Cardiopulmonale oefening testen (CPET) is toegepast bij patiënten met hartfalen met verminderde ejectiefractie (HF) voor meerdere doeleinden, waaronder de kwantificering van Cardiopulmonaire fitheid, prognose, diagnose van oorzaken van inspanningsbeperkingen, en oefening voorschriften1,2,3. Tijdens het testen worden hemodynamische variabelen en gegevens afkomstig van automatische gasuitwisseling bewaakt en geanalyseerd. De bewaking van cerebrale weefsel zuurstofverzadiging (scto2) heeft een waarde voor de beoordeling van de prognose en de ernst van de ziekte4,5.
Near-Infrared spectroscopie (NIRS) gebruikt infraroodlicht om de schedel binnen te dringen en de oxygenatie van hersenweefsel continu en niet-invasief6te schatten. Aangezien oxyhemoglobine en deoxyhemoglobine verschillende lichtabsorptie Spectra hebben en de primaire chromoforen zijn die licht absorberen, kunnen hun concentraties worden gemeten met behulp van lichttransmissie en-absorptie6,7. Echter, achtergrondlicht absorbers verstrooien ook licht en kunnen invloed hebben op de meting8. Deze studie nam een ruimtelijk opgeloste NIRS aan om SctO2 te meten van rust tot piek oefening9. SctO2 vertegenwoordigt het aandeel van de zuurstoftoevoer versus het verbruik in cerebrale weefsel. Cerebrale desaturatie wordt geassocieerd met verstoorde cerebrale doorbloeding (CBF), verlaagde arteriële zuurstofconcentratie en verhoogd cerebrale weefsel zuurstofverbruik11. Andere dan cardiale uitgangs insufficiëntie, geavanceerde HF veroorzaakt cerebrale hypoperfusie tijdens de oefening door indirect inducerende cerebrale vasoconstrictie via afnemende arteriële partiële druk van koolstofdioxide (PaCO2) door middel van hyperventilatie 12.
De klinische significantie van cerebrale oxygenatie in HF werd onthuld door Chen et al.4. Ten eerste werd SctO2 significant verlaagd in de HF-groep in vergelijking met gezonde controles. SctO2 is niet alleen verminderd in rust, maar daalde ook verder tijdens de oefening. Het is niet waargenomen in de gezonde groep. Tweede, SctO2rest en scto2peak werden gecorreleerd met vo2peak, Brain natriuretisch peptide (BNP), en de zuurstofopname efficiëntie helling (oues), die allemaal zijn gevestigd prognostische markers. Daarom zijn SctO2rest en scto2peak zeer waarschijnlijk prognostisch en weerspiegelen de ernst van de ziekte bij HF-patiënten. Een andere studie door Koike et al. suggereerde dat de verandering in cerebrale oxyhemoglobine, gemeten op het voorhoofd van rust tot piek oefening, significant lager was bij niet-overlevenden in vergelijking met die bij overlevenden van patiënten met coronaire hartziekte5. Daarom kan cerebrale oxygenatie worden gebruikt om de ernst van de ziekte en de prognose van patiënten met HF te stratificeren.

Cerebrale hypo oxygenatie tijdens rust of oefening heeft een negatieve invloed op de Inspanningscapaciteit van patiënten met hartfalen met verminderde ejectiefractie (HF). Echter, in klinische Cardiopulmonaire oefening testen (CPET), cerebrale hemodynamiek wordt niet beoordeeld. Cerebrale weefsel zuurstofverzadiging gemeten door NIRS is eenvoudig en handig.Dit protocol-geïntegreerde NIRS in conventionele CPET om de waarde van oefening testen bij patiënten met hartfalen te verhogen. Demonstreren van de procedure zal Yu Chen Liu, een technicus van ons laboratorium. Om deze procedure te beginnen, reinig het voorhoofd van de patiënt twee keer met een alcoholpad om zweet en vuil van de huid te verwijderen.Verkrijg grote NIRS-sensoren waarbij de afstand tussen de zender en de detector vijf cm bedraagt en de sensoren bilateraal op het voorhoofd plaatst. Zorg ervoor dat de sensoren stevig zijn bevestigd. Breng vervolgens ECG-elektroden aan op de voorste borst, bilaterale acromioclaviculaire gewrichten en lage rug.Laat de patiënt op de fiets ergometer zitten en plaats de armband van de sphygmomanometer op hen. Draag vervolgens het masker van gasanalyse voor de patiënt plaats de sensoren voor de pulsoximeter op de wijsvinger van de patiënt. Vertel de patiënt eerst minstens twee minuten te rusten om een stabiele basiswaarde te verkrijgen, inclusief de zuurstofverzadiging van hersenweefsel en de uitwisselingsverhouding van de luchtwegen.Laat de patiënt vervolgens de opwarmfase op de cyclus ergometer voltooien met een werksnelheid van 10 watt gedurende een minuut. Verhoog de snelheid met 10 watt per minuut en vraag de patiënt om te trappen bij ongeveer 60 tpm totdat hij er niet in slaagt om een cadans van meer dan 50 rpm bij te houden, ondanks sterke aanmoediging. Automatisch gemiddelde van de cerebrale weefsel zuurstofverzadiging waarde elke seconde van de gegevens gescand op een frequentie van 100 Hz.Automatisch meten van de bloeddruk om de twee minuten met de sphygmomanometer.Analyseer de gascomponent, adem-door-adem, met inbegrip van de zuurstofopname en de eindgetijdenkoolstofdioxidedruk. Laat de patiënt vervolgens de herstelfase gedurende twee tot zes minuten voltooien met een werksnelheid van nul watt. In deze studie, zowel hartfalen patiënten en gezonde controles onderging cardiopulmonale oefening testen die cerebrale weefsel zuurstofverzadiging monitoring door NIRS bevat.De cerebrale weefsel zuurstofverzadiging waarden worden gezien om kort aanzienlijk lager in de groep hartfalen dan in de controlegroep. In de groep hartfalen, het hersenweefsel zuurstofverzadiging in rust en op piek zijn lineair gecorreleerd met Brain Natriuretic Peptide, de piek zuurstofopname, en de zuurstofopname efficiëntie helling. Met name de cerebrale weefsel zuurstofverzadiging in rust wordt bepaald door de gedeeltelijke druk van de eindgetijden kooldioxide in rust, hemoglobine, en gemiddelde arteriële druk in rust.De procedure is eenvoudig en handig. Patiënten met een lage cerebrale verzadigingsindex garanderen hoge medische aandacht. Integratie van NIRS in CPET helpt de betrokkenheid van cerebrale hemodynamic onderdrukking af te bakenen waardoor intolerantie voor oefeningen bij patiënten met hartfalen ontstaat.Naast de prognostische waarde, moet het longitudinaal onderzoek bevestigen.
