O tecido cicatricial representa o ponto final da reparação tecidual. No entanto, em muitos indivíduos, especialmente aqueles que sofrem de queimaduras outraumas 1, cicatrizes podem ser excessivas e impor efeitos indesejáveis sobre a morfologia e o funcionamento da pele curada. Embora os mecanismos exatos da formação de cicatrizes patológicas (hipertróficas e quelóides) não sejam totalmente compreendidos, a deposição excessiva de colágeno durante a cicatrização de feridas tem sido demonstrada como uma contribuição essencial2.
Está bem estabelecido que a transformação do fator de crescimento beta 1 (TGF-β1) e a actina muscular alfa lisa (αSMA) desempenham papéis-chave na formação de cicatrizes hipertróficas. As evidências sugerem que o TGF-β1 elevado estimula diretamente a deposição excessiva de colágeno através da regulação da via de sinalização SMAD3. Além disso, verificou-se que αSMA contribui para a formação de cicatrizes hipertróficas, regulando a contração celular e a reepitelalização no processo de cicatrização da ferida4. A falta de modelos in vitro e in vivo adequados é um grande impedimento para o desenvolvimento e avaliação de intervenções e terapias para remediação de cicatrizes. O objetivo deste estudo é utilizar a técnica existente de MMC para construir um modelo de cicatriz hipertrófica "in vivo" adequado para avaliar intervenções novas e emergentes relacionadas à cicatriz.
Reproduzir tecido vivo fora do corpo tem sido uma meta há anos na comunidade científica. O desenvolvimento de técnicas in vitro no início do século XX atingiu parcialmente esse objetivo. As técnicas in vitro atuais evoluíram ligeiramente da demonstração original de Roux de que as células embrionárias podem sobreviver ex vivo por vários dias em soro quente5. No entanto, as metodologias in vitro são limitadas principalmente a tipos de células únicas cultivadas em 2-D e não recapitularem tecidos in vivo com precisão. Embora úteis para examinar a bioquímica celular, fisiologia e genética, os tecidos nativos são 3D e incorporam vários tipos de células. Sistemas in vitro simples 2-D submetem células mamíferas a ambientes altamente artificiais nos quais a arquitetura nativa específica do tecido é perdida6. Por sua vez, isso afeta eventos intracelulares e extracelulares, resultando em morfologia celular anormal, fisiologia e comportamento7.
O interesse por trás desse protocolo está no desenvolvimento e manejo clínico de cicatrizes hipertróficas e quelóides. Embora esteja bem estabelecido que os fibroblastos dérmicos são em grande parte responsáveis pela produção abundante de colágenos presentes no tecido cicatricial, o cultivo de fibroblastos dérmicos utilizando sistemas in vitro 2-D não consegue reproduzir o volume de negócios de colágeno observado in vivo8. Métodos in vitro contemporâneos ainda usam essencialmente "soro fisiológico quente", um ambiente completamente diferente daquele dos tecidos vivos. Os tecidos in vivo são extremamente aglomerados, com proteínas, ácidos nucleicos, ribonucleoproteínas e polissacarídeos, ocupando 5%-40% do volume total. Como nenhuma molécula pode ocupar o mesmo espaço ao mesmo tempo, há pouco espaço livre disponível e uma ausência quase completa de água livre9.
A técnica mmc impõe restrições que afetam as propriedades termodinâmicas de citostes e fluidos intersticiais. Interações moleculares, complexos de sinalização receptor-ligantes, enzimas e organelas estão confinadas e restritas de interagir livremente9. As interações dentro do ambiente pericelular (ou seja, interstício) também são restritas. Evidências recentes confirmam que altas concentrações de macromoléculas inertes em soluções lotadas perturbam a difusão, associação física, viscosidade e propriedades hidrodinâmicas10.
Curiosamente, vários agentes populares de aglomeração (ou seja, Ficoll, dextran, polivinilpirrorolido [PVP], e sódio 4-estirenosulfonato [PSS]) não são equivalentes quando aplicados a diferentes tipos de células e em diferentes configurações. Em um estudo anterior, Ficoll foi relatado como menos citotóxico para células-tronco mesenquimais em comparação com PVP. Esses resultados foram interpretados como conseqüência de sua carga neutra e do raio hidrodinâmico relativamente pequeno11. Em contraste, um segundo estudo descobriu que dextran é mais eficaz em estimular a deposição de colágeno I por fibroblastos pulmonares humanos em comparação com Ficoll12. Dados de nosso próprio estudo sugerem que Ficoll aumenta a deposição de colágeno por fibroblastos hipertróficos derivados de cicatrizes, enquanto pvp é tóxico para essas células13.
