2つの耳からの非対称入力を補償する前庭システムの印象的な能力にもかかわらず、研究は、速い動きへの応答が完全に補償されることはない12、13を示しています。自然前庭補償は、システムの全容量を使用しないことが知られており、補償されたVOR応答は、視覚前庭トレーニング14、15に参加した動物において改善することができる。前庭リハビリテーション演習は、バランスコントロール16、17の(非前庭)多感覚性を高めることによって慢性不均衡問題を有する患者の補償を改善することは長い間知られている。18歳,19歳,20歳,21.これらの前庭リハビリテーション演習の目的は、症状を改善するために生理学的または行動的アプローチを使用するだけでなく、患者の生活の質と独立性 22,23.
本明細書に記載されているのは、「弱い」側に向かって単方向回転を使用するリハビリテーション方法である(図1A)。この方法の基本的な考え方は、刺激されると神経接続が強くなるヘビアン可塑性から来ています。この方法は、他の前庭リハビリテーション演習の基礎である多感覚統合を強化するのではなく、前庭の入力を特に変更します。以前の研究では、単方向回転は、一方的な前庭機能不全3の患者における1-2セッションでVOR非対称性を減少させることが示されている。この効果は、主に、より低い応答(LR)を有する側の活性の増加、ならびに高い応答(HR)を有する側の活性のわずかな減少によるものであった。この変化は、中央経路の変更(例えば、VN接続やコミッサル入力の変化など)の改変によって媒介される可能性が高い。実質的には、この技術は、長年の前庭非対称性を持つものにおける前庭リハビリテーションの監督方法として使用することができる。
