Wij bieden een protocol voor een muismodel van onvolledige chirurgische resectie van weke delen sarcoom om peri-operatieve therapieën te testen. We hebben ook gestandaardiseerd de chirurgische incisie om beoordeling van wondgenezing tussen muizen na de behandeling mogelijk te maken.

Chirurgie blijft de belangrijkste behandelingsoptie voor veel vaste tumoren1, waaronder weke delen sarcoom2,3. Ondanks verbeteringen in kankerchirurgietechnieken en combinaties met (neo)adjuvante therapieën, is er nog steeds een hoog risico op kankerreval en metastase na primaire tumorresectie4,5. In weke delen sarcoom, recidieven optreden bijzonder locoregionaal, op de plaats van de operatie, wat resulteert in een verhoogde morbiditeit en mortaliteit. In de klinische setting kan het moeilijk zijn om ruim genoeg marges te verkrijgen (bijvoorbeeld vanwege anatomische beperkingen), wat resulteert in onvolledige resectie en daaropvolgende tumorrecidief6. Chirurgische stress en het daaropvolgende proces van wondgenezing zijn bekend om een immunosuppressieve tumor micro-omgeving gunstig voor tumor recidief7,8te creëren. Daarom moet de ontdekking en ontwikkeling van nieuwe therapieën voor weke delen sarcoom, met name immunotherapieën, idealiter rekening houden met de chirurgische wondgenezingsrespons.
De meeste preklinische studies voor adjuvante therapieën worden in eerste instantie uitgevoerd met behulp van onderhuidse syngenetische of xenotransplantatie muismodellen, zonder integratie van de chirurgische stress en wondgenezing respons9,10. Daarom ontwikkelden we een syngeneic onderhuids muiskecotweefsel sarcoom model waarin onvolledige chirurgische resectie. WEHI 164 fibrosarcoomcellen worden onderhuids ingeënt, en zodra tumoren zijn vastgesteld, verwijderen we 50-75% van de tumorbulk(figuur 1A-E). Tumoren consequent opnieuw groeien van de resterende tumor. Dit model maakt het mogelijk voor het testen van adjuvante therapieën, terwijl rekening houdend met het effect van chirurgische stress en wondgenezing. Soortgelijke chirurgische modellen van onvolledige resectie zijn in een aantal studies door verschillende groepen gebruikt en reproduceerbaar en effectiefbevonden 11,12,13. Hier geven we een gedetailleerde beschrijving van dit protocol.

Chirurgie is vaak de eerste behandeling voor veel vaste tumoren. Echter, lokale recidieven optreden vaak na primaire tumor resectie, ondanks adjuvante of neo-adjuvante therapieën. Dit gebeurt wanneer chirurgische marges onvoldoende tumorvrij zijn, wat resulteert in resterende kankercellen. Vanuit biologisch en immunologisch perspectief is chirurgie geen nulgebeurtenis; de wondhelende omgeving is bekend dat zowel pro- als anti-tumorigenische paden induceren. Als gevolg hiervan moeten preklinische modellen voor medicijnontwikkeling die gericht zijn op het voorkomen van lokale terugval chirurgische resectie bevatten bij het testen van nieuwe (neo)adjuvante therapieën, om de klinische instellingen te modelleren bij patiënten die met een operatie worden behandeld.
Hier beschrijven we een muismodel van onvolledige chirurgische resectie van WEHI 164 weke delen sarcoom waarmee (neo)adjuvante therapieën kunnen worden getest in de setting van een wondgenezingsrespons. In dit model wordt 50% of 75% van de tumor verwijderd, waardoor er in situ wat kankerweefsel achterblijft om een bruto restziekte te modelleren na een operatie in de klinische setting. Dit model maakt het mogelijk om therapieën te testen in de context van een operatie, terwijl ook rekening wordt gehouden met de wondgenezingsrespons, die de werkzaamheid van (neo)adjuvante behandelingen kan beïnvloeden. De onvolledige chirurgische resectie resulteert in reproduceerbare hergroei van de tumor bij alle muizen bij afwezigheid van adjuvante therapie. Adjuvante behandeling met checkpoint blokkade resulteert in verminderde tumor hergroei. Dit model is dus geschikt voor het testen van therapieën in het kader van debulking chirurgie en de bijbehorende wondgenezing reactie en kan worden uitgebreid tot andere vormen van vaste kanker.

