1. 配置前の評価

	<li>食道温度管理装置をデバイスコネクタを介して熱交換器に取り付け、ユニットの電源を入れ、手動モードにします。水が食道温度管理装置を通って流れていることを確認し、漏れがないことを確認してください。</li>
</ol>2. 配置
<ol><li>標準的な口管と同様の方法で食道温度管理装置に適した挿入深さを決定する。患者の唇から耳たぶまで、耳たぶからxiphoidプロセスまで測定し、デバイス上のこの深さに注意してください(図2)。</li>
	<li>水溶性潤滑剤を使用して、食道温度管理装置を寛大に、少なくとも15cm、および遠位端の25cmまで潤滑する(図3)。 注:患者は通常、一般的に一般的な吸入麻酔(例えば、セボフルランを使用)下にありますが、静脈内麻酔下(例えば、プロポフォールを使用)、または意識的な沈静(例えば、メペリジンまたはメペリジンまたは使用)の下で起こることもできますミダゾラム)。</li>

	<li>デバイスの先端がダイアフラムの下にあるかどうかを確認するために、透視検査による配置位置を決定します(図5)。</li>
	<li>誤って外れが出ないように、水ホースとデバイスを固定します。一般的な方法は、患者の左フォームアームレストの下に接続ホースを配置することです。</li>
	<li>胃の減圧が望ましい場合は、標準的な吸引管を使用して、中央の管腔を低間欠吸引に接続します。</li>
</ol>3. 温度変調 — RF アブレーション
<ol><li>熱交換器が手動モードに設定され、適切な水温が設定されていることを確認します。たとえば、一般的な熱交換器で[一時制御]ボタンを押してから、上矢印/下矢印を使用して目標水温を選択します。デジタルディスプレイに希望する目標温度が表示されたら、手動制御ボタンを押して水の流れを開始します。典型的な目標は、左心房壁の後部で無線周波アブレーションを行う場合の水温4°Cである。</li>
	<li>熱交換器が温度を下げるために必要な時間を予測するために、経皮穿刺を待つ間RFケースの初期挿入に約14°Cの水温セットポイントを使用してください。経中性穿刺後、RFエネルギーを心房後壁に塗布する前に約15~20分経過した後、水温設定点を4°C(手動モード)に変更します。 注:胃不全麻痺や胸痛後の手順を減らす可能性のある冷却の追加の抗炎症効果については、オペレータは後壁アブレーションの完了後20分間、4°Cの水温設定点を維持することができます。マシンをオフにすることができます。</li>
</ol>4. 温度変調 — クライオアブレーション

</ol>5. 患者の温度モニタリング
注意:食道の温度は食道伝熱装置の存在によって変調されるため、患者の温度測定には別の場所が必要です。患者の温度測定のための選択は、鼻咽頭温度計(深さが10cm未満であることを確認する)、フォーリー温度センサー、直腸温度センサー、鼓膜温度計、または額温度計(ゼロフラックスを含む)を含むサーモメトリー)。

</ol>6. トラブルシューティング
<ol><li>水の流れが発生せず、水のパドルホイールが存在する場合は、継続的に回転しているか、低流量アラームが作動していないことを確認します。</li>
	<li>システム内の水の流れの閉塞は、パドルホイールが回転を停止し、外部熱交換器の閉塞警報を発生させ、処理を停止し、障害物の位置と原因を特定します。必要に応じて、食道温度管理装置を取り外し、交換してください。</li>
	<li>適切な圧力(装置はしっかりしている)と適切な温度を確認するために、セットポイントと触れる装置を確認することによって、正しい温度での水の流れを確認します。</li>
</ol>7. デバイスの削除
<ol><li>適切なボタンを押して、水の流れを一時停止します。これは「モニター」または「一時セット」と表示されますが、モデルによって異なる場合があります。</li>
	<li>存在する場合は、ホースセットおよび/またはデバイスチューブ上のクランプを閉じ、標準的なオロ胃管除去と同様の方法で前眼を緩やかに引っ張ることによって患者から装置を引き出す。</li>

