Dit artikel beschrijft een muis cryoblessure model om te onderzoeken ACS en verwante farmacologische en therapeutische opties.
De meest cruciale stap is de toepassing van de cryoprobe op het hartweefsel. De contact duur moet streng worden gecontroleerd om de optimale infarct grootte te verkrijgen en om reproduceerbare resultaten te garanderen. Acuut coronair syndroom (ACS) is de belangrijkste doodsoorzaak in de westerse wereld1,2. Acute occlusie van de coronaire slagaders leidt tot activering van ischemische Cascade en necrose van het aangetaste hartweefsel3. Beschadigd myocardium wordt geleidelijk vervangen door niet-contracactiel littekenweefsel, die klinisch manifestz als hartfalen4,5. Ondanks de recente ontwikkelingen in de behandeling van ACS stijgt de prevalentie van ACS-en ACS-gerelateerde hartfalen en zijn therapeutische opties beperkt tot6,7. Daarom zijn het ontwikkelen van diermodellen om ACS en zijn complicaties te bestuderen van enorm belang.
Tot op heden is het meest gebruikte diermodel om ACS en ACS-geïnduceerde myocardiale remodellering te bestuderen de ligatie van de linker aflopende coronaire slagader (LAD). Ligatie van de jongen leidt tot acute ischemie van het myocardium, vergelijkbaar met menselijk myocard weefsel tijdens ACS.  Echter, inconsistente infarct maten blijven de Achilles hiel van LAD ligatie. Chirurgische variatie en anatomische variabiliteit van de jongen leiden tot inconsistente infarct maten en belemmeren de reproduceerbaarheid en replicabiliteit van deze procedure8,9,10. Daarnaast heeft LAD ligatie een hoge intra-en postoperatieve sterfte. Ondanks de recente inspanningen om de reproduceerbaarheid te verbeteren en de sterfte te verminderen11,12, zijn grote aantallen dieren nog steeds nodig om anti-herinrichting therapieën naar behoren te evalueren.
Alternatieve modellen van ACS zijn in de afgelopen jaren voorgesteld en bestudeerd, waaronder radiofrequentie13, thermische14 of cryogene verwondingen15,16,17,18. Huidige cryoblessure methoden passen een metalen staaf voorgekoeld in vloeibare stikstof om het hartweefsel van het onderwerp te beschadigen15,16. Deze procedure moet echter meerdere malen worden herhaald om een voldoende infarct grootte te genereren. Als gevolg van de hoge geleidbaarheid en lage warmtecapaciteit van de staaf in vergelijking met het weefsel, de sonde warmt snel, en het weefsel wordt gekoeld (en dus infarcted) heterogene. Om deze beperkingen te overwinnen, beschrijven we hierin een cryoinfarct model met behulp van een hand-held vloeibaar stikstoftoevoer systeem. Dit model is reproduceerbaar, gemakkelijk uit te voeren en kan snel en betrouwbaar worden vastgesteld. Een reproduceerbare Transmurale infarct laesie onafhankelijk van coronaire anatomie wordt gegenereerd, die uiteindelijk leidt tot hartfalen. Deze methode is vooral geschikt om het renovatieproces te bestuderen voor de evaluatie van nieuwe therapeutische farmacologische en weefsel-engineering gebaseerde strategieën.

Het gebruik van diermodellen is essentieel voor het ontwikkelen van nieuwe therapeutische strategieën voor acuut coronair syndroom en de complicaties. In dit artikel demonstreren we een Murine cryoblessure infarct-model dat precieze infarct-grootten genereert met een hoge reproduceerbaarheid en replicabiliteit. In het kort, na intubatie en sternotomie van het dier, wordt het hart van de thorax opgeheven. De sonde van een handheld vloeibaar stikstoftoevoer systeem wordt aangebracht op de myocardiale wand om cryoletsel te induceren. Een verminderde ventriculaire functie en elektrische geleiding kan worden bewaakt met echocardiografie of optische mapping. Transmural myocardiale remodellering van het infarcten gebied wordt gekenmerkt door collageen afzetting en verlies van cardiomyocytes. In vergelijking met andere modellen (bijvoorbeeld LAD-ligatie), maakt dit model gebruik van een handheld vloeibaar stikstoftoevoer systeem om meer uniforme infarct groottes te genereren.

Het cryoblessure infarct model is geschikt om ACS en zijn complicaties te bestuderen. Lage sterftecijfers en efficiënt postchirurgisch herstel worden in dit model gezien. Cryoletsel veroorzaakte myocardiale beschadiging leidt tot een verminderde hartfunctie, elektrische ontkoppeling en Transmurale remodeling.

