Het beschreven circuit kan worden gebruikt om verschillende onderzoeksvragen te onderzoeken, bijvoorbeeld de rol van verschillende serumcomponenten en weefselbarrières bij de levering van geneesmiddelen, of immuun- en stamcelhandel. Verschillende systemen voor de levering van geneesmiddelen (bijvoorbeeld liposomen en nanodeeltjes) kunnen aan het perfusaat worden toegevoegd om de rol van fysiologische en biochemische factoren bij de levering te begrijpen. Geïsoleerde orgaanperfusie werd oorspronkelijk ontwikkeld om orgaanfysiologie te bestuderen voor transplantatie1,2,3, en maakte het mogelijk om het inzicht in functies van de organen te begrijpen zonder interferentie van andere lichaamssystemen. Geïsoleerde nier- en hartperfusie was bijvoorbeeld enorm nuttig bij het begrijpen van basisprincipes van hemodynamica en effecten van vasoactieve agentia, terwijl leverperfusie belangrijk was voor het begrijpen van de metabole functie, inclusief medicijnmetabolisme in gezond en ziek weefsel4,5,6,7. Bovendien waren perfusiestudies van cruciaal belang voor het begrijpen van de levensvatbaarheid en functie van organen die bestemd waren voor transplantatie. In Cancer Researchearch, geïsoleerde tumor perfusie is beschreven door verschillende groepen met behulp van muis, rat, en vers gereseceerde menselijke weefsels8,9. In sommige geïsoleerde tumorperfusie werd de tumor geïmplanteerd in het eierstokvetkussen om de groei van de tumor die bloedvaten levert van de mesenterieslagader10te forceren. De Jain-groep voerde baanbrekende studies uit met behulp van geïsoleerde perfusie van colonadenocarcinomen om tumorhemodynamica en metastase8,11,12,13te begrijpen . Andere innovatieve engineered ex vivo setups omvatten een 96-well plate-based perfusie apparaat om de primaire menselijke multiple myeloom cellen14 en een modulaire flow kamer voor engineering beenmerg architectuur en functie onderzoek15cultuur.
Naast fysiologie en pathologiestudies is orgaanperfusie gebruikt om de basisprincipes van de levering van geneesmiddelen te bestuderen. Zo beschreef een groep geïsoleerde rattenledemaatperfusie en bestudeerde hij de ophoping van liposomen in geïmplanteerde sarcoman16, terwijl een andere groep ontleede menselijke nierperfusie uitvoerde om de endotheel targeting van nanodeeltjes17te bestuderen. Ternullo et al. gebruikte een geïsoleerde geperfuseerde menselijke huidflap als een ingebouwde vivo huidmedicijn penetratiemodel18.
Ondanks deze vooruitgang in perfusie van grote organen en weefsels, zijn er geen rapporten over in situ perfusiemodellen in muizen die: a) klaringsorganen zoals lever, milt en nieren omzeilen; b) bekkenorganen, huid, spieren, voortplantingsorganen (bij mannen), blaas, prostaat en beenmerg omvatten. Vanwege de geringe omvang van deze organen en de leverende vasculatuur is ex vivo cannulation en de totstandbrenging van een perfusiecircuit niet haalbaar geweest. De muis is het belangrijkste diermodel in kanker en immunologie onderzoek, en de levering van geneesmiddelen. De mogelijkheid om kleine muisorganen te doornemen zou het mogelijk maken interessante vragen met betrekking tot de levering van geneesmiddelen aan deze organen, waaronder tumoren geïmplanteerd in het bekken (blaas, prostaat, eierstok, beenmerg), worden beantwoord, evenals studies van de fundamentele fysiologie en immunologie van ziekten van deze organen. Om dit tekort aan te pakken, ontwikkelden we een in situ perfusiecircuit bij muizen dat mogelijk weefselletsel kan voorkomen en veel beter geschikt is voor functioneel onderzoek dan geïsoleerde orgaanperfusie.

Ex vivo perfusie is een belangrijk fysiologisch hulpmiddel om de functie van geïsoleerde organen (bijvoorbeeld lever, nieren) te bestuderen. Tegelijkertijd is een uitdaging of niet haalbaar vanwege de kleine omvang van de muisorganen, ex vivo perfusie van bot, blaas, huid, prostaat en voortplantingsorganen. Hier rapporteren we voor het eerst een in situ lager lichaam perfusie circuit in muizen die de bovenstaande weefsels omvat, maar omzeilt de belangrijkste klaring organen (nier, lever, en milt). Het circuit wordt vastgesteld door het afzwmen van de abdominale aorta en inferieure vena cava boven de iliacale slagader en ader en cauterizing perifere bloedvaten. Perfusie wordt uitgevoerd via een peristaltische pomp met perfusate flow gehandhaafd voor maximaal 2 uur. In situ kleuring met fluorescerende lectine en Hoechst oplossing bevestigd dat de microvasculatuur met succes werd geperfuseerd. Dit muismodel kan een zeer nuttig hulpmiddel zijn voor het bestuderen van pathologische processen en mechanismen voor de levering van geneesmiddelen, migratie/metastase van circulerende tumorcellen in/van de tumor en interacties van het immuunsysteem met geperfundeerde organen en weefsels.

