למרות היכולת המרשימה של מערכת השמיעה לפיצוי על תשומות אסימטריים משתי האוזניים, המחקר הראה שתגובות לתנועות מהירות מעולם לא מפצות באופן מלא12,13. ידוע כיום שפיצויים טבעיים אינם משתמשים במלוא הקיבולת של המערכת, וניתן לשפר את תגובת הבור בבעלי חיים שהשתתפו בהכשרה בטווח הראייה14,15. כבר מזמן ידוע כי תרגילי שיקום בעלי האיזון משפרים את הפיצוי בחולים עם בעיות בלתי מאוזנת כרונית על ידי שיפור האופי הרב חושי של שליטה בשיווי משקל16,17, מיכל בן 18 , מיכל בן 19 , מיכל בן 20 , 21. מטרת תרגילי השיקום הללו היא להשתמש בגישות פיזיולוגיות או התנהגותיות כדי לשפר את הסימפטומים, כמו גם את איכות החיים והעצמאות של המטופל,22,23.
מתוארת להלן שיטת שיקום המשתמשת בסיבובים חד כיווני לכיוון הצד "החלש" (איור 1A). הרעיון הבסיסי לשיטה זו נובע מפלסטיות הבבית, בו הקשרים העצביים נעשים חזקים יותר כאשר הם ממריצים. שיטה זו משנה באופן מפורש את תשומות השמיעה במקום לשפר את האינטגרציה הסנסורית, שהיא הבסיס לתרגילי שיקום אחרים בעלי שיווי משקלים. מחקרים קודמים הראו כי סיבובים חד-כיוונית מקטין את הבור א-סימטריה ב-1-2 הפעלות בחולים עם בעיות בעלי הסדר חד צדדית3. אפקט זה נבע בעיקר מגידול בפעילות הצדדית עם תגובה נמוכה יותר (LR), כמו גם ירידה קלה בפעילות הצד עם תגובה גבוהה יותר (HR). שינוי זה הוא ככל הנראה מתווכת על ידי שינויים במסלולים המרכזיים (למשל, חיזוק מסלולים כלי לימפה, כגון חיבורי VN או שינויים בכניסות מערכת). למעשה, ניתן להשתמש בטכניקה זו כשיטה מבוקרת לשיקום הפרוזדור בבעלי שיווי מידה ובעלי-סימטריה.
