1. 设置

icVEP 的不同设置可以刺激不同的 M 细胞通路并创建不同的 EEG 信号。在时间频率（15赫兹）的光度对比条件下，一项涉及15名OAG患者和14名正常观察者的研究表明，灵敏度为73.33%，特异性为100%22。然而，其中一半的患者有先进的OAG。因此，对于早期OAG，由于样本量小，无法估计灵敏度。

SNR 定义为 FFC 平均振幅与 95% 置信圈半径的比率。> 1 的 SNR 表示在 0.05 水平下有显著反应，这意味着视神经中正常的电生理活动。SNR = 1 表示响应类似于或弱于 0.05 水平的背景噪声，这意味着视神经中电生理活动异常。然而，在目前使用ROC曲线的研究中，0.93的SNR是早期OAG患者和控制对象歧视的最佳方法。因此，对于本研究，0.93 的 SNR 标准可以区分早期 OAG 患者中 GON 的严重程度。
超过50%的M细胞在黄斑区域;因此，如果受刺激，很可能有一个强信号，导致SNR > 1。因此，2×2阵列固定交叉在屏幕的中心没有正弦时间信号能够提示方便仔细固定，以及避免假负错误与不良固定34。此外，最近的SD-OCT研究证明，在黄斑地区的RGC即使在青光眼的早期阶段也受到损害，因为蛋白细胞解和继发性斧头切除术后，对视神经头部的损伤可能会导致RGC凋亡35，36，37，38。35,36,37,38
根据HFA中的模式偏差，对当前研究中的16个中心测试点进行分析，该测试点与Bjerrum区域的5°-10°相对应，其中近一半的M细胞分布10、11、12、13、14。10,11,12,13,14本研究显示不同可能性标准与SNR（负R值）负相关异常测试点的数量;但是，只有当 p < 0.5% 的相关性显著时，才表明 icVEP 能够检测功能异常并反映早期 OAG 中中央视场损失的严重性。
据报道，在青光眼的早期阶段，即使没有中央视场测试26的功能参与，对P细胞和M-ON通路刺激的反应也受到严重干扰。然而，本研究的一个限制是，icVEP测试要求BCVA值大于0.3的患者，球面折射在-6和+3屈光之间，以及透明的眼介质。研究只显示icVEP在早期OAG眼睛中具有更好的视觉敏锐性的有用性。因此，需要进一步的研究，以创造更好的刺激，并定义更准确的OAG眼睛与较差的视力标准。这将有助于确定icVEP能否作为区分青光眼怀疑者的最佳功能测试，以及OAG的早产和早期阶段。此外，另一个限制是，该研究没有考虑到主导性眼睛和非显性眼睛之间的差异。这些通路和测试这两只眼睛之间的差异可能会影响脑电图信号。最重要的是，在进行进一步研究后，流程图将得到改进。
总之，icVEP能够检测近70%的早期OAG患者的视觉功能异常，特异性约为95%。测量的功能与标准自动测量的中央 11° 视场损耗的严重程度和 OCT 检测到的 RNFL 厚度的降低相关。因此，icVEP可以作为一个有用和客观的电生理视觉场功能测试，用于诊断早期OAG。

开角青光眼（OAG）是一种慢性、不可逆转的疾病，也是失明的主要原因之一。先前的研究表明，视场测试是目前用于高知性视觉损失检测的黄金标准，基于传统的标准自动围测量（SAP），在20%-40%的视网膜结节细胞（RDC）受损1，2之前无法检测早期高知性功能损失。此外，SAP也被证明只有中等测试重新测试的可靠性，因为它是一个主观的心理物理测试和耗时的任务，病人3。
客观电生理视场功能测量在检测青光眼时具有较好的测试可靠性。此类措施包括多焦视觉唤起电位 （mfVEP） 和图案电雷电图 （pERG）。然而，pERG无法提供地形信息，mfVEP比SAP,,4、5、6、7、84,57,更耗时。6幸运的是，隔离检查视觉诱发电位（icVEP）最近被设计为一种额外的技术，以检测更早和更快的高大损伤9。

assessing early stage open-angle glaucoma in patients by isolated-check visual evoked potential