Tumorgroei tot een grootte van 50 mm2 is een ideale grootte voor gedeeltelijke debulk. De onvolledige chirurgische resectie van 50 mm2 tumoren resulteert in 100% (n=5) reproduceerbare hergroei van de tumoren bij afwezigheid van adjuvante immunotherapie (figuur 4A). Dit protocol is bedoeld om de klinische setting van restziekte na te bootsen na tumordebulking chirurgie, en stelt ons daarom in staat om nieuwe kankertherapieën te testen in het kader van postoperatieve wondgenezing. Het voordeel van deze techniek is dat het standaardisatie van de hoeveelheid toegebrachte wond en de hoeveelheid tumor toestaat om te blijven. Deze techniek is vooral belangrijk voor preklinische studies in die identificatie van nieuwe behandelingen voor kanker, dat het lokaal een voorbeeld van is weke delen sarcoom.Deze visuele demonstratie is van cruciaal belang om de inversietechniek aan te tonen, waardoor we de tumor aan de huidkant kunnen blootstellen. Begin deze procedure met de onderhuidse inenting van voorbereide kankercellen bij muizen, zoals beschreven in het tekstprotocol. Op dag 12, post-inenting, wanneer tumoren een grootte van ongeveer 50 vierkante millimeter hebben bereikt, dosis muizen met 100 microliter buprenorfine onderhuids in de scruff van de nek, 30 minuten voor de operatie.Zet het chirurgische gebied met het warmtekussen bedekt met banklaag en zet een neuskegel op voor anesthesie. Schik schone chirurgische apparatuur, zodat ze binnen handbereik, met inbegrip van chlorhexadine, wattenstaafje, gaas, ooggel, twee gebogen tangen, schaar, clip applicator, clip remover, en clip vullingen. Verwarm de verwarmingskamer tot 37 graden Celsius en zet een ander warmtekussen op voor herstel.Houd er rekening mee dat deze procedure twee personen vereist. Een assistent moet de ademhaling van de muis tijdens de procedure nauwlettend in de gaten houden. Na het verdoven van de muis zoals beschreven in het tekstprotocol, voert u een pinch-test en hoornvliesreflextest uit om ervoor te zorgen dat de muis volledig verdoofd is voordat u met een operatie begint.Bedek de ogen van de muis met een kleine hoeveelheid oogheelkundige gel om oogdroogte te voorkomen. Begin deze operatie door het chirurgische gebied te laten uitstrijkjes met chlorhexadine. Met behulp van tangen en een schaar maken een een centimeter rechte incisie langs de rugkant, drie millimeter afstand van de tumor.Snijd de huid niet rechtstreeks die de tumor bedekt, omdat deze niet goed geneest. Vermijd het gebruik van een scalpel om de tumor te snijden en gebruik pincet om de stukken te scheppen. Met behulp van pincet, weg te trekken de fascia en onderhuids vetweefsel tussen de tumor en buikvlies.De onderhuidse tumor is normaal bevestigd aan de huidzijde. Open de wond door de huid voorzichtig op de tumorlagerkant te houden met behulp van een pincet en de tumor om te keren, zodat deze buiten zichtbaar is. Met behulp van een schaar, snijd de tumor capsule uit de helft te verwijderen, te beginnen vanaf de basis van de tumor het dichtst bij de opening.Voor 50% debulk operatie, dwars door het midden van de tumor. Met behulp van gebogen tang schep het gedeelte van de tumor te verwijderen. Schep alle restanten uit het debulkgebied op.Voor 75% debulk, beginnen met het uitvoeren van een 50%tumor debulk dan gesneden in de helft van de resterende 50% van de tumor. Schep 25% van de tumor op met gebogen tangen. Om de chirurgische site te sluiten, plaats de resterende tumor terug onder de huid en met behulp van tangen, trek de huid flappen samen en line-up van de huid langs de wond.Houd de huid vijf millimeter van de rand van de wond bij elkaar en gebruik chirurgische clips om de wond te sluiten die aan de zijkant begint, het dichtst bij de tangen. Breng zoveel clips aan als nodig is om ervoor te zorgen dat er geen onderliggend weefsel wordt blootgesteld. Over het algemeen worden drie tot vier clips toegepast met twee millimeter tussen de clips.Laat de muizen herstellen door ze in de warme verwarmingskamer te plaatsen. Plaats de kooi van de muis op het warmtekussen. Controleer de muizen in de verwarmingskamer totdat ze hersteld zijn van de verdoving.Zet vervolgens de muizen terug in de kooi, laat de kooi nog 10 minuten op het heatpad staan totdat de muizen actiever zijn geworden. Geef de muizen nat en zacht voedsel. Controleer de muizen een uur na de operatie voor herstel.Controleer de muizen aan het eind van de dag en de volgende ochtend opnieuw en zorg ervoor dat de clips op hun plaats blijven. Behandel muizen perioperitief met advent of neoadjuvante therapie op een bepaald moment, afhankelijk van de behandeling van belang. Behandel bijvoorbeeld muizen met één dosis van 100 microgram anti-CTLA-4 intraperitoneally op dag 15 na inenting.U de muizen ook behandelen met drie doses van 200 microgram anti-PD-1 intraperitoneally op dagen, 15, 17 en 19 na inenting. De onvolledige chirurgische resectie van 50 vierkante millimeter tumoren, hier aangegeven door de stippellijn resulteert in 100% reproduceerbare hergroei van de tumoren bij afwezigheid van adjuvante immunotherapie. Daarom is tumorgroei tot een grootte van 50 vierkante millimeter een ideale grootte voor gedeeltelijke debulk.Het model werd vervolgens gebruikt om adjuvante immunotherapieën te testen met behulp van antilichamen tegen checkpointmodules, cytoxisch T-lymfatisch geassocieerd eiwit vier en geprogrammeerde doodsreceptor één. Behandeling van muizen met anti-CTLA-4, resulteerde in een genezingspercentage van 80%, Terwijl de behandeling van muizen met anti-PD-1 resulteerde in een genezingspercentage van 25%De respons met anti-PD-1 biedt een kans om nieuwe combinaties te testen, om de respons verder te verbeteren. Het is belangrijk dat de resterende tumor aan de huid blijft vastzitten, anders wordt de bloedtoevoer afgesneden en wordt de tumor de necrotische.Na deze procedure kunnen tumoren worden geanalyseerd door verschillende methoden, zoals stroomcytometrie om te beoordelen hoe chirurgie tumorinfiltrerende cellen beïnvloedt. Dit muismodel heeft ons in staat gesteld om te onderzoeken hoe wondgenezing gevolgen heeft voor kankerimmunotherapieën.
