शल्य चिकित्सा हस्तक्षेप का मुख्य लक्ष्य मेटाबोलिक प्रभाव और अधिक मूल्यांकन योग्य माउस मॉडल में आस्तीन गैस्ट्रेक्टॉमी के इंसुलिन प्रतिरोध को मापना है। इस प्रोटोकॉल का उद्देश्य उन तंत्रों के शोध के लिए एक पशु मॉडल का उत्पादन करना है जो मनुष्यों में बैरिएट्रिक सर्जरी के बाद देखे गए परिणामों को रेखांकित करते हैं, जो मोटापे और टाइप 2 मधुमेह उपचार के रोगजनन में अंतर्दृष्टि प्रदान कर सकते हैं। आस्तीन गैस्ट्रेक्टॉमी और नकली ऑपरेशन के लिए, पैर की अंगुली उत्तेजना द्वारा संज्ञाहरण की उचित गहराई की पुष्टि करने के बाद, सूखी आंखों को रोकने के लिए माउस की आंखों पर कार्बोमर 0.2% आई जेल लागू करें।माउस रखा और सर्जिकल टेबल पर तय किया गया है। बचे हुए बालों को डिपिलेटरी क्रीम से निकालें और साफ करें। माउस के पेट को तीन अनुप्रयोगों के लिए शराब के साथ बारी-बारी से पोविडोन-आयोडीन समाधानों के साथ कीटाणुरहित करें।किसी भी अप्रत्याशित रक्तस्राव और हाइपोवोलेमिक सदमे को रोकने के लिए आवश्यक के रूप में एक आवर्धक विच्छेदन माइक्रोस्कोप या आवर्धक की सहायता से एसजी प्रक्रिया का प्रदर्शन करें। पेट के ऊपरी हिस्से में लंबाई मिडलाइन चीरा में एक से डेढ़ सेंटीमीटर करें। पेट को धीरे-धीरे स्थानांतरित करने और इसे बाह्य करने के लिए दो घुमावदार चिकनी दांतेदार माइक्रो-संदंश का उपयोग करें।आंत को साइड में नंगे त्वचा पर ले जाएं और निर्जलीकरण को रोकने के लिए गर्म नमकीन गीली धुंध के साथ कवर करें। जरूरत के अनुसार कपास-इत्तला दी गई जांच और इलेक्ट्रोक्यूटरी का उपयोग करके बाएं पेट को तिल्ली से जोड़ने वाले गैस्ट्रोस्प्लेनिक स्नायु को सावधानीपूर्वक विभाजित करें, जिससे आसपास की तिल्ली और अन्य आंतरिक अंगों से गैस्ट्रिक फंडस को विच्छेदन किया जा सके। पेट के फंडस और पाइलोरस को धीरे-धीरे और बाद में संदंश के साथ फैलाएं, पेट के मिडलाइन की पहचान करें और ध्यान से दो सर्जिकल क्लिप लागू करें जो गैस्ट्रोसोफेगल जंक्शन से पेट के 15% मध्यीय खंड पर लागू होता है।क्लैंप और पेट के लगभग 75-80% को अलग करता है, जिससे पेट की पूरी पार्श्व आस्तीन बन जाता है। पार्श्व पृथक पेट को माइक्रोन कैंची से फिर से काटें। पेट से किसी भी अवशिष्ट भोजन को हटाने के लिए कपास झाड़ू का उपयोग करें और फिर पोविडोन-आयोडीन जलीय समाधान के साथ पेट के कटे हुए किनारे को साफ करें।कोई रिसाव के बारे में सुनिश्चित करने के लिए एक 5-0 मोनोफिलामेंट nonabsorbable सीवन के साथ क्लिप लाइन oversew । सीवन के सिरों को गाँठ और दोनों छोर पर क्लिप करने के लिए लंगर । पेट और आंत को पेट की गुहा में उचित स्थिति में लौटाएं और पेट की दीवार पर चलने वाले 5-0 मोनोफिलमेंट नॉनएबलब्बेबल सीवन के साथ पेट को बंद कर दें।एसजी की नकली प्रक्रिया के लिए, एक समान प्रक्रिया करें जैसा कि पहले मिडलाइन लेप्रोटॉमी के साथ वर्णित है, पांच मिनट के लिए 37 डिग्री सेल्सियस गीले गर्म नमकीन धुंध कवरेज का उपयोग करके आंत और पेट को बाहरी करें। पेट और आंत को इन आंतरिक अंगों की उचित साइटों पर वापस करें। दर्द नियंत्रण और संक्रमण की रोकथाम के लिए सर्जरी के बाद दो से पांच मिलीग्राम प्रति किलोग्राम और सेफाजोलिन 25 मिलीग्राम प्रति किलोग्राम चमड़े के नीचे या इंट्रापेरिटोनली प्रशासित करें।आइसोफ्लुनाणे को रोकें और माउस पूरी तरह से जागने तक तीन से पांच लीटर प्रति मिनट के कमरे हवा के प्रवाह के साथ जारी रखें। माउस देखते रहें, जबकि यह गतिशीलता हासिल करता है और पिंजरे के चारों ओर चलना शुरू कर देता है। पांच दिनों के लिए तापमान की स्थिति में 30 डिग्री सेल्सियस के आसपास स्वतंत्र इनक्यूबेटर में माउस रखें, सुनिश्चित करें कि पिंजरे प्रति केवल एक माउस है।हमारे अनुभवी हाथों में, एसजी की सफल दर सीखने की अवधि के बाद लगभग 90% थी। नकली संचालित और उच्च वसा वाले आहार खिलाया चूहों के शरीर के वजन तेजी से प्राप्त की । लेकिन आस्तीन गैस्ट्रेक्टॉमी और उच्च वसा वाले आहार खिलाया चूहों लगातार कमी आई।कोलेस्ट्रॉल के स्तर का अंतर उच्च वसा वाले आहार के बीच महत्वपूर्ण था चूहों को नकली और आस्तीन गैस्ट्रेक्टॉमी के साथ खिलाया गया था। यह इंगित कर सकता है कि आस्तीन गैस्ट्रेक्टॉमी के बाद ऑक्सीडेटिव तनाव में सुधार होता है। हीमोग्लोबिन A1C और IPGTT के स्तर का अंतर भी महत्वपूर्ण था ।उच्च वसा वाले आहार के बीच चूहों को नकली और आस्तीन गैस्ट्रेक्टॉमी के साथ खिलाया जाता है और आस्तीन गैस्ट्रेक्टॉमी के बाद इंसुलिन प्रतिरोध और चीनी नियंत्रण सुधार का संकेत देता है। यह पशु मॉडल हमें आस्तीन गैस्ट्रेक्टॉमी के साथ एक संक्षिप्त शल्य प्रक्रिया का उपयोग करने में मधुमेह और मोटापे की नैदानिक प्रासंगिकता की जांच करने में मदद कर सकता है। यह प्रक्रिया ऑपरेशन समय को छोटा कर सकती है और सर्जिकल सर्वाइवल रेट में सुधार कर सकती है।इस अध्ययन का मुख्य लाभ एक साधारण सर्जिकल माउस मॉडल का उपयोग करके मानव रोगियों में बैरिएट्रिक सर्जरी के नैदानिक परिणामों की नकल करना है।
