再生医療は、加齢や病気、外傷などによって傷ついた組織や臓器を治癒・置換し、先天性欠損症を矯正することを目的としています。自家療法では、細胞または組織は患者から引き出され、拡張または修飾され、その後ドナーに再導入されます。この治療法は、皮膚科の分野で幅広い可能性を秘めています1。自家線維芽細胞療法では、患者の線維芽細胞を培養して再注入し、しわ、鼻水、またはにきびの傷跡を治療します。線維芽細胞は真皮の主要な機能細胞であるため、自家線維芽細胞の注射は、顔の若返りにおいて他の治療法よりも有益である可能性があります2。
皮膚では、線維芽細胞は細胞外タンパク質(すなわち、コラーゲン、エラスチン、ヒアルロン酸、およびグリコサミノグリカン)の合成と分泌に関与しています。また、正常な皮膚恒常性と創傷治癒において、細胞機能、遊走、細胞-マトリックス/細胞-細胞相互作用を調節する成長因子を放出します3。皮膚線維芽細胞は、皮膚創傷治癒4、組織再生5、または審美的および形成外科手術における皮膚充填剤6の潜在的な臨床細胞療法としてすでに導入されています。いくつかの研究は、再生医療の文脈では、線維芽細胞が間葉系幹細胞よりも実用的で効果的な細胞療法である可能性があることを示唆しています7。
臨床応用に十分な数の線維芽細胞を得るためには、通常、細胞増殖が必須である。Ex vivo/in vitro細胞培養には、細胞の接着と増殖をサポートするために、成長因子、タンパク質、酵素を添加した基礎培地が必要です。ウシ胎児血清(FBS)は、胎児の血液が成人の血液と比較して成長因子が豊富であり、2)抗体含有量が低いため、細胞培養培地の一般的なサプリメントです8。細胞治療が進むにつれて、FBSを培地に添加する古典的な細胞培養条件の安全性が懸念される。さらに、現在、FBSを代替案9に置き換える傾向があります。いくつかのFBS代替品は有望な結果を示しています10。
ここでは多血小板血漿(PRP)の代替品を選択し、CuteCell-PRPと名付けられたPRPの標準化された製剤を製造するための医療機器を開発しました。この装置の使用目的は、GMP条件下での自家細胞のin vitro増殖のための培地サプリメントとして使用される自家PRPの調製である。
PRPは、血漿中の濃縮血小板懸濁液として定義される。1)必要な血液量、2)使用するデバイスの種類、および3)遠心分離プロトコルが異なる多数の準備プロトコルがあるため、結果として得られる血小板濃度は、血液ベースライン値のわずかに上から10倍以上まで変化します。さらに、PRP製剤には、さまざまなレベルの赤血球および白血球汚染が含まれています。したがって、「PRP」という用語は、生物学的組成および潜在的な治療効果が大きく異なる製品を説明するために使用されます。
ほとんどの研究では、FBS置換は、(トロンビンまたはカルシウムによって)活性化される異なる濃度のPRPを使用して達成されます。この人工的な活性化は、血小板成長因子の即時かつ重要な放出を15分から24時間11まで誘発する。したがって、血小板活性化は、段階的な血小板脱顆粒からの成長因子の徐放が必要とされる細胞培養における用途には望ましくないと考えられている。
PRP療法には、濃縮血漿中の自家血小板の調製が含まれます12。最適な血小板濃度は不明であり、PRP13を調製するために幅広い商用デバイスが利用可能です。この標準化の欠如は、研究間の不一致に起因し、注射の投与量とタイミングに関するブラックボックスにつながっています。このプロトコルは、この専用PRPデバイスを使用して自家PRPを取得し、100%自家ex vivo培養モデルにおいて皮膚線維芽細胞を拡張する手順を説明する。
