SctO2は、脳組織における酸素送達と消費の割合を表す。脳飽和は、破壊された脳血流(CBF)、動脈酸素濃度の低下、および増加した脳組織酸素消費量11に関連する。心拍出量不全以外の高度なHFは、過換気を介して二酸化炭素(PaCO2)の動脈部分圧を減少させることによって間接的に脳血管収縮を誘発することにより、運動中に脳過灌流を引き起こす12.

心不全における脳組織飽和の程度は、機能的能力および疾患の重症度を反映することが証明されており、潜在的な予後値を有する。運動テスト中のラドンモニタリングは臨床価値がある。NIRSで測定した脳組織の酸素飽和度は簡単で便利です。このプロトコル統合NIRSは、心不全患者の運動検査の価値を高めるために、従来のCPETに統合された。この手順のデモンストレーションは、当研究室の技術者である劉劉有です。この手順を開始するには、アルコールパッドで患者の額を2回洗浄し、皮膚から汗や汚れを取り除きます。エミッタと検出器の間の距離が5cmの大型NIRSセンサーを取得し、両側で額にセンサーを配置します。センサーがしっかりと取り付けられていることを確認します。次に、EKG電極を前胸部、両側性関節関節、腰に塗布します。患者に自転車のエルゴメーターに座らせ、血圧計の腕章を置いさせます。次に、患者の人差し指にパルスオキシメーター用のセンサを配置するためのガス分析のマスクを着用する。まず、患者に少なくとも2分間休んで、脳組織の酸素飽和度および呼吸交換比を含む安定したベースライン値を得るように指示する。次に、患者に1分間10ワットの作業レートでサイクルエルゴメーターのウォームアップステージを完了させます。1分あたり10ワットのレートを上げ、強い励ましにもかかわらず50rpm以上のケイデンスに追いつきがつくまで、患者に約60rpmでペダルを踏むよう依頼してください。100 Hzの周波数でスキャンされたデータから脳組織の酸素飽和値を毎秒自動的に平均します。酸素取り込みと終潮二酸化炭素圧力を含むガス成分を呼吸ごとに分析します。次に、患者に2〜6分間ゼロワットの作業レートで回復段階を完了させる。本研究では、心不全患者と健康コントロールの両方が、NIRSによる脳組織の酸素飽和モニタリングを組み込んだ心肺運動検査を受けた。脳組織の酸素飽和値は、対照群よりも心不全群において有意に低くなるのが一時的に見られる。心不全群では、安静時およびピーク時の脳組織酸素飽和度は、脳性ナトリウレチンペプチド、ピーク酸素取り込み、および酸素取り込み効率の傾きと線形に相関している。特に、安静時の脳組織の酸素飽和度は、安静時の末潮二酸化炭素の分圧、ヘモグロビン、および安静時の動脈圧を意味することによって決定される。手順は簡単で便利です。低い脳飽和指数を有する患者は高い医学の注意を保証する。NIRSをCPETに統合することで、心不全患者に運動不耐症を引き起こす脳血力学的抑制の関与を示す。その予後的価値に加えて、それは確認するために縦方向の研究を必要とします。