最近的研究表明，青光眼诊断的icVEP的准确率在91%-100%9，22，269,22,之间。这里介绍了中国的横截面研究，以进一步评估icVEP在早期OAG的潜在诊断价值。
科目 受试者为2015年及2016年北京大学第三医院眼科眼科招募的OAG患者和健康志愿者。OAG患者的纳入标准包括：25-75岁;最佳校正的视觉敏锐度 （BCVA） < 0.3 （最小分辨率角度的对数， 日志 MAR）;-6 和 +3 屈光之间的球面折射;和透明的眼部介质。此外，患者还展示了OAG（具有开放角、视场缺陷的受试者，与高光神经病变（GON]相对应，并且具有正常或升高的IAOP，无继发原因），其中IAOP在医学上控制良好，并且有可靠的视场测试结果（假阳性错误=20%，假阴性错误=20%，固定损失=30%）显示了SAP上早期的高大视场缺陷。
控制受试者的纳入标准包括：没有眼部异常，特别是任何眼睛没有贡则;和一个正常的 Iop， 从来没有超过 21 毫米汞柱升高。排除标准包括：糖尿病或任何其他全身性疾病;眼病或神经病史;不均等瞳孔直径和瞳孔直径 < 2.0 mm;固定不良;目前使用可能影响视场敏感性的药物（即麻丁醇、羟基氯奎、氯丙胺）;和以前眼内手术或难治手术的历史。
OAG 诊断检查 对于所有患者，使用眼镜校正来减少模糊对视场灵敏度的可能影响。汉弗莱现场分析仪 II 30-2 SITA 标准程序在基线上至少执行了两次可靠的 SAP 测试。本研究采用第二个可靠的视场结果，以尽量减少学习效果27。发光视场损耗的早期阶段被定义为 = -6.00 dB 的均值偏差 （MD），并且至少有一个：1） 在视觉场抑郁 < 5% 水平的预期位置存在 = 3 点的聚类，其中至少有一个是模式偏差图上的 < 1% 水平;2） 修正型板标准差或型板标准差显著在 p < 0.05;3）青光眼中场测试结果"超出正常限值"28。
基线检查包括视觉敏锐度和折射测试、自然光用尺子进行瞳孔直径测量、狭缝灯生物显微镜、古生物显微镜、戈德曼应用测量（GAT）以及所有科目的扩张立体基金测试。
在收到icVEP测试报告的第二天，GAT在收到icVEP测试报告后一天（当地时间上午8点至11点）测量基线 IOP。每位患者还接受中央角膜厚度（CCT）测量使用超声波膜测量下局部麻醉29。平均连续记录了五次读数。
在瞳孔扩张后，从每个患者获得立体基金照片，并由两位经验丰富的医生以蒙面方式进行评估。两位医生之间的差异要么通过协商一致解决，要么由第三位有经验的医生裁决解决。GON 被定义为至少一项： 1） 上轮或下轮圈的轮圈与光盘比率为 < 0.1;2）存在视网膜神经纤维层（RNFL）缺陷;3）光学光盘显示夹板出血30，31。30,
每位患者还接受光学相干断层扫描 （OCT） 测试，以确认与立体照片和 HFA 结果对应的 RNFL 缺陷。RNFL 厚度在时间优越 （TS） 象限和时差 （TI） 象限中的更改计算如下：RNFL 厚度的变化 = RNFL 厚度值 - 来自正常人数据库的标准值 （图 4B） 。
统计分析 当两只眼睛都符合包含标准时，随机选择一只眼睛进行分析。每个科目需要在 3 个月内建立所有数据。SPSS 22.0 统计包与统计测试使用如下：独立的样本 t 测试用于正向分布的变量;曼-惠特尼 U 测试用于非正则分布的数字变量;和二项变量比较奇方测试或费舍尔的确切测试，必要时。采用接收机-操作特性（ROC）曲线分析估计存在高大损伤的预测精度32。Pearson 相关系数用于分析 SNR 与 OCT 参数之间的相关性，以及 SNR 与 SAP 中央 11° 场异常之间的相关性。如果 p < 0.05，则差异被认为显著。
结果 共有44名OAG患者和39名对照对象包含完整的数据。这些受试者在icVEP测试期间都没有抱怨。所有83名受试者为中国人（48名男性和35名女性），平均年龄为48.54~16.70岁（25-74岁）。患者与对控组在年龄、性别、右/左眼、BCVA、球形等价物或瞳孔直径方面不存在统计差异（表1，p> 0.05），但患者的SNR明显低于对控组（表1，p< 0.05）。
关于icVEP结果，早期OAG患者的30只眼睛是SNR阳性（68.18%）对照组只有两只眼睛（5.13%）。使用 SNR 标准 1，icVEP 显示 68.18% 的灵敏度和 94.87% 的特异性诊断早期 OAG（计算精度为 67/83 [80.72%]）。然而，ROC 分析表明，先验SNR 标准 0.93 是患者与对照对象之间区分的最佳标准（图 5）。使用 SNR 标准 0.93，测试的特异性达到 100%，灵敏度为 65.90%（计算精度为 82.10%）。
对于患者，异常在中央11°视场测试（HFA，模式偏差，中央16个测试点;图4C） 由具有不同可能性条件的异常点数计算。在标准级别 p < 0.5 时，中央 11° 视场中的异常测试点数量与 SNR（p < 0.05，r = -0.332，表 2 ）显著负相关。 r时间上级象限中 RNFL 的厚度变化与 SNR（p < 0.05，r = 0.370，表 2）显著正相关，而其他眼睛的 SAP-MD、SAP-MD、时下级象限中 RNFL 的厚度变化以及基准 IOP 和 CCT 均与 SNR（p > 0.05，表 3 ） 不相关。 r
