De rat halsslagader ballon letsel is een van de meest gebruikte en bestudeerde restenosis diermodellen. Zowel het oorspronkelijke ballonletselmodel3 als de gewijzigde drukgecontroleerde segmentale verwondingsvariatie10 hebben veel aspecten van de arteriële letselrespons die ook bij mensen optreedt, geïnformeerd, met als enkele beperkingen dat fibrinerijke trombus zich zelden ontwikkelt en lokale ontsteking minimaal is in vergelijking met andere letselmodellen zoals bij hy...

Hart- en vaatziekten (CVD) blijft wereldwijd de belangrijkste doodsoorzaak1. Atherosclerose is de onderliggende oorzaak van de meeste CVD-gerelateerde morbiditeit en mortaliteit. Atherosclerose is de opbouw van plaque in slagaders die resulteert in een vernauwd lumen, waardoor een goede bloedperfusie naar organen en distale weefsels wordt belemmerd2. Klinische interventies voor de behandeling van ernstige atherosclerose omvatten ballon-angioplastiek met of zonder stentplaatsing. Deze ingreep omvat het oppompen van een ballonkatheter naar de plaats van plaque en het opblazen van de ballon om de plaque naar de arteriële wand te comprimeren, waardoor het luminale gebied wordt verbreed. Deze procedure beschadigt echter de slagader en initieert de arteriële letselrespons3. Langdurige activering van deze letselrespons leidt tot arteriële restenose, of hervernauwing, secundair aan neointimale hyperplasie en remodellering van bloedvaten. Tijdens angioplastiek wordt de intimale laag gedenudeerd van endotheelcellen, wat leidt tot onmiddellijke werving van bloedplaatjes en lokale ontstekingen. Lokale signalering induceert fenotypische veranderingen in vasculaire gladde spiercellen (VSMC) en adventitiale fibroblasten. Dit leidt tot de migratie en proliferatie van VSMC en fibroblasten naar binnen naar het lumen, wat leidt tot neointimale hyperplasie4,5. Circulerende voorlopercellen en immuuncellen dragen ook bij aan het totale volume van restenose6. Indien van toepassing zijn drug-eluting stents (DES) de huidige standaard voor het remmen van restenosis7. DES remt echter arteriële re-endothelialisatie, waardoor een pro-trombotische omgeving ontstaat die kan leiden tot late in-stent trombose8. Daarom zijn diermodellen integraal voor zowel het begrijpen van de pathofysiologie van restenose als voor het ontwikkelen van betere therapeutische strategieën om de werkzaamheid van revascularisatieprocedures te verlengen.
Verschillende grote en kleine diermodellen9 worden gebruikt voor het bestuderen van deze pathologie. Deze omvatten ballonletsel3,10 of draadletsel11 van de luminale kant van een slagader, evenals gedeeltelijke ligatie12 of manchetplaatsing13 rond de slagader. De ballon- en draadletselen denudeen beide de endotheellaag van de slagader en bootsen na wat klinisch gebeurt na angioplastiek. In het bijzonder maken ballonletselmodellen gebruik van vergelijkbare hulpmiddelen als in de klinische omgeving (d.w.z. ballonkatheter). De ballonblessure kan het beste worden uitgevoerd in rattenmodellen, omdat rattenslagaders een geschikte maat zijn voor in de handel verkrijgbare ballonkatheters. Hierin beschrijven we een drukgecontroleerde segmentale arteriële verwonding, een gevestigde, gewijzigde versie van de ballonletsel van de ratcarotisslagader. Deze drukgecontroleerde benadering bootst de klinische angioplastiekprocedure nauw na en maakt reproduceerbare neointimale hyperplasievorming mogelijk twee weken na verwonding14,15. Bovendien resulteert deze drukgecontroleerde arteriële verwonding in volledige endotheellaagherstel met 2 weken na operatie16. Dit staat in schril contrast met het oorspronkelijke ballonletselmodel, beschreven door Clowes, waarbij de endotheellaag nooit terugkeert naar volledige dekking3.
Na de operatie kunnen therapeutische middelen via verschillende benaderingen op de gewonde slagader worden aangebracht of gericht. De hierin beschreven methode maakt gebruik van periadventitial toepassing van een klein molecuul ingebed in een Pluronische geloplossing. In het bijzonder brengen we een oplossing van 100 μM cinnamic aldehyde in 25% Pluronisch-F127 gel direct na verwonding aan op de slagader om neointimale hyperplasievorming te remmen15. Pluronisch-F127 is een niet-toxische, thermokeerbare gel die in staat is om geneesmiddelen lokaal op een gecontroleerde manier af te leveren17. Ondertussen is arteriële verwonding lokaal, vandaar dat de lokale overheid het mogelijk maakt om een actief principe te testen en tegelijkertijd off-target effecten te minimaliseren. Niettemin zal de effectieve levering van een therapeutisch gebruik van deze methode afhangen van de chemie van het kleine molecuul of de biologische gebruikt.

Hart- en vaatziekten blijven wereldwijd de belangrijkste doodsoorzaak en invaliditeit, deels als gevolg van atherosclerose. Atherosclerotische plaque vernauwt het luminale oppervlak in slagaders, waardoor voldoende bloedtoevoer naar organen en distale weefsels wordt verminderd. Klinisch gezien zijn revascularisatieprocedures zoals ballonangioplastiek met of zonder stentplaatsing gericht op het herstellen van de bloedstroom. Hoewel deze procedures de bloedstroom herstellen door de plaquelast te verminderen, beschadigen ze de vaatwand, die de arteriële genezingsreactie initieert. De langdurige genezingsreactie veroorzaakt arteriële restenose, of opnieuw vernauwing, waardoor uiteindelijk het succes op lange termijn van deze revascularisatieprocedures wordt beperkt. Daarom zijn preklinische diermodellen integraal voor het analyseren van de pathofysiologische mechanismen die restenose veroorzaken en bieden ze de mogelijkheid om nieuwe therapeutische strategieën te testen. Murine-modellen zijn goedkoper en gemakkelijker te bedienen dan grote diermodellen. Ballon- of draadletsel zijn de twee algemeen aanvaarde letselmodaliteiten die in murine-modellen worden gebruikt. Met name ballonletselmodellen bootsen de klinische angioplastiekprocedure na en veroorzaken voldoende schade aan de slagader voor de ontwikkeling van restenose. Hierin beschrijven we de chirurgische details voor het uitvoeren en histologisch analyseren van het aangepaste, drukgecontroleerde ratcarotis slagaderballonletselmodel. Bovendien benadrukt dit protocol hoe lokale periadventitial toepassing van therapieën kan worden gebruikt om neointimale hyperplasie te remmen. Ten slotte presenteren we lichtplaatfluorescentiemicroscopie als een nieuwe benadering voor beeldvorming en visualisatie van de arteriële verwonding in drie dimensies.

Figuur 1 toont alle materialen en chirurgische hulpmiddelen die worden gebruikt om deze operatie uit te voeren. Hematoxyline & eosine (H&E) kleuring van twee weken gewonde arteriële dwarsdoorsneden zorgt voor een duidelijke visualisatie van neointimale hyperplasie.
