脂肪組織は、再建および美容手術の充填剤として長く使用され、最近では間葉系幹細胞(MSC)1の供給源として認識された再生医療においてより一般的になってきている。単細胞懸濁液を単細胞懸濁液に解離すると、患者に変わらず使用される脂肪細胞フリー間質血管画分(SVF)を得るか、より一般的には、MSC2に数週間培養される。
しかし、酵素解離は組織微小環境を破裂させ、インビトロ培養によってかなり修飾される推定再生細胞から隣接する調節細胞を隔離する。このような実験的なアーティファクトおよび結果的な機能的変化を避けるために、可能な限りそのネイティブ構成をそのまま維持しながら、治療用の脂肪組織を処理する試みがなされている3,4。特に、機械的組織の破壊は酵素解離に取って代わり始めている。この目的のために、完全な浸漬閉塞系マイクロフラグメントリポアスは、サブミリメートル、血液および無油組織クラスター(例えば、リポゲム)にふるい質濾過および鋼大理石誘発破壊3のシーケンスを介して、マイクロ断片化脂肪組織の自家移植は、この閉じたシステム技術を使用して、複数の適応症、化粧品、整形外科、直腸および婦人科4、5、複数の適応症で成功している。6,7,8,9,10,11,12,13.
閉じたシステム装置と同位体SVFで得られたヒト微細断片脂肪組織(MAT)との比較は、血管/間質細胞分布および培養中の分泌活性に関して、MATはより多くのペリサイトを含有することを明らかにした。推定MSC14、および成長因子およびサイトカイン15のより高い量を分泌する。
本記事は、閉じたシステム装置を用いたヒト皮下脂肪組織の酵素フリー微小断片化、及びインビトロ培養用微小化脂肪組織のさらなる処理、免疫組織化学およびFACS分析、存在する細胞型と分泌される可溶性因子を同定するために(図1)。記載された方法は、生存可能な脂肪組織細胞集団を含む脂肪由来のサブミリメートルオルガノイドを無傷のニッチで安全に生成し、さらなる応用および研究に適している。

このシステムを用いて、再生細胞の微小環境全体がそのままのヒト脂肪組織のサブミリクラスターを収集することができます。微小血管系全体もそのままであり、生体内培養や細胞分離のために生体内移植のためにこのような微小臓器を使用してさらなる特徴付けを行うことができます。研究所でのシステムの使用の主な根拠は、プロセスが完全に酵素フリーであるため、マイクロアーキテクチャを維持しながら、人間の脂肪組織の迅速かつ効率的な処理を可能にし、それは完全に無傷の組織の微小血管系になります。そして、私たちは直接クリニックですでに非常に有用であることが証明されている細胞療法製品を研究することができます。この閉じたシステム技術は、収穫、加工、および精進した脂肪組織を手術中に再注入するための使用が容易な技術であり、化粧品整形外科、肛門科および婦人科にもうまく適用することができます。この方法は、組織再生における脂肪の影響のさらなる研究を可能にする。この手順は、博士研究員のビアンカ・ヴェッツァーニと、博士課程の学生であるマリオ・ゴメス=サラザールによって実証されます。収穫から16時間以内に無菌状態で働き、腹部形成術サンプルを外科布の上に置き、皮膚を上向きにする。使い捨てティッシュ浸潤カニューレを使用して、皮下脂肪組織に摂氏0.9%のナトリウム溶液を50〜100ミリリットル注入します。使い捨て脂肪吸引カニューレに10ミリリットルのルアーロックシリンジを接続し、サンプルの端付近の皮領域でサンプルをつかみ、慎重に脂肪組織にカニューレを導入します。カニューレが組織の内部に入ったら、プランジャーを抽出し、脂肪組織サンプル内でカニューレを放射状に移動します。脂肪吸引の間に脂肪組織をしっかり扱うことが重要である。十分な脂肪が収集されない場合, 追加の生理的注射を行う検討.注射器がリポ吸引でいっぱいになったら、慎重にカニューレを組織から取り出し、合計60ミリリットルの吸気が収穫されるまで、完全な注射器を新しい空の注射器に置き換えます。リポ吸引のマイクロフラグメンテーションの場合は、パッケージからデバイスを取り外し、本体が廃棄物袋に接続されていることを確認します。廃棄物回収袋を地面に置き、バルブがプロセスユニットのキャップに固定されていることを確認します。