Chronische longinfecties met Pseudomonas aeruginosa zijn een belangrijke oorzaak van morbiditeit en mortaliteit bij patiënten met cystische fibrose (CF). Tijdens de vroege kindertijd worden patiënten gekoloniseerd door meerdere bacteriële pathogenen, waaronder niet-mucoïde isolaten van P. aeruginosa1,2. De totstandkoming van de kleine kolonievariant (SCV) isoleert evenals slijmervormige isolaten is een teller voor het begin aan chronische besmettingen. SCV isolaten zijn zeer resistente3 als gevolg van hun trage groeipercentages4, waardoor ze een ernstig afschrikmiddel in de behandeling regimenten en andere chronische infecties5 door P. aeruginosa. Werk van Al Ahmar et al.6 toonde een verband tussen SCV en slijmvlies verbonden door de novo pyrimidine biosynthese. Pyrimidinehonger, als gevolg van mutaties in genen die betrokken zijn bij de productie van pyrimidine, resulteerde in SCV fenotype in de nonmucoid referentiestam PAO1 en het slijmvliesderivaat PAO581 (PAO1mucA25).
Hoewel alginaatoverproductie een belangrijke ziektemarker is voor chronische longinfecties in CF, is het niet duidelijk of er een directe correlatie is tussen de hoeveelheid alginaat en longpathologie, en het is onduidelijk of alginaat kan worden gebruikt als prognosemarker voor behandeling7. De productie van alginaat wordt voornamelijk gereguleerd door twee operonen, een regelgevende operon (algUmucABCD)8,9 en de biosynthetische operon (algD operon)10,11. Alginaat productie wordt strak gereguleerd door de sigma factor AlgU9,12 (ook bekend als AlgT) en de afbraak van de anti-sigma factor MucA13. De mogelijkheid om de productie van alginaat ter plaatse te controleren van de sputummonsters van de patiënten kan helpen bij de ontwikkeling van nieuwe therapeutische opties.
Hier beschrijven we een groeitoestand die de aanwezigheid van SCV detecteert die wordt veroorzaakt door mutanten die de pyrimidine de novo niet kunnen synthetiseren. Suppletie van uracil en/of cytosine, de stikstofachtige basis van pyrimidine nucleotide, aan het medium activeert het bergingstraject, waardoor de normale groei in mutanten wordt hersteld. Deze groeimethode voor deze specifieke SCV mutanten kan worden gebruikt als een screeningmethode om pyrimidinemutaties in patiëntenmonsters te identificeren. Daarnaast bespreken we twee methoden voor detectie en meting van alginaat geproduceerd en afgescheiden door P. aeruginosa. De eerste is de traditionele methode14,15,16 van het vernederen van de polysaccharide met behulp van een hoge concentratie zuur en vervolgens het toevoegen van een colorimetrische indicator om de concentratie in het monster te kwantificeren. De tweede methode, ontwikkeld in ons laboratorium, maakt gebruik van de Enzym-Linked Immunosorbent Assay (ELISA) met behulp van een anti-alginaat monoklonale antilichaam (mAb) ontwikkeld door QED Biosciences. De ELISA-methode blijkt specifieker en gevoeliger te zijn dan de uronic zuurtest en maakt veiliger gebruik mogelijk door het vermijden van het sterk geconcentreerde zwavelzuur. Met het vermogen van de ELISA om direct op de sputummonsters van de patiënt te worden gebruikt om alginaat te meten, kan het worden ontwikkeld als een diagnostisch hulpmiddel voor monitoring om de hoeveelheid alginaat in de longen te volgen in verschillende perioden van de infectie.

Pseudomonas aeruginosa, een opportunistische Gram-negatieve bacteriële ziekteverwekker, kan overproduceren een exopolysaccharide alginaat resulteert in een unieke fenotype genaamd slijmvlies. Alginaat is gekoppeld aan chronische longinfecties wat resulteert in een slechte prognose bij patiënten met cystische fibrose (CF). Inzicht in de paden die de productie van alginaat reguleren, kan helpen bij de ontwikkeling van nieuwe therapeutische strategieën die gericht zijn op de alginaatvorming. Een ander ziektegerelateerd fenotype is de kleine kolonievariant (SCV). SCV is te wijten aan de trage groei van bacteriën en vaak geassocieerd met verhoogde resistentie tegen antimicrobiële stoffen. In dit artikel tonen we eerst een methode om een genetisch gedefinieerde vorm van P. aeruginosa SCV te kweken als gevolg van pyrimidine biosynthesemutaties. Suppletie van stikstofhoudende bases, uracil of cytosine, geeft de normale groei terug aan deze mutanten, het aantonen van de aanwezigheid van een bergingstraject dat vrije bases uit het milieu scharrelt. Vervolgens bespreken we twee methoden voor het meten van bacteriële alginaat. De eerste methode is gebaseerd op de hydrolyse van de polysaccharide tot zijn uronic acid monomeer, gevolgd door derivatisatie met een chromogene reagens, carbazole, terwijl de tweede methode een ELISA gebruikt op basis van een commercieel verkrijgbare, alginaat-specifieke mAb. Beide methoden vereisen een standaardcurve voor kwantificering. We tonen ook aan dat de immunologische methode specifiek is voor alginaatkifie en kan worden gebruikt voor de meting van alginaat in de klinische monsters.

