A disfunção vestibular é uma desordem comum com uma prevalência de ~ 35% em adultos acima de 40 anos1. A maioria das desordens vestibulares resultam em uma assimetria entre a entrada de ambos os lados, resultando em uma ilusão de rotação chamada vertigem. Na ausência de função vestibular normal, mesmo as atividades diárias simples podem ser desafiadoras. A disfunção vestibular é frequentemente quantificada pelo reflexo vestíbulo-ocular (VOR). Durante as atividades naturais, como caminhar ou correr, o VOR move os olhos na direção oposta e com a mesma velocidade que o movimento da cabeça. Este reflexo tem uma curta latência de ~ 5 ms, e é mediada no plano horizontal através de um simples, três-Neuron Arc2. A disfunção vestibular unilateral produz alterações compensatórias centrais e mudanças periféricas centradas centralmente para superar o VOR assimétrico defeituoso e o desequilíbrio vestibular resultante. Mesmo após lesões vestibulares permanentes, como uma neurectomia vestibular unilateral, a vertigem e os sintomas acompanhantes melhoram em um curto período (dias a semanas) de tempo. Devido a essa habilidade, o sistema vestibular tem sido um modelo para estudar adaptação e compensação em vias neurais. Tem sido demonstrado anteriormente3 que mudanças nas vias vestibulares centrais podem ser implementadas por uma rotação unidirecional com base em uma hipótese proposta por um dos autores (N.R.) há cerca de 20 anos. Estas mudanças compensatórias resultam em um novo equilíbrio na atividade dos neurônios VN em ambos os lados.
Apesar da impressionante capacidade do sistema vestibular de compensar as entradas assimétricas das duas orelhas, a pesquisa mostrou que as respostas aos movimentos rápidos nunca são totalmente compensadas12,13. Sabe-se agora que a compensação vestibular natural não utiliza a capacidade total do sistema, e a resposta do Vor compensado pode ser melhorada em animais que participaram do treinamento Visual-vestibular14,15. Há muito se sabe que os exercícios de reabilitação vestibular melhoram a remuneração em pacientes com problemas de desequilíbrio crônico, reforçando a natureza multissensorial (não vestibular) do controle de equilíbrio16,17, 18 anos de , 19 anos de , 20 anos de , 21. o objetivo destes exercícios de reabilitação vestibular é o uso de abordagens fisiológicas ou comportamentais para melhorar os sintomas, bem como a qualidade de vida e a independência do paciente22,23.
Descrito aqui é um método de reabilitação que utiliza rotações unidirecionais em direção ao lado "mais fraco" (Figura 1a). A idéia básica para este método vem da plasticidade Hebbian, em que as conexões neurais se tornam mais fortes quando são estimulados. Este método modifica especificamente as entradas vestibulares em vez de melhorar a integração multissensorial, que é a base para outros exercícios de reabilitação vestibular. Pesquisas anteriores mostraram que as rotações unidirecionais diminuem a assimetria do VOR em 1-2 sessões em pacientes com disfunção vestibular unilateral3. Este efeito foi principalmente devido a um aumento na atividade do lado com uma resposta mais baixa (LR), bem como uma ligeira diminuição na atividade do lado com uma resposta mais elevada (HR). Esta mudança é provavelmente mediada por modificações nas vias centrais (por exemplo, fortalecimento de vias aferentes, tais como conexões VN ou mudanças nas entradas commissurais). Com efeito, esta técnica pode ser utilizada como método supervisionado para a reabilitação vestibular naqueles com assimetria vestibular de longa data.

using unidirectional rotations to improve vestibular system asymmetry in patients with vestibular dysfunction

O sistema vestibular fornece informações sobre o movimento da cabeça e medita reflexos que contribuem para o controle do equilíbrio e estabilização do olhar durante as atividades diárias. Os sensores vestibulares estão localizados na orelha interna de ambos os lados da cabeça e projetam-se para os núcleos vestibulares no tronco cerebral. A disfunção vestibular é muitas vezes devido a uma assimetria entre a entrada dos dois lados. Isso resulta em entradas neurais assimétricas das duas orelhas, que podem produzir uma ilusão de rotação, manifestada como vertigem. O sistema vestibular tem uma capacidade impressionante de compensação, que serve para reequilibrar a forma como a informação assimétrica dos órgãos finais sensoriais em ambos os lados é processada no nível central. Para promover a compensação, vários programas de reabilitação são utilizados na clínica; no entanto, eles usam principalmente exercícios que melhoram a integração multissensorial. Recentemente, o treinamento Visual-vestibular também tem sido utilizado para melhorar o reflexo vestíbulo-ocular (VOR) em animais com lesões unilaterais compensadas. Aqui, um novo método é introduzido para reequilibrar a atividade vestibular de ambos os lados em indivíduos humanos. Este método consiste em cinco rotações unidirecionais no escuro (velocidade de pico de 320 °/s) em direção ao lado mais fraco. A eficácia deste método foi mostrada em um ensaio clínico seqüencial, duplo-cego em 16 pacientes com assimetria do VOR (medido pela preponderância direcional em resposta às rotações sinusoidal). Na maioria dos casos, a assimetria do VOR diminuiu após uma única sessão, atingiu valores normais dentro das duas primeiras sessões em uma semana, e os efeitos duraram até 6 semanas. O efeito de rebalanceamento é devido a um aumento na resposta do VOR do lado mais fraco e uma diminuição na resposta do lado mais forte. Os resultados sugerem que a rotação unidirecional possa ser usada como um método supervisionado da reabilitação para reduzir a assimetria do VOR nos pacientes com deficiência orgânica vestibular de longa data.

Estimular o lado mais fraco e inibir o lado mais forte, uma ideia originalmente proposta pelo Doutor Nayer Rassaian, tem sido demonstrada para diminuir a assimemetria em indivíduos com desequilíbrio vestibular. Esta técnica fornece um método único para estimular efetivamente o sistema vestibular no escuro e não requer equipamentos complicados. A rotação unidirecional poderia ser utilizada como opção de reabilitação para pacientes com desequilíbrio entre entradas vestibulares dos dois lados, quantificadas pelo reflexo vestibulo-ocular.Este método requer uma cadeira rotativa e supervisão profissional. O aspecto crítico é a lenta desaceleração para evitar a estimulação do lado oposto no final da rotação unidirecional. Para medir o reflexo vestibulo-ocular, tenha o participante em uma cadeira rotativa e segure o participante com o arreio.Dê ao participante um par de óculos infravermelhos e fixe a cabeça no encosto da cabeça na posição do nariz para baixo em um ângulo de 30 graus. Após a rotação unidirecional, a resposta às rotações e à direção do lado com a resposta inferior foi aumentada e a resposta às rotações na direção com a resposta mais forte diminuiu resultando em uma mudança no valor direcional de preponderância devido à diminuição da assymetria reflexo vestibulo-ocular. Expor os sujeitos a rotação unidirecional durante várias sessões diminui ainda mais o valor direcional de preponderância, um efeito que é mantido entre as sessões e que resulta na maioria dos sujeitos que apresentam uma preponderância direcional na faixa normal após apenas duas sessões.Embora seja importante atingir a velocidade de pico eficaz, a declaração lenta é parte crítica da rotação para garantir que a estimulação permaneça predominantemente unidirecional. Este método é seguro e foi bem tolerado pelos sujeitos.
