في القرن الحادي والعشرين، يشكل السل المقاوم للأدوية كارثة متطورة بالنسبة للبرامج الوطنية الضعيفة بالفعل لمكافحة السل، حيث تضاعفت الحالات المؤكدة في السنوات الخمس الماضية وحدها، وهو ما يمثل ما يقرب من ثلث جميع الوفيات المرتبطة بمقاومة مضادات الميكروبات على الصعيد العالمي1،,2. وقد تطلب العلاج الناجح للسل المقاوم للأدوية تقليدياً أنظمة الخط الثاني الأطول سمية من علاج السل الحساس للأدوية. وعلاوة على ذلك، غالباً ما يواجه المرضى المصابون بالسل المقاوم للأدوية تحديات كبيرة قائمة من قبل للتمسك، مما ساهم في ظهور المقاومة في البداية3.
على عكس عدوى فيروس نقص المناعة البشرية حيث يمكن استخدام الأحمال الفيروسية لمراقبة العلاج، يتم تأخير نقاط النهاية البديلة للاستجابة العلاجية في السل ولا يمكن الاعتماد عليها على المستوى الفردي4. كما أن مراقبة التزام المرضى، وهو مؤشر هام على تركيز الأدوية المضادة للسل دون العلاجية وفشل العلاج، أمر صعب أيضاً. الالتزام الذاتي المبلغ عنها يعاني من التحيز استدعاء والرغبة في إرضاء مقدمي5,6. يمكن أن يكون عدد حبوب منع الحمل وأنظمة مراقبة الأحداث الدوائية (MEMS) أكثر موضوعية7 ولكن لا تقيس الاستهلاك الفعلي للأدوية8،9،10. يمكن أن توفر مستويات الدواء في المصفوفات الحيوية كل من الالتزام والبيانات الدوائية. لذلك ، تستخدم مستويات أدوية البلازما عادة في مراقبة الأدوية العلاجية11،12. غير أنه في سياق رصد التقيد بالمخدرات، تمثل مستويات البلازما التعرض القصير الأجل وتحد منها تفاوتات كبيرة داخل المريض وفيما بين المرضى عند تحديد النطاق المرجعي المناسب للتقيد. آثار "معطف أبيض"، حيث يتحسن الالتزام قبل العيادة أو زيارات الدراسة، يزيد من تعقيد قدرة مستويات البلازما لتوفير أنماط دقيقة الالتزام المخدرات13.
الشعر هو مصفوفة بيولوجية بديلة يمكن قياس التعرض للأدوية على المدى الطويل14،15. العديد من الأدوية والأيض الذاتي دمج في مصفوفة بروتين الشعر من الدورة الدموية الجهازية كما ينمو الشعر. كما تستمر هذه العملية الديناميكية خلال نمو الشعر، كمية الدواء المودعة في مصفوفة الشعر يعتمد على وجود مستمر للدواء في الدورة الدموية، مما يجعل الشعر قراءة زمنية ممتازة من تناول المخدرات. الشعر كمصفوفة حيوية لديه ميزة إضافية من يجري جمعها بسهولة دون الحاجة إلى سلسلة التبريد للتخزين والشحن مقارنة بالدم. وعلاوة على ذلك، الشعر غير خطرة بيولوجيا، والتي توفر مزايا جدوى إضافية في هذا المجال.
وقد استخدمت مستويات المخدرات الشعر منذ فترة طويلة في تطبيقات الطب الشرعي16. وعلى مدى العقد الماضي، أثبتت مستويات مضادات الفيروسات العكوسة للشعر فائدة في تقييم مدى التزام هاوية المخدرات في علاج فيروس نقص المناعة البشرية والوقاية منه، وهو ما أسهمت فيه مجموعتنا. وقد ثبت أن مستويات ARV في الشعر لتكون أقوى المؤشرات المستقلة لنتائج العلاج في عدوى فيروس نقص المناعة البشرية17,18,1919,20,21. لتحديد ما إذا كانت مستويات الشعر من مرضى السل المقاوم للأدوية سيكون لها نفس الأداة المساعدة في التنبؤ بنتائج العلاج، استخدمنا LC-MS/MS لتطوير والتحقق من صحة طريقة لتحليل 11 دواء ً من أدوية السل المقاوم للأدوية في عينات الشعر الصغيرة. كتقييم أولي لأداء القياس ، قمنا بقياس مستويات أدوية السل المقاوم للأدوية في عينة مريحة من المرضى الذين يعانون من السل المقاوم للأدوية الذين يتلقون العلاج الملاحظة مباشرة (DOT) في كيب الغربية ، جنوب أفريقيا22.