Foi demonstrado que a conversão de procolágeno para colágeno é mais rápida em um ambiente in vivo altamente lotado14, enquanto a taxa de reações biológicas é retardada em um sistema de cultura 2D diluído15. Otimizamos o protocolo in vitro aqui, incorporando o MMC para mostrar o cultivo de fibroblastos dérmicos servindo como um modelo mais "in vivo" para fibrose dérmica e formação de cicatrizes. Em contraste com o sistema de cultura 2D comum, cultivar hHSFs com MMC estimula significativamente a biossíntese e a deposição do colágeno13. Notavelmente, outras características da fibrose (ou seja, aumento da expressão das metaloproteinases matriciais [MMPs] e citocinas proinflamatórias) também são evidentes sob este protocolo otimizado de McMc13. Quando cultivadas usando este método, mostra-se que as células dérmicas recapitulam os parâmetros fisiológicos, bioquímicos e funcionais medidos in vivo.
O protocolo in vitro otimizado do MMC tem sido utilizado para avaliar a expressão de colágeno e outras proteínas de ECM por fibroblastos dérmicos isolados da derme de cicatriz hipertrófica e derme adjacente não envolvido. Quando cultivados em ambientes de MMC in vitro, observou-se que os HHSFs expressam certas características (ou seja, mRNA, bioquímica, fisiologia e fenótipo) semelhantes ao tecido cicatrizhipertrófico dérmico in vivo. As evidências indicam que propriedades físicas e químicas são considerações importantes ao selecionar crowders e otimizar as condições de MMC para cultivo in vitro.
Para a prova de princípio, o protocolo MMC é aplicado aqui para avaliar qualitativamente e quantitativamente a capacidade de Shikonin e seus análogos para induzir a apoptose. Isso permite avaliar as aplicações potenciais desses compostos de Medicina Tradicional Chinesa (TCM) naturalmente derivados para o gerenciamento de cicatrizes dérmicas13. Não obstante, a simplicidade, a eficácia e a pontualidade deste protocolo In vitro MMC também satisfaz regulamentos recentes para eliminar a experimentação em mamíferos pela Diretiva DA UE 2010/63/UE e pela Agência de Proteção Ambiental dos EUA (EPA).

Foi demonstrado que os tecidos in vivo são altamente lotados por proteínas, ácidos nucleicos, ribonucleoproteínas, polissacarídeos, etc. O protocolo a seguir aplica uma técnica de aglomeração macromolecular (MMC) para imitar essa aglomeração fisiológica através da adição de polímeros neutros (crowders) às culturas celulares in vitro. Estudos anteriores usando Ficoll ou dextran como crowders demonstram que a expressão de colágeno I e fibronectina em linhas celulares WI38 e WS-1 são significativamente melhoradas usando a técnica MMC. No entanto, essa técnica não foi validada em fibroblastos de pele humana derivados de cicatrizes hipertróficas primárias (hHSFs). Como a cicatriz hipertrófica surge da deposição excessiva de colágeno, este protocolo visa construir um modelo de cicatriz hipertrófica in vitro rico em colágeno, aplicando a técnica mmc com hHSFs. Este modelo otimizado de MMC tem mostrado possuir mais semelhanças com o tecido in vivo cicatriz em comparação com os sistemas tradicionais de cultura celular bidimensional (2-D). Além disso, é econômico, econômico e eticamente desejável em comparação com modelos animais. Portanto, o modelo otimizado aqui relatado oferece um modelo avançado "in vivo" para estudos hipertróficos relacionados à cicatriz.

Amostras triplicadas foram realizadas em cada experimento, e cada experimento foi repetido 3x usando células de três pacientes individuais. Os dados são expressos como percentuais do grupo controle. O teste pós-hoc de ANOVA e Tukey foi aplicado para analisar diferenças estatísticas (*p. 0,05).

