De belangrijkste drijfveer achter de ontwikkeling van dit keratitismodel met behulp van ex vivo varkensmaïs is om onderzoekers die nieuwe antimicrobiële stoffen ontwikkelen een representatief in vitro model te bieden om de antimicrobiële werkzaamheid in de preklinische stadia nauwkeuriger te bepalen. Hoornvliesinfecties zijn belangrijke oorzaken van blindheid en komen voor in epidemische proporties in lage- en middeninkomenslanden. De etiologie van de ziekte varieert van regio tot regio, maar bacteriën zijn verantwoordelijk voor een grote meerderheid van deze gevallen. Pseudomonas aeruginosa is een belangrijke ziekteverwekker die een snel progressieve ziekte veroorzaakt. In veel gevallen blijven patiënten achter met stromale littekens, onregelmatig astigmatisme, moeten ze worden getransplanteerd of verliezen ze in het ergste geval een oog1,2.
Bacteriële keratitis veroorzaakt door P. aeruginosa is een moeilijke ooginfectie om te behandelen, vooral vanwege de toenemende opkomst van antimicrobiële resistente stammen van P. aeruginosa. In het afgelopen decennium is gebleken dat het testen en ontwikkelen van nieuwe behandelingen voor hoornvliesinfecties in het algemeen, en die veroorzaakt door Pseudomonas sp., in het bijzonder, essentieel zijn om de huidige trend in antibioticaresistentie te bestrijden3.
Voor het testen van de werkzaamheid van nieuwe behandelingen voor hoornvliesinfecties zijn conventionele in vitro microbiologische methoden een slecht surrogaat vanwege het verschil in bacteriële fysiologie tijdens laboratoriumkweek en tijdens infecties in vivo en vanwege het ontbreken van de gastheerinterface4,5. In vivo diermodellen zijn echter duur, tijdrovend, kunnen slechts een klein aantal replica's opleveren en zorgen oproepen over dierenwelzijn.
In dit artikel demonstreren we een eenvoudig en reproduceerbaar organotypisch ex vivo varkensmodel van keratitis dat kan worden gebruikt om verschillende behandelingen voor acute en chronische infecties te testen. We hebben P. aeruginosa gebruikt voor dit experiment, maar het model werkt ook goed samen met andere bacteriën en organismen zoals schimmels en gist die keratitis veroorzaken.

Bij de ontwikkeling van nieuwe antimicrobiële stoffen is het succes van dierproeven afhankelijk van nauwkeurige extrapolatie van antimicrobiële werkzaamheid van in vitro tests naar dierinfecties in vivo. De bestaande in vitro tests overschatten doorgaans de antimicrobiële werkzaamheid omdat de aanwezigheid van gastheerweefsel als diffusiebarrière niet wordt verantwoord. Om dit knelpunt te overwinnen, hebben we een ex vivo varkens corneamodel van bacteriële keratitis ontwikkeld met Pseudomonas aeruginosa als prototypisch organisme. Dit artikel beschrijft de voorbereiding van het varkensvlies en het protocol voor de vaststelling van de infectie. Op maat gemaakte glasvormen maken een eenvoudige installatie van het hoornvlies mogelijk voor infectiestudies. Het model bootst in vivo infectie na omdat bacteriële proliferatie afhankelijk is van het vermogen van de bacterie om hoornvliesweefsel te beschadigen. De vaststelling van de infectie wordt geverifieerd als een toename van het aantal kolonievormende eenheden dat via levensvatbare plaattellingen wordt beoordeeld. De resultaten tonen aan dat infectie op een zeer reproduceerbare manier kan worden vastgesteld in de ex vivo hoornvliezen met behulp van de hier beschreven methode. Het model kan in de toekomst worden uitgebreid om keratitis na te bootsen die wordt veroorzaakt door andere micro-organismen dan P. aeruginosa. Het uiteindelijke doel van het model is om het effect van antimicrobiële chemotherapie op de voortgang van bacteriële infectie te onderzoeken in een scenario dat meer representatief is voor in vivo infecties. Hierdoor zal het hier beschreven model het gebruik van dieren voor tests verminderen, de slagingspercentages in klinische proeven verbeteren en uiteindelijk een snelle vertaling van nieuwe antimicrobiële stoffen naar de kliniek mogelijk maken.

Het ontwerp van de glasvormen is een innovatief en origineel idee, waarvan het gebruik ons in staat stelde om het model op een consistente manier op te zetten met minimale / geen problemen met vervuiling. De mallen werden bereid door een glasblazer aan de Universiteit van Sheffield op basis van een ontwerp (Figuur 1A).
