الهدف الرئيسي للتدخل الجراحي هو قياس الآثار الأيضية ومقاومة الأنسولين من استئصال المعدة كم في نموذج الفأر أكثر تقييما. يهدف هذا البروتوكول إلى إنتاج نموذج حيواني للبحث في الآليات التي تؤكد النتائج التي شوهدت بعد جراحة لعلاج البدانة في البشر ، والتي قد توفر رؤى حول التسبب في السمنة وعلاج مرض السكري من النوع 2. بالنسبة لاستئصال المعدة والزُم، وبعد التأكد من العمق المناسب للتخدير عن طريق تحفيز اصبي اعب، قم بتطبيق جل العين Carbomer 0.2٪ على عين الفأر لمنع جفاف العينين.يتم وضع الماوس وثابتة على طاولة الجراحية. إزالة وتنظيف الشعر المتبقي مع كريم إزالة الشعر. تطهير البطن من الماوس مع حلول بوديوني اليود بالتناوب مع الكحول لثلاثة تطبيقات.تنفيذ الإجراء SG بمساعدة مجهر مكبر تشريح أو المكبر حسب الضرورة لمنع أي نزيف لا يمكن التنبؤ بها وصدمة نقص في الدم. جعل واحد إلى 1.5 سنتيمتر في طول شق خط الوسط في الجزء العلوي من البطن. استخدام اثنين من منحنية على نحو سلس مسننة صغيرة ملقط لنقل بلطف المعدة واستكمال خارجي.حرك الأمعاء إلى الجلد العاري على الجانب واغطيها بالشاش المبلل المحلولي الدافئ لمنع الجفاف. تقسيم بعناية الرباط المعدي المعوي يربط المعدة اليسرى إلى الطحال باستخدام مسبار القطن يميل والكهرباء حسب الحاجة، وبالتالي تشريح صندوق المعدة من الطحال المحيطة والأعضاء الداخلية الأخرى. تمتد على القاع والبواد من المعدة بلطف واتّصاليا مع ملقط، وتحديد خط الوسط في المعدة وتطبيق بعناية اثنين من مقاطع الجراحية باستخدام قضيب LIGACLIP إلى الجزء 15٪ من وسط المعدة من تقاطع المعدة والمريء أدنى والبوادل متفوقة.المشبك وعزل حوالي 75-80٪ من المعدة، وبالتالي خلق الأكمام الجانبي بأكملها من المعدة. استرجع المعدة المعزولة الجانبيّة باستخدام مُتَرّسِس. استخدام مسحات القطن لإزالة أي الطعام المتبقية من المعدة ومن ثم تنظيف حافة قطع من المعدة مع محلول مائي بوديوني اليود.Oversew خط مقطع مع خياطة أحادية 5-0 غير قابل للتسرب للتأكد من عدم وجود تسرب. عقدة نهايات خياطة ومرساة إلى لقطات على أي من الطرفين. إعادة المعدة والأمعاء إلى الوضع المناسب في تجويف البطن وإغلاق البطن مع خياطة أحادية 5-0 غير قابلة للغلق على جدار البطن.لإجراء صوري من SG، تنفيذ إجراء مماثل كما هو موضح سابقا مع استئصال البطن في منتصف الخط، externalize الأمعاء والمعدة باستخدام 37 درجة مئوية الرطب تغطية الشاش الملحية الدافئة لمدة خمس دقائق. إعادة المعدة والأمعاء إلى المواقع المناسبة لهذه الأعضاء الداخلية. إدارة كيتوبروفين 2-5 ملليغرام للكيلوغرام الواحد وcefazolin 25 ملليغرام لكل كيلوغرام تحت الجلد أو داخل الرحم بعد الجراحة للسيطرة على الألم والوقاية من العدوى.وقف isoflurane والاستمرار مع تدفق الهواء غرفة من ثلاثة إلى خمسة لترات في الدقيقة حتى الماوس مستيقظا تماما. استمر في مشاهدة الماوس بينما يستعيد القدرة على الحركة ويبدأ في التجول في القفص. ضع الفأر في الحاضنة المستقلة حوالي 30 درجة مئوية في حالة درجة الحرارة لمدة خمسة أيام ، مع التأكد من وجود فأر واحد فقط لكل قفص.في أيدينا من ذوي الخبرة، كان المعدل الناجح لـ SG حوالي 90٪ بعد فترة التعلم. وزن الجسم من صور تعمل والنظام الغذائي عالية الدهون تغذية الفئران المكتسبة باطراد. ولكن استئصال المعدة كم والنظام الغذائي عالية الدهون تغذية الفئران انخفض بشكل مستمر.وكان الفرق في مستوى الكوليسترول كبير بين النظام الغذائي عالية الدهون تغذية الفئران مع استئصال المعدة الأكمام والأكمام. وهذا قد يشير إلى أن تحسين الإجهاد التأسد بعد استئصال المعدة الأكمام. وكان الفرق بين مستويات الهيموجلوبين A1C و IPGTT كبيرة كذلك.بين النظام الغذائي عالي الدهون تغذية الفئران مع شموخ ومكم والتم، وتشير إلى مقاومة الأنسولين والسكر تحسين التحكم بعد استئصال المعدة كم. يمكن أن يساعدنا هذا النموذج الحيواني على التحقيق في الأهمية السريرية لمرض السكري والسمنة في استخدام إجراء جراحي قصير مع استئصال المعدة. يمكن أن يقلل هذا الإجراء من وقت العملية ويحسن معدل البقاء على قيد الحياة الجراحي.الميزة الرئيسية لهذه الدراسة هي تقليد النتائج السريرية لجراحة البدانة في المرضى البشريين باستخدام نموذج فأر جراحي بسيط.
