 표준화된 각도 측정 및 느낌 캐치를 제공할 수 있다는 점을 감안할 때, 임상 환경에서 경련을 평가하는 데 좋은 옵션이 될 수 있습니다.

신뢰성은 ICC 값에 따라 4 가지 등급으로 나뉩니다 : 매우 우수 (>0.90), 우수 (0.75 & ICC ≤ 0.90), 공정한 (0.40 & ICC ≤ 0.75), 그리고 가난한 (&0.40). 측정의 표준 오차(SEM)는 분산의 오차 성분을 결정하기 위해 계산되었습니다. 가장 작은 검출 가능한 차이(SDD)는 테스트 재테스트 데이터의 SEM에서 계산되었습니다.