Figuur 1 toont platen van PAO1 en PAO581 met of zonder in-frame deletie in het pyrd-gen (een gen in de pyrimidine biosyntheseroute) die resulteert in SCV6. De PAO1 SCV mutant werd hersteld tot een normale groei in reactie op uracil suppletie(figuur 1A,B). De kleine kolonievariant of SCV techniek maakt selectie en groei van kleine kolonievarianten van Pseudomonas aeruginosa mogelijk. De methode is zeer definitief en maakt gemakkelijker selectie dan normale methoden. Dit protocol beschrijft ook een veiligere, meer gevoelige ELISA-methode voor alginaat kwantificering in vergelijking met de standaard uronic acid carbazole-methode, waardoor directe alginaatkwantificering mogelijk is in monsters zonder cautery of monstermanipulatie.Het aantonen van de procedure zal Brandon Kirby, een lab technicus, en Roy Al Ahmar, een afgestudeerde student van mijn laboratorium. Om de kleine kolonievariant of SCV te detecteren, streept u eerst de P.aeruginosa-stam PAO1 delta PYRD op voorverwarmde PIA-platen. Laat de stammen 48 uur lang groeien op 37 graden Celsius.Op de groeiplaat, identificeer een enkele kolonie isolaat dat de SCV fenotype gekenmerkt door een kolonie grootte van een tot drie millimeter in tegenstelling tot de normale drie tot vijf millimeter kolonie grootte heeft. Om fysiologische activering van het bergingspad uit te voeren, gebruikt u een steriele inentingslus om de SCV-kolonie van de PIA-plaat te plukken.Streep de geselecteerde kolonie op een voorverwarmde PIA plaat aangevuld met 0,1 millimolar uracil. Laat de plaat 24 tot 48 uur groeien op 37 graden Celsius. Om te beginnen met de uronic acid carbazole test, identificeren van een enkele kolonie uit een zuivere cultuur van de gewenste stam te testen en kies de kolonie met behulp van een steriele tandenstoker.Plaats de tandenstoker in een kweek reageerbuis met vijf milliliter PIB. Groeien in een shaker incubator op 37 graden Celsius voor 24 uur. Voeg na incubatie een aliquot van de gekweekte PIB bouillon toe aan een voorverwarmde PIA-plaat.Met behulp van een steriele celspreader, verspreid de bouillon over de plaat en groeien op 37 graden Celsius voor 24 uur. Voeg met behulp van een pipetcontroller en een steriele pipet van 50 milliliter 0,85% natriumchloride toe aan het volwassen gazon en verzamel het monster door de plaat te schrapen met een celspreider. Aspirate het monster met behulp van een verse 50 milliliter pipet en breng over in een 50 milliliter collectie buis.Vortex het monster op hoog te mengen en plaats de monsters op ijs. Om de OD600 van de monsters te meten, moet u eerst de spectrofotometer leegmaken met één milliliter natriumchloride van 0,85%. Voeg vervolgens een milliliter van het monster toe aan een nieuwe wegwerpvvette en lees de OD. Herhaal deze stap twee keer om een drievoud van reads voor elk monster te verkrijgen.Voeg nu drie milliliter van de zwavelzuur borate oplossing in cultuur buizen en laat zitten op ijs. Voeg 350 microliter van het verzamelde monster langzaam toe aan het zuurmengsel in de reageerbuizen. Na kort vortexen op laag, voeg 100 microliter van 0,1% carbazool en ethanol oplossing aan het zuurmonster mengsel.Dop de buis en vortex vijf seconden op de medium instelling. Plaats vervolgens in een droog bad op 55 graden Celsius gedurende 30 minuten. Na incubatie, vortex de buizen kort op hoog en laat afkoelen gedurende vijf minuten.Gebruik de buis met 0,85% natriumchloride als blanco op de spectrofotometer. Voeg vervolgens een milliliter van het mengsel toe aan een schone cuvette en lees de OD van de monsters op 530 nanometer op een spectrofotometer. Bereid een standaardcurve voor door de OD530 te meten van seriële verdunningen van bekende concentraties d-mannuronzuur.Herhaal twee maal en extraheer een lineaire vergelijking uit deze metingen. Bereken de concentratie van de alginaat in elk monster met behulp van de standaardcurve en verdeel de alginaatconcentratie van lineaire