袋接続ポートを突き刺して入力ラインの端子スパイクを生理袋に接続し、処理ユニットよりも高い位置にサラインバッグを配置します。回路の管に接続された5つのクランプがすべて開いていることを確認し、処理装置を垂直位置に置き、生理液を処理装置に充填させ、フローが廃棄物袋に到達することを確認します。処理装置を振って気泡を取り除き、青色のキャップを上向きにして垂直位置に配置します。入力ラインの隣のクランプを閉じ、リポ吸引の注射器を青い入力キャップの自己閉塞弁に接続して、リポ吸引物の注入を開始します。処理装置を垂直に保ち、リポ吸気物のすべてがユニットに移されるまで、シリンジプランジャーをゆっくりと押し下げる。すべてのリポ吸入物を注入したら、入力クランプを開いて生理食い流れを回復させ、処理装置を少なくとも2分間激しく振り、定期的に生理食物の溶液の流れを廃棄物袋に入れ、チェックする。処理ユニットの生理液が透明になったら、グレーキャップのユニットを上に置き、青色とグレーの両方のキャップの近くのドレインにあるクランプを閉じます。処理された組織は上部に浮かびます。次に、10ミリリットルのルアーロックシリンジを生理食塩水で満たし、注射器を青色キャップのローディングバルブに接続します。灰色の帽子の弁に完全に挿入されたプランジャーと空の10ミリリットルルアーロックシリンジを接続します。青いキャップから加工ユニットに生理食音をしっかりと注入し、灰色のバルブに接続されたシリンジが結果的に加工組織で満たされていることを確認する。すべての生理食音が注入されたら、両方の注射器を慎重に取り出し、処理された組織がすべて採取されるまで生理食音注入を繰り返します。処理の最後に、すべてのシリンジを取り外し、すべてのクランプを閉じ、生理食音袋を取り外し、ローカルプロトコルに従ってデバイスを処分します。すべての処理された組織が採取されたら、微小断片化した脂肪組織を滅菌容器に移し、等量の消化媒体を容器に加える。密封容器を摂氏37度の揺れる水浴に入れ、毎分120回転に設定して45分間、インキュベーション終了時に同量のブロッキング溶液で消化を止める。サンプルを順次ろ過し、まず100マイクロメートルの細胞ストレーナーを通して、70マイクロメートルの細胞ストレーナーを通して濾過し、濾過された懸濁液を200回gで5分間遠心分離する。ペレットを約5ミリリットルの赤血球リシスバッファーに室温で15分間懸濁します。同量のブロッキング溶液で溶解を停止し、40マイクロメートルの細胞ストレーナーを介して溶液をフィルター処理します。次いで、遠心分離によってマイクロ断片化された脂肪組織細胞を収集し、カウントするための小麦粉混合で新鮮なブロッキング溶液中のペレットを中断します。解離および抗体染色後、微分脂肪細胞を細胞分析および/または選別のためにフローサイトメーターに取り込むことができます。手動リポ吸引の機械的解離は、微小血管ネットワークを包む脂肪細胞の集合体からなる微小断片化脂肪組織の産生をもたらす。凍結した微細断片化脂肪組織に埋め込まれたゼラチンの免疫蛍光分析は、構造を強調し、内皮細胞マーカーUlex Europaeus Agglutinin1受容体によってマークされ、主に小さな毛細血管様血管からなる血管ネットワークを示す。特に、ニューロングリア抗原2または血小板由来成長因子受容体βを発現する周皮細胞は、正常に分布し、内皮細胞を包皮する。CD31陽性内皮細胞およびCD45陽性造血細胞を除くことによって、CD146陽性CD34陰性の前腸細胞およびCD146陰性およびCD34陽性の出現細胞の存在は、MAT内で同定することができる。マイクロフラグメント化脂肪組織は、免疫組織化学的染色、インビトロ培養または分泌物分析によってさらに処理され、脂肪組織生物学に関するより深い洞察を得ることができる。この技術は、マウスに移植されたり、長期的な培養で維持されるのに十分な小さな小さなオルガノイドの一種として、自然でネイティブで3次元的な配置で多様な組織を研究することを可能にする。そして、このシステムを主に使用して、実験的に異なる器官特異的微小環境にアプローチします。私たちの脂肪組織ドナーは、伝染性感染症のキャリアとして知られていません。しかし、それ以上のスクリーニングがない場合、すべてのヒト組織は潜在的に汚染され、それに応じて処理されるべきであると考えるべきである。