We hebben een gesloten circuit perfusiesysteem opgezet door cannulation van de abdominale aorta en de inferieure vena cava van 8-10 weken oude muizen terwijl het volume van de perfusiebuffer minder dan 10 mL blijft. Figuur 3A toont confocale beelden na het doorsbrengen van weefsels met Ringer's oplossing met Hoechst 33342 en DyLight 649-lectine. Dit protocol kan worden gebruikt om vragen te beantwoorden over de levering van geneesmiddelen en immuuninteracties binnen de organen die in ons circuit zijn opgenomen. Deze techniek zorgt voor de perfusie van organen die niet mogelijk zijn om door te nemen door middel van traditionele ex-vivo methoden, terwijl het vermijden van de belangrijkste klaring organen. Voordat u met de operatie begint, stelt u een circulerend waterbad en alle onderdelen met waterjas in op 37 graden Celsius.Bevestig dat de slang wordt gereinigd, en gebruik een bellenval in een vochtige kamer als het perfuseren reservoir om het perfuseren volume te beperken. Om de katheterisatie procedure uit te voeren, bevestig een gebrek aan reactie op pedaalreflex, in een acht tot 10 weken oude mannelijke verdoofde lamp zie muis. En plaats de muis in de supine positie op een piepschuim bord, met het hoofd op zes uur.Immobiliseer de dieren ledematen met tape, en veeg de buik met isopropyl alcohol. Maak een T-vormige incisie in de buik, met behulp van elektrostollulatie om eventuele bloedingen rond de rand van de incisie te stoppen. Duw de maag, jejunum en dikke darm aan de rechterkant van de buik, om de abdominale aorta, vena cava en gemeenschappelijke iliacale en iliolumbar slagaders en aders onthullen.Onder een dissectiemicroscoop, lokaliseren en ligate de eierstokken en iliolumbar slagaders en aders met 4-O zijden hechtingen. Loop twee 4-O zijde hechtingen onder de abdominale aorta en inferieure vena cava ongeveer een centimeter boven de iliacale slagader en ader, een millimeter uit elkaar. Maak dan een losse knik in de hechting die het dichtst bij de iliacale vaten ligt.Gebruik een naaldhouder om zowel de inferieure vena cava als de abdominale aorta horizontaal uit te lijnen en uit te rekken. En, het houden van de vaten uitgerekt, gebruik maken van een 24-gauge gevleugelde schild IV katheter om de buikaorta te punctie. Zodra de naald ongeveer een millimeter in het vat doordringt, drukt u de knop ingedrukt om de naaldkern in te trekken en steek de katheter ongeveer vijf millimeter in het vat.Steek een katheter vijf millimeter in de inferieure vena cava, op dezelfde manier. En bind beide hechtingen rond de katheteriserende vaten. Breng vervolgens direct lijm aan om de katheters te immobiliseren op de erector spin a, en vervang de buikorganen.Om het perfusiesysteem op te zetten, plaatst u de muis op een siliconenpad in de vochtige kamer met waterjas en immobiliseert u de kathetervleugels aan het pad met naalden met 19 meter. Vul de arteriële katheterinlaat, met voorverwarmde perfusiebuffer. Gebruik hemostatische tangen en een schroefconnector om de katheterinlaat aan de slang te bevestigen en de inlaatperfusieslang met tape te immobiliseren.Pas de peristaltische stroomsnelheid aan op 0,6 milliliter per minuut en houd de uitstroom perfusie vijf tot tien minuten open om het bloed door de veneuze katheter te wassen. Sommige stolsels zullen aanwezig zijn in de uitlaatkatheter, spoelen de stolsels met perfusate buffer, en gebruik een schroef op connector te hechten aan het einde van de veneuze katheter aan de uitlaat slang om het circuit te sluiten. Bedek vervolgens de vochtige kamer met een voorverwarmd deksel voor maximaal twee uur perfusie, periodiek controleren op het niveau van perfusate.Hier worden confocale beelden na perfusie met ringeroplossing met Hoechst 33342 en DyLight-649 lectine getoond. De spier, beenmerg, teelballen, blaas, prostaat en voethuid tonen meestal een efficiënte nucleaire en vasculaire kleuring. Hematoxylin en eosine kleuring van deze weefsels na drie uur van normale thermische perfusie, onthult een succesvolle blozen van de organen, zonder schijnbare weefselletsel.Dit model kan worden gebruikt om drugsopnametrajecten te begrijpen, in de fysiologische irrelevante omgeving. Bijvoorbeeld om de rol van serumcomponenten in medicijnaccumulatie te begrijpen. We gebruiken het systeem, om het mechanisme van liposoom accumulatie in de huid te begrijpen.
