Pulmonale arteriële hypertensie (PAH) is een chronische en ernstige cardiopulmonale stoornis met een hoogte in longslagader druk (PAP) en rechter ventriculaire druk (RVP) die wordt veroorzaakt door cellulaire proliferatie en fibrose van kleine longslagaders 1. longslagader katheters, ook wel Swan-Ganz katheters2genoemd, worden vaak gebruikt in de klinische controle van RVP en PAP. Bovendien is een draadloos PAP-bewakingssysteem klinisch3,4,5gebruikt. Om de ziekte na te bootsen voor onderzoek bij muizen, wordt een hypoxische omgeving gebruikt om humane Klinische manifestaties van PAH6te simuleren. Bij de evaluatie van PAP bij dieren zijn grote dieren relatief gemakkelijk te controleren door middel van longslagader katheters met dezelfde techniek als voor menselijke proefpersonen, maar kleine dieren zoals ratten en muizen zijn moeilijk te beoordelen vanwege hun kleine lichaamsgrootte. Hemodynamische meting van het rechter ventrikel systeem bij muizen is mogelijk met een ultrakleine maat 1 FR katheter7. Een methode voor het meten van RVP en PAP bij muizen is gerapporteerd in de literatuur8,9, maar de methodologie mist een gedetailleerde beschrijving. RVP en PAP zijn moeilijker te meten dan de linker ventriculaire druk vanwege de anatomische verschillen tussen de linker en rechter hart systemen.
Om zowel PAP-als RVP-parameters in dezelfde muis te krijgen, beschrijven we een open-borstchirurgie-gebaseerde aanpak voor rechter hart hemodynamische metingen, de validering ervan met gezonde en PAH muizen, en hoe om te voorkomen dat kunstmatige gegevens worden gegenereerd tijdens de gecompliceerde open-Chest Chirurgie. Hoewel deze techniek het beste kan worden uitgevoerd door een goed opgeleide chirurg, heeft het het voordeel om PAP en RVP in dezelfde muis te kunnen beoordelen.

Pulmonale arteriële hypertensie (PAH) is een chronische en ernstige Cardiopulmonaire aandoening. Muizen zijn een populair diermodel dat wordt gebruikt om deze ziekte na te bootsen. De evaluatie van rechter ventriculaire druk (RVP) en pulmonale slagader druk (PAP) blijft echter technisch uitdagend bij muizen. RVP en PAP zijn moeilijker te meten dan de linker ventriculaire druk vanwege de anatomische verschillen tussen de linker en rechter hart systemen. In dit artikel beschrijven we een stabiele rechter hart hemodynamische meetmethode en de validatie ervan met behulp van gezonde en PAH muizen. Deze methode is gebaseerd op open-borstchirurgie en mechanische ventilatie ondersteuning. Het is een ingewikkelde procedure in vergelijking met gesloten borst procedures. Hoewel een goed opgeleide chirurg nodig is voor deze operatie, is het voordeel van deze procedure dat het zowel RVP-als PAP-parameters tegelijkertijd kan genereren, dus het is een voorkeurs procedure voor de evaluatie van PAH-modellen.

Dit protocol is een open borst chirurgie gebaseerde hemodynamische beoordeling en het is nuttig voor de juiste ventriculaire en longslagader druk evaluatie bij kleine dieren zoals muizen. Het voordeel van deze procedure is dat het zowel VP- als PAP-parameters tegelijkertijd kan genereren. Het heeft dus de voorkeur voor evaluaties van PAK-modellen.Begin met het opzetten van de apparatuur voor chirurgie. Week de druktransducerkatheter in 0,9% zout bij kamertemperatuur gedurende ten minste 30 minuten voor het hemodynamische experiment. Sluit vervolgens de katheter aan op een drukvolumesysteem.Kalibreer de druktransducer door de kalibratieknop op nul en 25 millimeter kwik te draaien om een verificatiedruksignaal naar de software voor gegevensverwerving te sturen en configureer vervolgens de kalibratie-instelling. Draai de knop om te transduceren, en stel de balans klep tot nul baseline. Stel een standaard stereomicroscoop, temperatuurgecontroleerde kleine dierlijke chirurgische lijst, en een licht verlichtingssysteem voor microchirurgie op.Zodra de muis goed is verdoofd, verwijder de borst en nek vacht met behulp van een scheerapparaat en ontharing lotion. Zet de muis in de supinepositie op de temperatuurgecontroleerde chirurgische tafel. Na ervoor te zorgen dat de anesthesie van kracht is, maak een middellijn incisie op de nekhuid.Ontleden de skeletspier met gebogen tangen en bloot de luchtpijp. Voer intubatie uit via de mond met een 22-meter intraveneuze schedekatheter, wat bevestigt dat de slang zich in de luchtpijp bevindt met tangen. Sluit de slang aan op een kleine dierlijke ventilator en zet het voor ventilatie met behulp van tape.Maak vervolgens een middellijn incisie op de borsthuid en ontleed zorgvuldig de borstspier met behulp van een cautery tool. Snijd het borstbeen met behulp van een schaar in het midden, en bloot de borstholte. Zorg ervoor dat u bloedingen met behulp van de cautery tool te voorkomen.Stel de rechter ventrikel bloot met oprolmechanismen en steek vervolgens de zoutlijn doorweekte druktransducerkatheter door een kleine tunnel die is gemaakt met een naald van 25 meter in de rechterventrikel om de juiste ventriculaire druk te meten, of RVP. Houd de katheterkabel vast en kruis de longklep op een coaxiale manier met de longslagader. Observeer de drukgolfvorm en verkrijg een stabiele longslagaderdruk of PAP-signaal.Neem vervolgens de heodynamische gegevens op met behulp van de software voor het verzamelen van gegevens. Verwijder de katheter voorzichtig uit het juiste hartsysteem en plaats deze in een spuit van één milliliter met een spijsverteringsenzymoplossing van 1%. Dit protocol werd met succes gebruikt om RVP- en PAP-golfvormen bij muizen te verkrijgen door een drukomvormerkatheter in de rechterventrikel en de longslagader in te voegen.Om kunstmatige gegevens te voorkomen, werden segmenten met ruis uitgesloten van de gegevensanalyse en werden correcties aangebracht indien het nulniveau van de katheter was afgedreven. RVP en PAP werden gemeten bij muizen met geïnduceerde chronische hypoxie, en de resultaten werden vergeleken met die verkregen uit een controlegroep. Systolische PAP, diastolische PAP, gemiddelde PAP, en rechts ventriculaire systolische druk waren allemaal aanzienlijk verhoogd in de chronische hypoxie groep.Het belangrijkste om te onthouden is om de druktransducer katheter in te voegen in de rechter ventrikel, zorg ervoor dat schade aan de katheter te voorkomen en bevestigen de gladde RVP golfvorm verschijnt op het scherm.
