1. 放置前的评估

</ol>3. 温度调制 + 射频消融

</ol>5. 患者温度监测

最近介绍了一个随机对照临床试验中评估专用冷却装置的数据，并汇总在表121中。控制和治疗臂的消融参数分别为：RF持续时间14.1分和14.5分钟;平均力，19.1 和 17.8 g，最大射频功率，33.9 比 34.1 W，平均消融指数 394 比 384，所有差异均不显著。所有患者都有PVI与额外的病变集，当需要时。在演示时，两组之间发现6个月心房颤动复发率没有差异（对照组4/27，治疗组3/17）。
射频消融结果示例： 一名59岁的女性，有高脂血症、糖尿病和复发性心房颤动病史，为射频消融手术提供。在食道中放置了一个循环14°C的水的食管传热装置，在转射穿刺后，设定点降至4°C，在消融开始前约8分钟。消融使用三维映射系统和 3.5 mm 灌溉消融导管进行分段肺静脉隔离。在肺静脉后部使用30 W的设置，前静脉使用高达40 W，持续时间高达20s。 PVI以及线性后壁隔离（盒病变） 被执行。患者体温通过鼻咽探针测量，将鼻咽探针置于鼻孔中，患者起始温度为36.4°C，终温为36.1°C。后壁消融约20分钟后，食管传热装置设定点提高到40°C，为患者提供体温，同时拆下通道护套，完成血管闭合。内窥镜检查在第二天进行，作为研究协议的一部分，证明没有食管病变。
冷冻结果示例： 一名68岁的男性，有高血压病史，并且有越来越多的心房颤动发作，为冷冻气球消融而出现。食管传热装置循环室温（22°C）水被放置在食道。放置后，设定温度提高到 42 °C。消融是使用低温气球系统进行的。通过Foley导管温度传感器在36.3°C下测量初始患者核心温度。食道的温度使用单传感器温度探头测量（不建议使用与传热装置同处的温度探头设备，因为通过传热之间的完全接触获得最佳效益设备和食管粘喉，但在这里描述显示对防止温度过度下降的影响）。从左上肺静脉的低温化开始，初步食管温度测量为38.6°C，在冷冻过程中达到36.4°C的最低点。纳迪尔气球温度为-51°C。块是在30秒以下获得的，只进行了180秒的冻结。在左下部肺静脉，起始温度为38.5°C，经过两个疗疗周期后达到38.0°C的低温（由于初始冻结时延迟达到70s，因此进行了120s的加点冻结）。纳迪尔气球温度为-48°C。在右上肺静脉中，初始食管温度为38.4°C，通过两个周期保持不变，以38.5°C结束。纳迪尔气球温度为-47°C。最后，在右下肺静脉中，初始食管温度为38.9°C，在两个治疗周期中达到38.8°C的最低点。纳迪尔气球温度为-39°C。手术结束时患者体温为36.0°C，所有冷冻气球治疗均维持食管温度远高于普通停止阈值（15°C至25°C）。