Deze methode is betrouwbaar om de generatie van het hartinfarct te bestuderen en een uitstekend platform te gebruiken voor het onderzoeken van cardiale herstel na bijvoorbeeld stamceltransplantatie. Het belangrijkste voordeel van deze procedure is dat het kan worden gebruikt om uniforme infarctmaten te genereren op een snelle, gemakkelijke en reproduceerbare manier. Nieuwe experimentele interventies, zoals farmacologische, of het weefsel ontworpen strategieën, kunnen worden getest in dit model.En met succes testen dat opties verder kunnen worden bevestigd in grote diermodellen. Na bevestiging van een gebrek aan reactie op pijnreflex in een verdoofde 27 gram, 14 weken oude muis, gebruik een haartrimmer om de vacht van de borst en nek van het dier te verwijderen en de muis in een supine positie op een verwarmde pad te plaatsen. Plak de achter- en voorpoten in een gespreide adelaarspositie en gebruik sequentiële povidone-jodium en 80% ethanolscrubs om de blootgestelde huid te desinfecteren.Maak een middellijn huid incisie van het onderste derde deel van het borstbeen naar de kin, en gebruik gebogen tangen om zorgvuldig te scheiden van de spieren rond de nek om de luchtpijp bloot te stellen. Gebruik een microschaar om een tracheotomie uit te voeren tussen de tweede en derde kraakbeenringen. En zet de ventilator op een ventilatiefrequentie van 110 per minuut met een getijdenvolume van 0,5 milliliter.Steek vervolgens een plastic, 20 meter canule aangesloten op de ventilator in de luchtpijp. Gebruik vervolgens cautery om de rechter pectoralis spier los te maken van zijn sternale oorsprong tussen de derde en zevende ribben. En gebruik kant schuine veerschaar om de vierde tot zes ribben zo dicht mogelijk bij het borstbeen te snijden.Ontleden het onderliggende bindweefsel om een duidelijk zicht in de borstholte te verkrijgen, waardoor de borstslagader wordt dichtgeschroeid om bloedingen zo nodig te stoppen. Steek een mini gouden vlek oprolmechanisme om de ribben te verspreiden en houd de borstholte open. En gebruik botte tangen om het hartzakje te openen om het hart bloot te leggen.Gebruik een stompe staaf om het hart uit de borstholte te tillen en de spanning van het oprolmechanisme te verminderen om de borstopening te verminderen en om te voorkomen dat het hart terugvalt in de buikholte. Pre-cool een drie millimeter diameter cryoprobe voor 10 seconden, en breng de cryoprobe op het interieur linker ventrikel muur voor 10 seconden om een linker ventriculaire, cryoinjury infarct te genereren. Maak de cryoprobe los van de linker ventriculaire wand met zoutirrigatie op kamertemperatuur en verbreed de borstopening met het oprolmechanisme.Gebruik de staaf om het hart voorzichtig terug te brengen naar de borstholte en verwijder het oprolmechanisme. Gebruik een 6-0 hechting om de sternotomie te verbinden met een enkele knoop. En gebruik een lopende 6-0 hechting om de borstholte te sluiten.Gebruik een spuit van 10 milliliter om de resterende lucht uit de borst te evacueren voordat u de knoop bindt en pas de huid aan de staartrand aan voordat u een lopende 5-0 hechting gebruikt om het weefsel te sluiten tot het punt van de tracheale opening. Na het verminderen van het isoflurane en het dier krijgt spontane ademhaling verwijder de tracheale katheter en sluit de tracheale incisie met een 8-0 hechting. Vervang de ventrale nekspieren terug naar hun anatomische positie over de luchtpijp en voltooi de hechting van de huid.Voeg vervolgens metamizool toe aan het drinkwater voor pijnanalgesie gedurende drie dagen met dagelijkse dierenbewaking gedurende acht weken. In cryoinjured harten echocardiografie onthult een aanzienlijk verminderde uitwerpfractie en fractionele gebiedsverandering, met functionele stoornis waargenomen vanaf dag zeven na de operatie tot het experimentele eindpunt op 56 dagen. Zoals beoordeeld door druk-volume lus analyse, cryoinfraction leidt tot een verminderde linker ventrikel functie die wordt weerspiegeld als een daling van de slag volume, beroerte werk, cardiale output, en pre-load aangepast maximaal vermogen.Ex-vivo optische mapping van fluorescerende spanningsgevoelige geverfde harten toont een verstopping van elektrische geleiding aan de rand van letsel, wat wijst op een lokale elektrische ontkoppeling. Histologische kleuring met Masson's trichrome onthult transmurale fibrotische weefselvorming op de plaats van letsel. De infarctgrootte kan worden berekend door het infarctlittekengebied of de lengte van het litteken in de middellijn te meten.Bovendien bevestigen immunofluorescente kleuring tegen een cardiomyocyte marker en Collageen-I de aanwezigheid van fibrotische remodellering en het verlies van cardiomyocyten op de plaats van letsel. Het is belangrijk om dezelfde contactduur van de cryoproblem elk dier te behouden als verschillende duur kan leiden tot incongruent infarct maten. Eenmaal onder de knie deze techniek kan worden uitgevoerd in 20 minuten als het goed wordt uitgevoerd.Na het bekijken van deze video moet je een goed begrip hebben hoe je het cryoinjury-model moet uitvoeren.