vergelijking door OD600 om de totale hoeveelheid alginaat per OD600 te verkrijgen. Voeg met behulp van een micropipet 50 microliter van het verzamelde monster toe aan een onbehandelde 96-putplaat.Voeg vervolgens 50 microliter ELISA coating buffer toe aan de putten. Broed de plaat twee uur lang op 37 graden Celsius. Met behulp van een spuitfles, was de plaat putten twee keer met PBS-T door het vullen van de putten en vervolgens aftappen ze door het omdraaien van de plaat over.Voeg 200 microliters van het blokkeren buffer aan de putten met een micropipet en incubbaat op vier graden Celsius 's nachts. De volgende dag, was de plaat twee keer met PBS-T als voorheen. Voeg vervolgens 100 microliter verdunde primaire antilichamen toe aan de putten en broed gedurende 37 graden Celsius gedurende één tot twee uur.Na incubatie, was de plaat putten drie keer met PBS-T als voorheen. Voeg vervolgens 100 microliter verdund secundair antilichaam toe aan de putten en incubbate bij 37 graden Celsius gedurende één tot twee uur. Na het wassen van de plaat opnieuw drie keer, gebruik een micropipet om 100 microliter tmb ELISA oplossing toe te voegen.Incubeer de plaat op kamertemperatuur gedurende 30 minuten in het donker door het plaatsen van de plaat in een lade of wikkelen in folie. Voeg vervolgens 100 microliter stopoplossing toe. Gebruik een plaatlezer om de OD op 450 nanometer te meten.Produceer een standaardcurve door de OD450 van seriële verdunningen van bekende concentraties d-mannuronzuur te meten. Herhaal de metingen twee keer en extraheer een lineaire vergelijking. Bereken de concentratie van de alginaat in elk monster met behulp van de standaardcurve en verdeel de alginaatconcentratie van de lineaire vergelijking door OD600 om de totale hoeveelheid alginaat per OD600 te verkrijgen.Hier zijn monsters van PAO581 en PAO581 met mutaties in genen die pyrimidine de novo biosynthese reguleren geteeld op PIA en PIA aangevuld met 0,1 millimolar uracil. De gegevens tonen aan dat de aanwezigheid van uracil in de media resulteert in de omzetting van de mutant stam terug naar slijmvisme zoals gezien door de herstelde alginaat productie. Hier worden resultaten getoond voor het anti-alginaat monoklonale antilichaam gebaseerde ELISA.Kolonies worden geteeld op PIA platen met arabinose voor de inductie van de PBAD promotor en de pHERD-20T plasmide. Getoond zijn PAO1, PAO1 die de uitdrukking plasmid pHERD-20T met de belangrijkste alginaat specifieke sigma factor algU, en PAO1 met een in-frame schrapping van het algD-gen bij het coderen van de belangrijkste alginaat biosynthetische enzym GDP-mannose dehydrogenase. Deze gegevens vergelijken de niet-mucoide niveaus van alginaat gemeten voor PAO1 en PAO1 delta algD versus de mucoid niveaus van alginaat gemeten voor PAO1 met pHERD-20T algU.Anti-alginaat ELISA werd ook getest op patiënt sputum monsters zonder voorafgaande groei op platen. Drie CF sputum monsters die de groei van mucoid P.aeruginosa had toonde detecteerbare alginaat in vergelijking met twee patiënt sputum monsters die ofwel niet-mucoid P.aeruginosa of geen P.aeruginosa groei bevatte. De voorbereiding van de standaardcurven voor de alginaatkwantificering is van cruciaal belang, aangezien dit een nauwkeurige en nauwkeurige meting van alginaat in het monster mogelijk maakt.De zuuroplossing die in de carbazole-methode wordt gebruikt, is zeer gevaarlijk, dus goede persoonlijke beschermingsmiddelen en voorzorgsmaatregelen moeten worden gebruikt om de veiligheid te garanderen. Na het isoleren van SOV's kan verdere genetische profilering worden gedaan om mutaties die voorkomen en de pathogeniteit die aan dergelijke mutaties wordt geassocieerd, beter te begrijpen. De ELISA-methode kan worden aangepast om chronische longinfectie te monitoren bij patiënten met cystische fibrose.Bovendien kan onze groeimethode helpen bij het diagnosticeren van infecties veroorzaakt door specifieke SPV's.
