골관절염(OA)은 세계에서 가장 흔한 관절 질환으로 성인 인구의 약 10-12%에 영향을 미칩니다1. 가장 일반적으로 관련된 관절은 무릎이며 OA는 노인,특히 여성에서 더 높은 발병률을 보입니다2. 만성 질환인 OA는 연골 손실, 활액 염증, 골형성증, 기능 저하 및만성 통증과 같은 증상과 함께 수십 년에 걸쳐 점진적으로 관절 부전으로 발전합니다3. 세계 보건기구 (WHO)에 따르면 OA는 여성에서 네 번째로 널리 퍼진 질병이며 남성에서 8 번째로 널리 퍼진 질병입니다. 2020년까지 골관절염은 인간에게 네 번째로 장애를 일으키는 질병이 될 수 있습니다4. 그러나 현재 이용 가능한 OA 치료법은 증상만을 해결하고 관절 치환술까지 기간을 연장합니다5.
인간 환자의 자발적인 OA는 종종 관절 관련 통증과 같은 임상 증상을 나타내는 데 오랜 시간이 걸립니다6. OA의 초기 단계에서 통증은 일반적으로 간헐적이며 질병이 진행됨에 따라 더 빈번하고 심해져 환자의 주된 불만이됩니다7. 따라서 통증 완화 요법을 촉진하기 위해 지난 반세기 동안 OA 통증에 대한 광범위한 동물 모델이 개발되었습니다. OA 모델은 고전적으로 자발적 모델과 유도 모델로 나뉩니다. 자발적 모델에는 자연 발생 모델과 유전자 변형 모델이 포함되며,이는 인간에서 일차 OA의 과정을 보다 밀접하게 시뮬레이션할 수 있습니다8. 유도된 모델은 일반적으로 두 가지 범주로 나눌 수 있다: 1) 수술 또는 다른 외상에 의해 유도된 외상후 OA; 또는 2) 연골 독성 또는 전 염증성 물질의 관절 내 주사3. 이러한 모델은 OA의 병태생리학적 연구를 위한 토대를 마련하고 통증을 줄이고 기능을 증가시키는 약물 개발에 크게 기여합니다.
최근 OA 모델링에 가장 널리 사용되는 유도제는 모노 요오드 아세테이트 (MIA)입니다. 글리 세르 알데히드 -3- 인산 탈수소 효소의 억제제 인 MIA는 연골 기질의 변화, 분해, 연골 손실, 활막염 및 인간 골관절염의 병리학 적 변화와 유사한 기타 변화를 일으킬 수 있습니다9. MIA의 관절내 주사는 MIA 투여 후 28일에 지속적인 통증을 유발했으며, 이는 MIA 모델이 만성 침해수용성 통증10,11,12를 조사하는데 유용할 수 있음을 나타낸다. 이 연구에서 수컷 Sprague-Dawley 쥐는 무릎 관절에 0.5, 1.5 또는 3mg의 MIA가 포함 된 관절 내 주사를 받았습니다. MIA-유도된 관절 통증의 중증도는 주사 후 1, 7, 14, 21, 28 및 35일에 기계적 및 열적 민감도의 평가에 의해 측정되었다. 이를 바탕으로 주사 후 28 일에 보행 패턴과 조직 학적 변화를 평가하기 위해 1.5mg의 MIA를 최종 농도로 선택했습니다.