Criamos um modelo individualmente rico em colágeno para imitar o ambiente que as células experimentam in vivo usando a técnica de aglomeração macromolecular. A pele é um ambiente lotado, ao contrário do ambiente líquido que já usamos para cultivar células in vitro. Comparado ao sistema tradicional de cultura celular monocamada, este modelo recria o ambiente molecular lotado que as células experimentam in vivo, especialmente em tecidos cicatricias onde a abundante matriz extracelular desloca a água líquida.Além das cicatrizes hipertróficas, essa técnica de aglomeração macromolecular pode ser usada para estudos onde a matriz extracelular é abundante, como a fibrose pulmonar. Cresça fibroblastos derivados da cicatriz e normais de acordo com as instruções do manuscrito. Em seguida, semeá-los em 24 placas de poço a 50.000 células por poço em um mililitro de médio.Incubar a placa durante a noite a 37 graus Celsius e 5% de dióxido de carbono. Prepare a aglomeração macromolecular ou a mídia MMC misturando Ficoll 70, Ficoll 400 e ácido ascórbico em DMEM 10% FCS. Incubar a mídia em um banho de água para dispersar os crowders na solução.Em seguida, esterilize-o com um filtro de 0,2 micrômetro. Aspire a mídia gasta dos fibroblastos e substitua-a por mídia MMC recém-feita. Incubar as células a 37 graus Celsius durante seis dias, mudando a mídia MMC a cada três dias.Prepare uma solução Sirius Red dissolvendo 0,2 gramas de pó Direct Red 80 em 200 mililitros de água destilada desomada com um mililitro de ácido acético. Aspire a mídia MMC das células e adicione 300 microliters de solução Sirius Red a cada poço. Adicione 200 microliters de anticorpo secundário anti-coelho-fitc de cabra e DAPI a cada poço.Cubra a placa com papel alumínio e incuba-a por 30 minutos em temperatura ambiente. Descarte o anticorpo secundário e o DAPI e repita as lavagens com PBS. Em seguida, visualize a fluorescência diretamente sob um microscópio.Depois de lavar as células duas vezes com PBS, adicione 40 microliters de tampão de lise em cada poço e raspe a camada celular com uma ponta de pipeta. Em seguida, transfira o lise de proteína em tubos de micro centrífugas para medir a concentração proteica. Carregue 10 microgramas de proteína em cada poço de um gel de proteína Bis-Tris de quatro a 12% e execute SDS-PAGE a 200 volts por 30 minutos.Quando terminar, transfira a proteína para uma membrana de nitrocelulose, executando uma mancha ocidental a 90 volts por 90 minutos, tomando o cuidado de evitar a formação de bolhas entre o gel e a membrana. Bloqueie a membrana com 10 mililitros de tampão de bloqueio. Em seguida, use um kit de síntese cDNA para realizar uma síntese de DNA de primeira linha. Misture o cDNA com 10 microlitros do SYBR Green Supermix e transfira as amostras para uma placa personalizada de 96 poços pré-revestida com primers oligonucleotídeos.Ajuste o volume total para 20 microliters por poço com água e execute RT-PCR de acordo com as instruções do manuscrito. A densidade celular de fibroblastos de pele humana derivados da cicatriz hipertrófica aumentou significativamente após a cultura com Ficoll em comparação com o uso de PVP ou o controle. A aglomeração macromolecular com Ficoll a 9% de ocupação de volume fracionado aumentou significativamente a deposição de colágeno, incluindo colágeno um, em comparação com outras formulações.Isso foi demonstrado ainda com a análise quantitativa. Fibroblastos derivados da cicatriz e fibroblastos dérmicos normais cultivados em ambientes MMC foram encontrados para regular a expressão de espécies de ECM, além do colágeno. Notavelmente, uma expressão elevada de metaloproteínas matricial ou MMPs foi encontrada para se acumular em tecidos cicatricicos hipertróficos em comparação com tecidos nativos.Observou-se que a expressão de MMP2, nove e 13 foram significativamente reguladas em ambas as culturas celulares cultivadas em condições de aglomeração macromolecular. Os MMPs desempenham um papel importante durante a cicatrização da ferida e da formação de cicatrizes, regulando a montagem e a remodelação do ECM. A síntese de interleucina seis e fator de crescimento endotelial vascular foi indetectável em uma mancha ocidental.Mas a análise do RT-PCR revelou que a expressão da interleucina seis foi significativamente regulada, enquanto a expressão do fator de crescimento endotelial vascular foi regulada em fibroblastos cultivados em condições de MMC. Ao tentar este protocolo, é muito importante escolher fibroblastos dérmicos que tenham muito poucos patógenos de CO. Também temos que ter em mente que o ácido ascórbico é um ingrediente essencial no meio MMC.Estamos particularmente interessados em detectar ainda mais a deposição e alinhamento do colágeno neste modelo MMC. Planejamos usar técnicas microscópicas avançadas para comparar este modelo in vitro com tecidos de cicatriz nativos in vivo. Encorajamos outros a usar essa técnica de aglomeração macromolecular para melhorar a autenticidade in vitro dos tecidos modais auxiliares.É claro que a aglomeração macromolecular também pode ser usada para recalcular modelos de doenças e patologia in vitro.