重要なステップには、(a)熱伝達装置(b)の適切な配置を確保する、適切な水温セットポイント、および(c)熱伝達装置を通して継続的な水循環を保証する。装置の適切な配置は、透視検査で容易に確認され、特に熱交換装置の先端が終了すると予想される上胃領域付近に注意を払う。水温は熱交換器コンソールで簡単に調整でき、循環水が開始温度から設定点温度を得るためには最大7〜10分が必要になる可能性があることを念頭に置いています。装置が適切に熱を伝達するためには、絶え間ない水循環が必要です。水循環は、一部の熱交換器モデルに存在する回転水流パドルホイールの可視化によって確認することができる。水流パドルホイールを備え付けた熱交換器モデルでは、流れが妨げられるとアラームがトリガーされます。水流閉塞の潜在的な原因は、熱交換装置の不適切な配置である(深く置き過ぎると、遠位胃の管の曲げ/キンキングを引き起こす場合、またはまれに、配置中に中咽頭または近位食道で巻き上げて曲がることを許される場合)。この場合のトラブルシューティングでは、蛍光検査の下で簡単に視覚化して配置レベルを決定し、必要に応じて調整します。

RFアブレーション中に冷たい液体を食道に直接注入して食道冷却を使用して多くの患者が研究されている(例えば、LETがベースライン以上0.5°C増加したときに、オロガス胃管を介して氷冷生理食塩水の20 mLボーラスを上食道に注入することによって)。この手法を用いた既存の研究のメタ分析の結果を図620にまとめておきます。
専用冷却装置を評価する無作為化制御臨床試験のデータが最近提示され、表121に要約されている。コントロールアームと治療アームのアブレーションパラメータはそれぞれ次のとおりです: RF期間、14.1対14.5分;平均力は、19.1対17.8g、最大RF電力、33.9対34.1W、平均アブレーション指数、394対384で、すべての差は有意ではありません。すべての患者は、必要に応じて追加の病変セットを有するPVIを有していた。発表時には、2群間に6ヶ月における心房細動の再発率の差は見つからなかった(対照群では4/27、治療群では3/17)。
RF アブレーションの結果の例: RFアブレーション処置のために提示された高脂血症、糖尿病、および再発性発作性心房細動の過去の病歴を有する59歳の女性。14°Cの水を循環する食道熱伝達装置を食道に入れ、セッティングポイントは経皮穿刺後4°C、アブレーション開始の約8分前に減少した。アブレーションは、3次元マッピングシステムと3.5mmの灌漑アブレーションカテーテルを使用して、分節性肺静脈分離のために行った。肺静脈の後部側面に30Wの設定を行い、前部に最大40Wを使用し、最大20s.PVIの持続時間と線形後壁分離(ボックス病変)を行った。患者温度は、ナレスに10cm未満に入れ、患者開始温度は36.4°C、終了温度は36.1°Cで鼻咽頭プローブを介して測定した。後壁のアブレーション終了後約20分、食道熱伝達装置の設定点を40°Cに引き上げて、アクセスシースを除去し、血管閉鎖を完了した間に患者の温暖化を提供した。内視鏡検査は、研究プロトコルの一部として翌日行われ、食道病変が示されていない。
凍結化結果の例: 高血圧の過去の病歴と、凍結バルーンアブレーションのために提示された発作性心房細動のエピソードを増加させる68歳の男性。食道内に循環液(22°C)の水を循環させる食道熱伝達装置を食道に入れた。一度置くと、設定点温度を42°Cに上げた。アブレーションは、クライオバルーンシステムで行われました。患者コア温度初期は、フォーリーカテーテル温度センサを介して36.3°Cで測定した。食道の温度は、単一センサ温度プローブで測定した(熱伝達装置と同じ位置にある温度プローブ装置の日常的な使用は、熱伝達間の完全な接触で得られるため、推奨されない装置および食道粘膜は、過大温度低下を防止する効果を示すためにここに記載されている。左上肺静脈の凍結凝結から始まり、最初の食道温度は38.6°Cで、凍結凝結中に36.4°Cのナディールに達した。ナディールバルーン温度は-51°Cであった。ブロックは30s以下で得られ、180秒の凍結を1回行った。左下肺静脈では、開始温度は38.5°Cで、2サイクルの治療後に38.0°Cの低値に達した(70sまでの初期凍結でブロックを得るのが遅れたため、120sのボーナス凍結が行われた)。ナディールバルーン温度は-48°Cであった。右上の肺静脈では、初期食道温度は38.4°Cであり、2サイクルを経て変化せずに、38.5°Cで終了した。ナディールバルーン温度は-47°Cであった。最後に、右下肺静脈では、初期食道温度は38.9°Cであり、治療の2サイクルを通して38.8°Cのナディールに達した。ナディールバルーン温度は-39°Cであった。処置終了時の患者温度は36.0°Cであり、すべての凍結バルーン治療は、一般的な停止閾値(15°C〜25°C)をはるかに上回る食道温度を維持した。
